Background:Pen-pal clubs(PPC)are used worldwide for students to learn about different cultures and other skillsets without the need for *** medical students are interested in global health opportunities abroad but costs,scheduling,and other barriers allow few to participate in such *** is important that medical students have nuanced global medical perspectives and can contribute to the global medical ***:The purpose of this study is to demonstrate that an international medical student PPC improves medical students'perspectives of cultural competency and global health ***:In 2021,a novel medical student PPC was established that began between an American and japanese medical *** a shareholders meeting,it was decided that the number of medical schools involved globally be expanded through previous institutional affiliations and online *** total,the club connected 50 American medical students and 52 medical students from 17 high-and middle-income *** primary form of communication was online;pen-pals were encouraged to communicate monthly using provided topics,although frequency and way of communication was their *** February 2022,American PPC members were emailed a qualitative survey to assess the PPC's ***:The survey was completed by 42%of American PPC members,95%of which were 22-26 *** were preclinical medical students,60%whom were female and the majority either white(47%)or Asian(43%).Overall,the PPC positively influenced American medical students'perception of global medicine,medical education,and their cultural competency after joining the PPC compared to prior(P=0.004).Conclusion:PPCs encourage medical students to think from a global perspective and foster open-mindedness within varying social and cultural *** a global communication platform for students during medical school education may be an additional way to train aspiring global l

The optimum parameters for the generation of synchrotron radiation in ultraintense laser pulse interactions with planar foils are investigated with the application of Bayesian optimization,via Gaussian process regression,to 2D particle-incell *** properties of the synchrotron emission,such as the yield,are maximized,and simultaneous mitigation of bremsstrahlung emission is achieved with multi-variate objective *** angle-of-incidence of the laser pulse onto the target is shown to strongly influence the synchrotron yield and angular profile,with oblique incidence producing the optimal *** is further explored in 3D simulations,in which additional control of the spatial profile of synchrotron emission is demonstrated by varying the polarization of the laser *** results demonstrate the utility of applying a machine learning-based optimization approach and provide new insights into the physics of radiation generation in laser-foil interactions,which will inform the design of experiments in the quantum electrodynamics(QED)-plasma regime.

Context Lipid-lowering agents are widely prescribed in the United *** estimates of rhabdomyolysis risk with various lipid-lowering agents are not available. Objective To estimate the incidence of rhabdomyolysis in patients treated with different statins and fibrates,alone and in combination,in the ambulatory setting. Design,Setting,and Patients Drug-specific inception cohorts of statin and fibrate users were established using claims data from 11 managed care health plans across the United *** with at least 180 days of prior health plan enrollment were entered into the cohorts between january 1,1998,and june 30,***-time was classified as monotherapy or combined statin-fibrate therapy. Main Outcome Measure Incidence rates of rhabdomyolysis per 10000 person-years of treatment,number needed to treat,and relative risk of rhabdomyolysis. Results In 252460 patients treated with lipid-lowering agents,24 cases of hospitalized rhabdomyolysis occurred during *** incidence per 10000 person-years for monotherapy with atorvastatin,pravastatin,or simvastatin was 0.44(95% confidence interval[CI],0.20-0.84);for cerivastatin,5.34(95%CI,1.46-13.68);and for fibrate,2.82(95%CI,0.58-8.24).By comparison,the incidence during unexposed person-time was 0(95%CI,0-0.48;P=.056).The incidence increased to 5.98 (95%CI,0.72-216.0) for combined therapy of atorvastatin,pravastatin,or simvastatin with a fibrate,and to 1035(95%CI,389-2117) for combined cerivastatin-fibrate *** year of therapy,the number needed to treat to observe 1 case of rhabdomyolysis was 22727 for statin monotherapy,484 for older patients with diabetes mellitus who were treated with both a statin and fibrate,and ranged from 9.7 to 12.7 for patients who were treated with cerivastatin plus fibrate. Amyloidosis is commonly systemic,occasionally organlimited,and rarely a solitary localized *** latter,commonly referred to as tumoral amyloidosis,is described as occurring in nearly every organ/*** a few reports of gastric amyloidosis exist *** describe a 72 year-old black male from Barbados presenting with 3 d of diffuse abdominal *** medical history included Non-Hodgkin's Lymphoma diagnosed five years ago,status-post six rounds of cyclophosphamide,adriamycin,vincristine,prednisone chemotherapy,and currently was in *** computed tomography scan of the abdomen,thickening and calcification of the gastric wall was noted along with *** esophagogastroduodenoscopy,a large circumferential friable mass was seen from the gastroesophageal junction to the body.A large nonbleeding 3 cm polyp was also seen in post bulbar area of *** were stained with Congo red and gave green birefringence under polarized light,consistent with tumoral *** emission tomography scan revealed diffuse gastric mucosa uptake compatible with gastric malignancy without metastatic *** for gastric amyloidomas has presently been one of observation or,at most,resection of the amyloid *** is not known if our patient required the same approach or if this warranted the re-institutionof chemotherapy for Non-Hodgkin's *** more reports of tumoral amyloidosis are made known,treatment as well as prognosis remain uncertain.
