Early cisternal fibrinolysis is more effective than rescue spasmolysis for the prevention of delayed infarction after subarachnoid haemorrhage

Background To compare the efficacy of two different concepts of cisternal therapy—PREVENTIVE fibrinolysis plus on-demand spasmolysis versus RESCUE spasmolysis—for the prevention of cerebral vasospasm(CVS)and delayed cerebral infarction(DCI)in patients with aneurysmal subarachnoid haemorrhage(aSAH).Methods Retrospective analysis of 84 aSAH patients selected for cisternal therapy for DCI prevention.66 high-risk patients received PREVENTIVE cisternal therapy to enhance blood clearance.Either stereotactic catheter ventriculocisternostomy(STX-VCS)or intraoperative placement of a cisterno-ventriculostomy catheter(CVC),followed by fibrinolytic cisternal lavage using urokinase was performed.In case of vasospasm,nimodipine was applied intrathecally.22 low-risk patients who developed CVS against expectations were selected for STX-VCS as RESCUE intervention for cisternal spasmolysis with nimodipine.Rates of DCI and mean flow velocities of daily transcranial Doppler(TCD)ultrasonographies were evaluated.Results Despite a higher prespecified DCI risk,patients selected for PREVENTIVE intervention primarily aiming at blood clearance had a lower DCI rate compared with patients selected for intrathecal spasmolysis as a RESCUE therapy(11.3%vs 18.2%).After intrathecal treatment onset,CVS(TCD>160 cm/s)occurred in 45%of patients with PREVENTIVE and 77%of patients with RESCUE therapy(p=0.013).A stronger response of CVS to intrathecal nimodipine was observed in patients with PREVENTIVE intervention as the mean CVS duration after start of intrathecal nimodipine was 3.2 days compared with 5.8 days in patients with RESCUE therapy(p=0.026).Conclusions PREVENTIVE cisternal therapy directed at blood clearance is more effective for the prevention of CVS and delayed infarction compared with cisternal RESCUE spasmolysis.

A Low Mach Number IMEX Flux Splitting for the Level Set Ghost Fluid Method

Considering droplet phenomena at low Mach numbers,large differences in the magnitude of the occurring characteristic waves are presented.As acoustic phenomena often play a minor role in such applications,classical explicit schemes which resolve these waves suffer from a very restrictive timestep restriction.In this work,a novel scheme based on a specific level set ghost fluid method and an implicit-explicit(IMEX)flux splitting is proposed to overcome this timestep restriction.A fully implicit narrow band around the sharp phase interface is combined with a splitting of the convective and acoustic phenomena away from the interface.In this part of the domain,the IMEX Runge-Kutta time discretization and the high order discontinuous Galerkin spectral element method are applied to achieve high accuracies in the bulk phases.It is shown that for low Mach numbers a significant gain in computational time can be achieved compared to a fully explicit method.Applica-tions to typical droplet dynamic phenomena validate the proposed method and illustrate its capabilities.

Carbon capture and storage in the USA:the role of US innovation leadership in climate-technology commercialization

To limit global warming and mitigate climate change,the global economy needs to decarbonize and reduce emissions to net-zero by mid-century.The asymmetries of the global energy system necessitate the deployment of a suite of decarbonization technologies and an all-of-the-above approach to deliver the steep CO_(2)-emissions reductions necessary.Carbon capture and storage(CCS)technologies that capture CO_(2) from industrial and power-plant point sources as well as the ambient air and store them underground are largely seen as needed to address both the flow of emissions being released and the stock of CO_(2) already in the atmosphere.Despite the pressing need to commercialize the technologies,their large-scale deployment has been slow.Initial deployment,however,could lead to near-term cost reduction and technology proliferation,and lowering of the overall system cost of decarbonization.As of November 2019,more than half of global large-scale CCS facilities are in the USA,thanks to a history of sustained government support for the technologies.Recently,the USA has seen a raft of new developments on the policy and project side signaling a reinvigorated push to commercialize the technology.Analysing these recent developments using a policy-priorities framework for CCS commercialization developed by the Global CCS Institute,the paper assesses the USA’s position to lead large-scale deployment of CCS technologies to commercialization.It concludes that the USA is in a prime position due to the political economic characteristics of its energy economy,resource wealth and innovation-driven manufacturing sector.

