The role of intravenous glutamine administration in critical care patients with acute kidney injury: a narrative review

The kidneys are complex organs responsible for waste removal and various regulatory functions. Critically ill patients often experience acute kidney injury (AKI). Although renal replacement therapy is used to manage AKI, nutritional therapy is crucial. Glutamine, an amino acid involved in cellular functions, has potential benefits when administered intravenously to critically ill patients. This administration is associated with reduced mortality rates, infectious complications, and hospitalization duration. However, its use in patients with AKI remains controversial. Glutamine is used by various organs, including the kidneys, and its metabolism affects several important pathways. Intravenous glutamine supplementation at specific doses can improve blood marker levels and restore plasma glutamine concentrations. Moreover, this supplementation reduces infections, enhances immune responses, decreases disease severity scores, and reduces complications in critically ill patients. However, caution is advised in patients with multiple organ failure, particularly AKI, as high doses of glutamine may increase mortality rates. Hyperglutaminemia can have adverse effects. Monitoring and appropriate dosing can help to mitigate these risks. Kidneys rely on glutamine for various essential functions. Thus, the use of intravenous glutamine in critically ill patients with AKI remains controversial. Despite its potential benefits in terms of infection reduction, immunomodulation, and improved outcomes, careful consideration of the patient's condition, dosage, and treatment duration is necessary. further research is needed to establish optimal guidelines for glutamine administration in this patient population. © Wolters Kluwer Health, Inc. In recent years, advances in microfabrication technology and tissue engineering have propelled the development of a novel drug screening and disease modelling platform known as organoid-on-a-chip. This platform integrates organoids and organ-on-a-chip technologies, emerging as a promising approach for in vitro modelling of human organ physiology. Organoid-on-a-chip devices leverage microfluidic systems to simulate the physiological microenvironment of specific organs, offering a more dynamic and flexible setting that can mimic a more comprehensive human biological context. However, the lack of functional vasculature has remained a significant challenge in this technology. Vascularisation is crucial for the long-term culture and in vitro modelling of organoids, holding important implications for drug development and personalised medical approaches. This review provides an overview of research progress in developing vascularised organoid-on-a-chip models, addressing methods for in vitro vascularisation and advancements in vascularised organoids. The aim is to serve as a reference for future endeavors in constructing fully functional vascularised organoid-on-a-chip platforms. © 2024, Chinese Medical Multimedia Press Co Ltd. Investigation on the influence of the initial RDX crystal size on the performance of shaped charge warheads

Intelligent Machine Learning Based Brain Tumor Segmentation through Multi-Layer Hybrid U-Net with CNN feature Integration

Brain tumors are a pressing public health concern, characterized by their high mortality and morbidity ***, the manual segmentation of brain tumors remains a laborious and error-prone task, necessitatingthe development of more precise and efficient methodologies. To address this formidable challenge, we proposean advanced approach for segmenting brain tumorMagnetic Resonance Imaging (MRI) images that harnesses theformidable capabilities of deep learning and convolutional neural networks (CNNs). While CNN-based methodshave displayed promise in the realm of brain tumor segmentation, the intricate nature of these tumors, markedby irregular shapes, varying sizes, uneven distribution, and limited available data, poses substantial obstacles toachieving accurate semantic segmentation. In our study, we introduce a pioneering Hybrid U-Net framework thatseamlessly integrates the U-Net and CNN architectures to surmount these challenges. Our proposed approachencompasses preprocessing steps that enhance image visualization, a customized layered U-Net model tailoredfor precise segmentation, and the inclusion of dropout layers to mitigate overfitting during the training ***, we leverage the CNN mechanism to exploit contextual information within brain tumorMRI images,resulting in a substantial enhancement in segmentation *** experimental results attest to the exceptionalperformance of our framework, with accuracy rates surpassing 97% across diverse datasets, showcasing therobustness and effectiveness of our approach. furthermore, we conduct a comprehensive assessment of ourmethod’s capabilities by evaluating various performance measures, including the sensitivity, jaccard-index, andspecificity. Compressive property and energy absorption characteristic of interconnected porous Mg-Zn-Y alloys with adjusting Y addition

In this study,interconnected porous Mg-2Zn-xY alloys with different phase compositions were prepared by various Y additions(x=0.4,3,and 6 wt.%)to adjust the compressive properties and energy absorption *** characterization methods were then applied to identify the microstructure of the porous Mg-Zn-Y and describe the details of the second *** tests were performed at room temperature(RT),200℃,and 300℃to study the impact of the Y addition and testing temperature on the compressive properties of the porous *** experimental results showed that a high Y content promotes a microstructure refinement and increases the volume fraction of the second *** the Y content increases,different Mg-Zn-Y ternary phases appear:I-phase(Mg_(3)Zn_(6)Y),W-phase(Mg_(3)Zn_(3)Y_(2)),and LPSO phase(Mg_(12)ZnY).When the Y content ranges between 0.4%and 6%,the compressive strength increases from 6.30MPa to 9.23 MPa,and the energy absorption capacity increases from 7.33 Mj/m^(3)to 10.97 Mj/m^(3)at RT,which is mainly attributed to the phase composition and volume fraction of the second ***,the average energy absorption efficiency is independent of the Y *** addition,the compressive deformation behaviors of the porous Mg-Zn-Y are altered by the testing *** compressive strength and energy absorption capacity of the porous Mg-Zn-Y decrease due to the softening effect of the high temperature on the *** deformation behaviors at different temperatures are finally observed to reflect the failure mechanisms of the struts.
