Single-operator cholangioscopy for biliary complications in liver transplant recipients

To evaluate cholangioscopy in addition to endoscopic retrograde cholangiopancreatography (ERCP) for management of biliary complications after liver transplantation (LT).METHODSTwenty-six LT recipients with duct-to-duct biliary reconstruction who underwent ERCP for suspected biliary complications between April and December 2016 at the university hospital of Muenster were consecutively enrolled in this observational study. After evaluating bile ducts using fluoroscopy, cholangioscopy using a modern digital single-operator cholangioscopy system (SpyGlass DS™) was performed during the same procedure with patients under conscious sedation. All patients received peri-interventional antibiotic prophylaxis and bile was collected during the intervention for microbial analysis and for antibiotic susceptibility ***-three biliary complications were found in a total of 22 patients, whereas four patients showed normal bile ducts. Anastomotic strictures were evident in 14 (53.8%) patients, non-anastomotic strictures in seven (26.9%), biliary cast in three (11.5%), and stones in six (23.1%). A benefit of cholangioscopy was seen in 12 (46.2%) patients. In four of them, cholangioscopy was crucial for selective guidewire placement prior to planned intervention. In six patients, biliary cast and/or stones failed to be diagnosed by ERCP and were only detectable through cholangioscopy. In one case, a bile duct ulcer due to fungal infection was diagnosed by cholangioscopy. In another case, signs of bile duct inflammation caused by acute cholangitis were evident. One patient developed post-interventional cholangitis. No further procedure-related complications occurred. Thirty-seven isolates were found in bile. Sixteen of these were gram-positive (43.2%), 12 (32.4%) were gram-negative bacteria, and Candida species accounted for 24.3% of all isolated microorganisms. Background: Forest management faces a climate induced shift in growth potential and increasing current and emerging new risks. Vulnerability analysis provides decision support based on projections of natural resources taking risks and uncertainties into account. In this paper we(1) characterize differences in forest dynamics under three management scenarios,(2) analyse the effects of the three scenarios on two risk factors, windthrow and drought stress, and(3) quantify the effects and the amount of uncertainty arising from climate projections on height increment and drought ***: In four regions in northern Germany, we apply three contrasting management scenarios and project forest development under climate change until 2070. Three climate runs(minimum, median, maximum) based on the emission scenario RCP 8.5 control the site-sensitive forest growth functions. The minimum and maximum climate run define the range of prospective climate ***: The projections of different management regimes until 2070 show the diverging medium-term effects of thinnings and harvests and long-term effects of species conversion on a regional scale. Examples of windthrow vulnerability and drought stress reveal how adaptation measures depend on the applied management path and the decision-maker’s risk attitude. Uncertainty analysis shows the increasing variability of drought risk projections with time. Canopy openings have a vital role in forest structure,regeneration,and plant *** this study,we investigated and compared the species composition and densities of seedlings and saplings between canopy gaps(openings)and forest understories(in dense canopies)in a sub-tropical forest of *** objectively identified 42 canopy openings between transect lines and 42 main plots in dense canopies,sampled for regeneration and young tree *** the regeneration study,we placed 2 m×2 m four subplots in each canopy opening and the main plot of dense canopy,thus making a total of 336 *** species diversity of seedlings,saplings,and trees were significantly(P≤0.05)higher in the dense canopy than in the canopy ***,most dominant and frequent regenerating species such as Diospyros pilosula,Garcinia cowa,Brownlowia elata,and Lithocarpus polystachya were common in both dense canopy and openings,the canopy openings had the highest Importance Value Index,suggesting the significance of gap dynamics in the dominance of native floral *** dense canopy played an important role in forest compositions because 12 regenerating species,including Stereospermum suaveolens and Diospyros montana,were not found in the canopy openings,implying that rain forests must maintain a high canopy cover for regeneration.

The crystal morphology grown from a solution composed of an organic solvent, solute and additive can be predicted reliably by a computational method. Modeling the supersaturated solution as liquid phase is achieved by employing commercial software. The molecular composition of this solution is a required input parameter. The face specific diffusion coefficient of the solid （crystal surface） and liquid （solution） system is determined using the molecular dynamics procedure. The obtained diffusion coefficient is related to the specific face growth rate via the attachment energy of the pure morphology. The significant improvements are achieved in the morphology prediction because the investigation on the face growth rates in a complex growth environment （as multi-component solutions with additives） can be carried out based on the diffusion coefficients.

Characterizing the thermal properties of MoS2 is important for the design of electronic devices based on this material. We used frequency domain thermoreflectance to study the cross-plane thermal transport in mechanically exfoliated MoS2 samples supported on SiO2 and muscovite mica substrates. The thickness of MoS2 ranged between one and five layers, and the MoS2 layers were sandwiched between a metal layer and the substrate. In the case of mica, heat transport into the substrate remained the same whether or not a monolayer of MoS2 was present, whereas, for SiO2, heat transport was reduced by surface roughness. We observed a significant improvement in heat transport across monolayer MoS2 as compared to few layer MoS2. For MoS2 on SiO2, the effective thermal interface conductance was improved by more than three times if a monolayer was used. For MoS2 on mica, the thermal interface conductance was approximately two times better for monolayer MoS2. This implies that monolayer MoS2 has superior thermal properties and can be used in electronic devices as an intermediate layer between two materials. Additionally, we also report on the measurement of anisotropic thermal conductivity in bulk MoS2 and mica.
