The association between radiological spreading pattern and clinical outcomes in necrotizing external otitis

Objectives: Necrotizing external otitis(NEO) is a rare infectious disease of the skull base. The purpose of this study was to determine whether clinical outcomes of NEO can be correlated to different infectious spread ***: Retrospective chart review from 2010 to 2019 with NEO patients, who were divided into two cohorts: single spreading patterns(group A) or complex spreading patterns(group B) as diagnosed by *** symptoms, diagnostic and treatment delay, course of disease, complications, and duration of antibiotic exposure were retrospectively collected from patient ***: 41 NEO patients were included, of which 27 patients belonged to group A(66%). The diseaserelated mortality rate was 12.2% among the entire cohort, no differences were found between group A and B. Higher rates of ***(42.9% vs 14.8% P = 0.047) and N. IX palsies were found in group B compared to group A(28.6% vs 3.7%, P = 0.039). The median duration of antibiotic use was significantly different for a complex spreading pattern, clinical recovery and hospitalizations. Complications were associated with higher diagnostic delay and with a complex spread pattern. The median duration of follow-up was 12.0(IQR 6.0-19.5) ***: NEO is a severe disease, with significant mortality and morbidity(cranial nerve palsies). The radiological spread pattern may assist in predicting clinical outcome. Furthermore, complex spread patterns are associated with higher rates of clinical nerve palsies(N. VII and ***), complications, surgery rates and longer duration of antibiotic use. Diagnostic delay was associated with mortality, complications and facial palsies.

Research to date indicates that the number of coronary artery bypass graft （CABG） surgery patients affected by depression （i.e., major, minor, dysthymia） approximates between 30% and 40% of all cases. A longstanding empirical interest on psychosocial factors in CABG surgery patients highlights an association with increased risk of morbidity in the short and longer term. Recent evidence suggests that both depression and anxiety increase the risk for mortality and morbidity after CABG surgery independent of medical factors, although the behavioral and biological mechanisms are poorly understood. Though neither depression nor anxiety seem to markedly affect neuropsy- chological dysfunction, depression confers a risk for incident delirium. Following a comprehensive overview of recent literature, practical advice is described for clinicians taking into consideration possible screening aids to improve recognition of anxiety and depression among CABG surgery patients. An overview of contemporary interventions and randomized, controlled trials are described, along with suggestions for future CABG surgery research.

We consider the design of an effective and reliable adaptive finite element method(AFEM)for the nonlinear Poisson-Boltzmann equation(PBE).We first examine the two-term regularization technique for the continuous problem recently proposed by Chen,Holst and Xu based on the removal of the singular electrostatic potential inside biomolecules;this technique made possible the development of the first complete solution and approximation theory for the Poisson-Boltzmann equation,the first provably convergent discretization and also allowed for the development of a provably convergent ***,in practical implementation,this two-term regularization exhibits numerical ***,we examine a variation of this regularization technique which can be shown to be less susceptible to such *** establish a priori estimates and other basic results for the continuous regularized problem,as well as for Galerkin finite element *** show that the new approach produces regularized continuous and discrete problemswith the samemathematical advantages of the original *** then design an AFEM scheme for the new regularized problem and show that the resulting AFEM scheme is accurate and reliable,by proving a contraction result for the *** result,which is one of the first results of this type for nonlinear elliptic problems,is based on using continuous and discrete a priori L¥*** provide a high-quality geometric model as input to the AFEM algorithm,we also describe a class of feature-preserving adaptive mesh generation algorithms designed specifically for constructing meshes of biomolecular structures,based on the intrinsic local structure tensor of the molecular *** of the algorithms described in the article are implemented in the Finite Element Toolkit(FETK),developed and maintained at *** stability advantages of the new regularization scheme are demonstrated with FETK through comparisons with the original regula

