Accuracy of endoscopic ultrasound-guided tissue acquisition in the evaluation of lymph nodes enlargement in the absence of on-site pathologist

To evaluate factors that influence the diagnostic accuracy of endoscopic ultrasound (EUS)-guided tissue acquisition for lymph node enlargement in the absence of an on-site pathologist. METHODSA retrospective analysis of patients who underwent EUS-guided tissue acquisition for the pathological diagnosis of lymph node enlargement between April 2012 and june 2015 is reported. Tissue acquisition was performed with both cytology and biopsy needles of different calibers. The variables evaluated were lymph node location and size, number of passes and type of needle used. Final diagnosis was based on surgical histopathology or, in non-operated cases, on EUS-guided tissue acquisition and imaging assessment with a minimum clinical follow-up of 6 mo. RESULTSDuring the study period, 168 lymph nodes with a median size of 20.3 mm (range 12.5-27) were sampled from 152 patients. Ninety lymph nodes (53.6%) were located at mediastinum, and 105 (62.5%) were acquired with biopsy needles. The final diagnosis was benign/reactive origin in 87 cases (51.8%), malignant in 65 cases (38.7%), and lymphoma in 16 cases (9.5%). The sensitivity, specificity, positive predictive value and negative predictive value for the detection of malignancy were 74.1%, 100%, 100% and 80.6%, respectively. The overall accuracy was 87.5% (95%CI: 81.7-91.7). No variables were independently associated with a correct final diagnosis according to the multivariate analysis. CONCLUSIONEUS-guided tissue acquisition is a highly accurate technique for assessing lymph node enlargement. None of the variables evaluated were associated with diagnostic accuracy.

The Category of Humanity: Language as a Reflection of the Opposition between the Natural Sphere and the Human Sphere

The article analyses the way in which human languages classify reality into two large categories: the natural sphere and the human sphere. Thus, a given language will use the resources of its different linguistic levels(phonetic-phonological, morphological, syntactic, and lexical) to configure the references of reality and classify them based on what the speaker considers as belonging to the sphere of human society or the sphere of nature, outwith the human community. The same object, animal, phenomenon, or event may be treated(and evaluated) differently depending on whether the speakers of a given language consider it as belonging to the human sphere or the natural sphere.

Adverse hepatic reactions associated with calcium carbimide and disulfiram therapy: Is there still a role for these drugs?

Disulfiram and calcium carbimide are two alcohol deterrants widely used in alcoholism treatment, however, there exist great concerns over their safety. Reports on hepatotoxicity, mainly related to disulfiram therapy, have been published. The hepatotoxic potential of calcium carbimide is less well characterized. Here, we describe four cases of liver damage related to this therapeutic group that were submitted to a Registry of hepatotoxicity and point out the limitations that we face when prescribing these compounds. A reassessment of the role of these compounds in the management of alcohol dependence is clearly needed.

Purpose： The main goal of this study is to discover the scientific evolution of Cancer-Related Symptoms in Complementary and Alternative Medicine research area, analyzing the articles indexed in the Web of Science database from 1980 to ***/Methodology/Approach： A co-word science mapping analysis is performed under a longitudinal framework（1980 to 2013）. The documental corpus is divided into two subperiods,1980–2008 and 2009–2013. Thus, the performance and impact rates, and conceptual evolution of the research field are ***： According to the results, the co-word analysis allows us to identify 12 main thematic areas in this emerging research field： anxiety, survivors and palliative care,meditation, treatment, symptoms and cancer types, postmenopause, cancer pain, low back pain, herbal medicine, children, depression and insomnia, inflammation mediators, and lymphedema. The different research lines are identified according to the main thematic areas,centered fundamentally on anxiety and suffering prevention. The scientific community can use this information to identify where the interest is focused and make decisions in different *** limitation： Several limitations can be addressed： 1） some of the Complementary and Alternative Medicine therapies may not have been included; 2） only the documents indexed in Web of Science are analyzed; and 3） the thematic areas detected could change if another dataset was *** implications： The results obtained in the present study could be considered as an evidence-based framework in which future studies could be ***/value： Currently, there are no studies that show the thematic evolution of this research area.