CONSPECTUS:Though relatively young,the mechanistic modeling approach has gained tremendous traction in the past decade as it was proven to be extremely versatile and effective for lithium-ion battery diagnosis and prognosis.The approach is relies on assembling digital twins by matching the individual voltage response of each electrode.Changing the matching,via scaling or translations,enables replication of the degradation modes electrochemical signature.Degradation modes comprise the loss of lithium inventory,the loss of active material,and kinetic changes and refer to the impact of degradation mechanisms on the electrodes rather than their root cause.Every degradation mechanism will affect,to some extent,the amount of material able to react,the amount of lithium able to go back and forth between the electrodes,and the overall reaction kinetics.Quantifying degradation modes open the gate for material-based diagnosis and prognosis without the need for complex models.This Account is first a reflection on a decade worth of discussion and validation of several key concepts about using digital twins for advanced diagnosis and prognosis since the seminal publications in the early 2010s.Since proposing our version of the framework,it has been used to diagnose the degradation of several hundred cells of multiple chemistries and blends.It made it possible to explain and predict the apparition of knees with the concept of hidden mechanisms and to emulate the impact of kinetics.The approach also proved useful to investigate overdischarge and overcharge and to generate big data with millions of synthetic voltage curves enabling the development of advanced diagnosis and prognosis tools.Herein,we will focus on the emulation of kinetic changes,of lithium plating both from rate-dependent and rate-independent origins,and on the utilization of synthetic datasets.This Account will also introduce the next decade with proof-of-concept implementations and simulations that will open new directions to enhance the modeling framework appl

The relationship of high-intensity cross-training with arterial stiffness

Background'.Central arterial stiffness is a cardiovascular risk factor that can be readily affected through engagement in physical exercise training,with resistance and aerobic exercise having disparate affects.Despite the growing popularity of high-intensity cross-training(HICT),little is currently known about the effects of this mixed modality exercise stimulus on arterial stiffness.Therefore,the purpose of this study was to characterize the arterial stiffness of habitual HICT participants vs.aerobically active and sedentary controls using a cross-sectional design.Methods'.A total of 30 participants were recruited:10 middle-aged long-term participants of HICT(CrossFit)and 20 age,sex,and height matched controls(10 recreationally active,10 sedentary).Central and peripheral pulse wave velocities were measured for the carotid-femoral and femoral-dorsalis pedis arterial segments.Aerobic fitness(maximal oxygen uptake,VChmax)was measured and typical exercise participation rates were self-reported for each group.Results'.HICT participants manifested central pulse wave velocity(PWV)(5.3±1.0 m/s,mean±SD)and VChmax(43±6 mL/kg/min)values nearly identical to active controls.Both active groups had significantly better values than sedentary controls(7.1±1.0 m/s,p≤0.001;and 32±7 mL/kg/min,p=0.01).No differences were observed in peripheral PWV between groups.Conclusion'.Habitual participation in HICT exercise was not associated with increased central nor peripheral arterial stiffness.Long-term HICT participants presented with similar fitness and arterial stiffness as compared with participants who practiced traditional aerobic exercise.Compared to sedentary living,HICT may offer musculoskeletal and cardiovascular health benefits without negatively impacting arterial stiffness.

Interactive map reports summarizing bivariate geographic data

Bivariate map visualizations use different encodings to visualize two variables but comparison across multiple encodings is challenging.Compared to a univariate visualization,it is significantly harder to read regional differences and spot geographical outliers.Especially targeting inexperienced users of visualizations,we advocate the use of natural language text for augmenting map visualizations and understanding the relationship between two geo-statistical variables.We propose an approach that selects interesting findings from data analysis,generates a respective text and visualization,and integrates both into a single document.The generated reports interactively link the visualization with the textual narrative.Users can get additional explanations and have the ability to compare different regions.The text generation process is flexible and adapts to various geographical and contextual settings based on small sets of parameters.We showcase this flexibility through a number of application examples.

Hydrologic experiments and modeling of two laboratory bioretention systems under different boundary conditions

Hydrologic performance of bioretention systems is significantly influenced by the media composition and underdrain configuration. This research measured hydrologic performance of column-scale bioretention systems during a synthetic design storm of 25.9 mm, assuming a system area：catchment area ratio of 5%. The laboratory experiments involved two different engineered media and two different drainage configurations. Results show that the two engineered mediawith different sand aggregates were able to retain about 36% of the inflow volume with tree drainage conlaguratlon. However, the medium with marine sand is better at delaying the occurrence of drainage than the one with pumice sand, denoting the better detention ability of the former. For both engineered media, an underdrain configuration with internal water storage （IWS） zone lowered drainage volume and peak drainage rate as well as delayed the occurrence of drainage and peak drainage rate, as compared to a free drainage configuration. The USEPA SWMM v5.1.11 model was applied for the tree drainage configuration case, and there is a reasonable fit between observed and modeled drainag.e-rates when media-specific characteristics are available. For the IWS drainage configuration case, air entrapment was observed to occur in the engineered medium with manne sand. F^lhng ot an IWS zone is most likely to be influenced by many factors, such as the structure of the bioretention system, medium physical and hydraulic properties, and inflow characteristics. More research is needed on the analysis and modeling of hydrologic process in bioretention with IWS drainage configuration.