Cardiac specific overexpression of human β 2 adrenergic receptor (β 2 AR) in transgenic mice (TG4) enhances the basal cardiac function due to a ligand independent,spontaneous β 2 AR ***,agonist mediated stimulation of either β 1 AR or β 2 AR fails to induce contractile response in this transgenic *** the unresponsiveness of β 2 AR has been attributed to an efficient coupling of the receptor to pertussis toxin (PTX) sensitive G proteins (G i),mechanisms underlying the loss of β 1 AR response is *** the present study,we demonstrated that,unlike β 2 AR,β 1 AR contractile response was not restored by PTX treatment,although G i protein abundance was significantly elevated in TG4 relative to non transgenic ***,the loss of β 1 AR function was not attributable to β adrenergic receptor kinase (βARK) mediated receptor desensitization,as evidenced by the unaltered βARK1 activity in TG4 *** contrast,preincubation of cells with a muscarinic receptor agonist,carbachol (CCh),or a β 2 AR inverse agonist,ICI 118,551 (ICI),largely restored the contractile response to β 1 AR stimulation by norepinephrine plus an α 1 adrenergic antagonist,*** cases were associated with a significant reduction in the baseline cAMP accumulation and contractility,and the rescued β 1 AR positive inotropic effect was completely reversed by a β 1 AR antagonist,CGP ***,we conclude that overexpression of β 2 AR in the moure heart causes a loss of β 1 AR contractile response,likely through a cAMP/protein kinase A(PKA) dependent heterologous receptor desensitization,independent of G i protein or β*** of the spontaneous β 2 AR activation by either muscarinic stimulation or the β 2 AR inverse agonist ICI can resensitize the β 1 AR contractile respinse in TG4 ventricular myocytes.

Contact with environmental microbes are arguably the most common species interaction in which any animal *** have noted diverse relationships between hosts and resident microbes,which can have strong consequences for host development,physiology,and behavior. Many of these studies focus specifically on pathogens or beneficial microbes,while the benign microbes,of which the majority of bacteria could be described,are often ***,we explore the nature of the relationships between the grass spider Agelenopsis pennsylvanica and bacteria collected from their cuticles in ***,using culture-based methods,we identified a portion of the cuticular bacterial communities that are naturally associated with these ***,we topically exposed spiders to a subset of these bacterial monocultures to estimate how bacterial exposure may alter 3 host behavioral traits:boldness,aggressiveness,and activity *** conducted these behavioral assays 3 times before and 3 times after topical application,and compared the changes observed in each trait with spiders that were exposed to a sterile control *** identified 9 species of bacteria from the cuticles of 36 spiders and exposed groups of 20 spiders to 1 of 4 species of cuticular *** found that exposure to Dermacoccus nishinomiyaensis and Staphylococcus saprophyticus was associated with a lO-fold decrease in the foraging aggressiveness of spiders toward prey in their *** bacterial exposure did not have survival consequences for hosts,these data suggest that interactions with cuticular bacteria,even non- pathogenic bacteria,could alter host behavior.

Cross-flow analysis of injection wells in a multilayered reservoir

During fluid injection into a multilayered reservoir,a different pressure gradient is generated across the face of each permeable *** pressure gradient generates driving forces in the wellbore during well shut-in that causes the injected fluid moves from higher pressure layers to lower pressure layers,a phenomenon known as interwell ***-flow behavior depends on the initial pressure in the permeable layers and may be referred to as natural cross-flow(identical or natural initial pressures)and forced cross-flow(different initial pressures because of exploitation).Cross-flow may induce sand production and liquefaction in the higher pressure layers as well as formation damage,filter cake build-up and permeability reduction in the lower pressure ***,understanding cross-flow during well shut-in is important from a production and reservoir engineering perspective,particularly in unconsolidated or poorly consolidated sandstone *** and forced cross-flow is modeled for some injection wells in an oil reservoir located at North *** solution uses a transient implicit finite difference approach for multiple sand layers with different permeabilities separated by impermeable shale *** and forced cross-flow rates for each reservoir layer during shut-in are calculated and compared with different production logging tool(PLT)*** appears that forced cross-flow is usually more prolonged and subject to a higher flow rate when compared with natural cross-flow,and is thus worthy of more detailed analysis.