Flavor violating Higgs signals in the Texturized Two-Higgs Doublet Model (THDM-Tx)

Flavor violating Higgs signals, such as the top FCNC decay t→ch0 and the LFV Higgs decay h0→τμ, have been studied at the LHC. These signals can arise within the general Two-Higgs Doublet Model （THDM）, where each Higgs doublet couples to all fermion types through Yukawa matrices Yf 1 and Y f2 . The Yukawa matrices can be assumed to have the same form or they could have different structures. In this paper we study the case when both Yf1 and Y f2 have completely different forms, but in such a way that they complement to produce a specific hermitian mass matrix. We find that for specific four-zero textures, the flavor violating Higgs couplings depend only on the free parameters tan β, γf and the fermion masses. We use the current bounds on the low energy processes to derive constraints on the heavy Higgs boson mass, tanβand γf Then, we use these constraints to evaluate the LFV Higgs decays, which reach branching ratios that could be tested at the LHC.

Rapid on-site evaluation of endoscopic-ultrasound-guided fine-needle aspiration diagnosis of pancreatic masses

Endoscopic ultrasound (EUS) has become an essential tool for the study of pancreatic diseases. Specifically, EUS plays a pivotal role evaluating patients with a known or suspected pancreatic mass. In this setting, differential diagnosis remains a clinical challenge. EUS-guided fine-needle aspiration (FNA) and fine-needle biopsy (FNB) have been proven to be safe and useful tools in this setting. EUS-guided FNA and FNB, by obtaining cytological and/or histological samples, are able to diagnose pancreatic lesions with high sensitivity and specificity. In this context, several methodological features, trying to increase the diagnostic yield of EUS-guided FNA and FNB, have been evaluated. In this review, we focus on the role of rapid on-site evaluation (ROSE). From data reported in the literature, ROSE may increase diagnostic yield of EUS-FNA specimens by 10%-30%, and thus, diagnostic accuracy. However, we should point out that many recent studies have reported adequacy rates of > 90% without ROSE, indicating that, perhaps, at high-volume centers, ROSE may not be indispensable to achieve excellent results. The use of ROSE can be considered important during the learning curve of EUS-FNA, and also in hospital with diagnostic accuracy rates < 90%.

In this study,the role of twin-twin interactions on the distributions of local defects(e.g.,dislocations)and stress fields in a magnesium alloy is investigated.A co-zone(1012)-(1012)tensile twin junction in a deformed Mg-3wt.%Y alloy is analyzed using transmission electron microscopy(TEM).The results show that the morphology of the impinging(1012)twin is asymmetric,and the non-interacting boundary of the recipient(1012)twin is *** analysis of TEM images reveals that type-II pyramidal[1213](1212)dislocations concentrate in the vicinity of the twin-twin junction *** samedislocations are also observed inside the interacting twin domains along with a few ***dislocations emanating from the impinging(1012)twin boundary have edge character and are extended with faults parallel to the basal *** contrast,thedislocations connected to the recipient(1012)twin are predominantly screw orientation and ***-viscoplastic fast Fourier transform based crystal plasticity calculations are performed to rationalize the observed twin morphology and local dislocation *** model calculations suggest that the local stress fields generated at the junction site where the two twins meet are responsible for the experimentally observed concentration of*** calculated stress fields are asymmetric with respect to the junction site,explaining the observed asymmetric morphology of the impinging ***,these findings show strong effects of twin-twin interactions on the distribution of dislocations as well as the evolution of the twinned microstructure and as such,can help advance understanding of twinning in Mg alloys and their effect on mechanical behavior.

Variations of OCT measurements corrected for the magnification effect according to axial length and refractive error in children

Purpose:The aim of this paper was to examine the distribution of macular,retinal nerve fiber layer(RNFL)thickness and optic disc parameters of myopic and hyperopic eyes in comparison withemmetropic control eyes and to investigate their variation according to axial length(AL)andspherical equivalent(SE)in healthy ***:This study included 293 pairs of eyes of 293children(145 boys and 148 girks),ranging in age from 6 to 17 years,Subjects were dividedaccording to SE in control(emmetropia,99 children),myopia(100 children)and hyperopia(94children)groups and according to axial AL in 68 short(25.00 mm,36).Macular parameters,RNFL thickness and opticdisc morphology were assessed by the Cirrus^(TM) *** was measured using the ***'s formula was used for calculating the corrected AL-related ocular *** Mean age(±SD)was 10.84±3.05 years;mean(±SD)SE was+0.14±0.51 D(range from8.75 to+8.25 D)and mean AL(±SD)was 23.12±*** RNFL thickness,averagemacular thickness and macular volume decreased as AL and myopia *** correlationsbetween AL/SE and optic disc parameters were found after correcting for magnification ***:AL and refractive error affect measurements of macular and RNFL thickness inhealthy *** make a correct interpretation of ocT measurements,ocular magnificationeffect should be taken into account by clinicians or OCT manufacturers.
