Endoscopic transluminal pancreatic necrosectomy using a self-expanding metal stent and high-flow water-jet system

Walled-off pancreatic necrosis and a pancreatic abscess are the most severe complications of acute pancreatitis. surgery in such critically ill patients is often associated with significant morbidity and mortality within the first few weeks after the onset of symptoms. Minimal invasive approaches with high success and low mortality rates are therefore of considerable interest. Endoscopic therapy has the potential to offer safe and effective alternative treatment. We report here on 3 consecutive patients with infected walled-off pancreatic necrosis and 1 patient with a pancreatic abscess who underwent direct endoscopic necrosectomy 19-21 d after the onset of acute pancreatitis. The infected pancreatic necrosis or abscess was punctured transluminally with a cystostome and, after balloon dilatation, a non-covered self-expanding biliary metal stent was placed into the necrotic cavity. Following stent deployment, a nasobiliary pigtail catheter was placed into the cavity to ensure continuous irrigation. After 5-7 d, the metal stent was removed endoscopically and the necrotic cavity was entered with a therapeutic gastroscope. Endoscopic debridement was performed via the simultaneous application of a high-flow water-jet system; using a flush knife, a Dormia basket, and hot biopsy forceps. The transluminal endotherapy was repeated 2-5 times daily during the next 10 d. supportive care included parenteral antibiotics and jejunal feeding. All patients improved dramatically and with resolution of their septic conditions; 3 patients were completely cured without any further complications or the need for surgery. One patient died from a complication of prolonged ventilation severe bilateral pneumonia, not related to the endoscopic procedure. no procedure related complications were observed. Understanding the Urban sprawl in the Mid-size latin American Cities through the Urban Form:Analysis of the Concepcion Metropolitan Area(Chile)

latin American cities, like those from north America and Europe, experience problems of urban sprawl. However, few studies have dedicated exclusively to this phenomenon in specific cities, and this omission is particularly noticeable regarding cities not considered among the megalopolis of the continent. The present work analyzes urban sprawl through an urban form in the Concepción Metropolitan Area, Chile, between 1990 and 2009, considering local aspects that may have played a role in the process. The main empirical results obtained from this study reveal a metropolitan area that has expanded intensely over a 20-year period, growing from 9000 hectares to more than 17,000 ha for a 96% increment in the built-up area. The new urban surfaces consolidate a central conurbation that strengthens the role of the main downtowns, with less-intense occupation towards the sub-centers but in a structure that follows the transportation infrastructure. Over the last 20 years, the distance between the shapes has grown progressively by around 2 km, increasing the size of the ellipse by more than 1000 km2. In particular the complexity of the urbanized surfaces has grown, becoming more irregular in shape and less compact as they come to occupy larger areas. so our principal findings include: an increment of nearly 100% in the urban surface, the importance of a polycentric urban structure in the process of consolidation as a support for analyzing different spatial dynamics, and the growing morphological irregularity of the territory of the sprawl.

Variations in the Patterns of Precipitation in the Watershed of the Ambato River Associated with the Eruptive Process of the Tungurahua Volcano in Ecuador

The Tungurahua, located in the Cordillera de los Andes, is the volcano with the most eruptive activity in Ecuador nowadays. 1993 records the eruptive initial process and in August of 1999, after almost 80 years of rest, the volcano begins an explosive eruptive period. This research examines the effects of the eruptive process of the volcano in the patterns of change in precipitation in the short term in a hydrographic watershed. Their results are intended to contribute to the studies carried out to understand the weather and the factors influencing its variability at local and global level. It aims also to contribute with technical data in the debate about experimenting with artificial volcanoes to weather modification. The analysis demonstrates a process of redistribution of rainfall, with significant increases in rainfall from 42.25% on December, and significant decreases of 40.03% on september, during the presence of the eruptive process.

Bilinear Model Proposal for seismic Analysis Using Triple Friction Pendulum (TFP) Bearings

An analytical model is presented for seismic analysis of triple friction pendulum bearings and validated using 81 bearing tests, each subjected to three cycles, with a duration of 12 seconds and using 250, 200 and 100 tons vertical loads. The main objective is to develop formulas for bilinear behavior using maximum, average and minimum friction coefficients to check which is the closest to the real behavior in the laboratory tests and comparatives curves plotting to observe the standard derivation. Parameters such as friction coefficients, effective stiffness, damping factor and vibration periods are analyzed to understand the structural behavior of the TPF bearings.

Comparison of Insecticide Resistance and Its Enzyme Mechanisms among Aedes aegypti Collected with Three Methods in a Dengue-Endemic City in southern Mexico

Background: Despite the physical and chemical effort to control Aedes aegypti, the arboviruses transmission in the south of Mexico remains latent. Trying to improve the methods of entomological surveillance routinely used, whether the estimation of resistance to insecticides used for its control, as well as their enzyme mechanisms, were influenced by the phase in which the mosquitoes were collected through three different collection methods was investigated. Materials and Methods: Mosquito collections from the “5 de Febrero” neighborhood in Tapachula, Mexico were obtained by ovitraps, larvitraps, and a CDC backpack aspirator. Insecticide resistance of Fsub>1sub> females was determined by WHO diagnostic doses and resistance ratios (RRsub>50sub>), furthermore, levels of insecticide metabolism enzymes were determined by biochemical assays. Results: Overall, in mosquitoes collected by ovitraps, larvitraps, and CDC backpack aspirator respectively, the low mortalities obtained with the discriminant dose to Malathion (27.57%, 26.97%, and 26.91%), and to Bendiocarb (50.5%, 45.36%, and 54.97%) suggest resistance. However, lCsub>50sub> for Malathion (0.922, 0.934, and 0.915) and for Bendiocarb (0.112, 0.109, and 0.107);and the low resistance ratios (RRsub>50sub>) for Malathion (3.34, 3.29, and 3.27) and for Bendiocarb (2.15, 2.1, and 2.06) does not suggest resistance. Although a slight numerical variation is observed between the three lCsub>50sub> values, the overlap observed between their confidence intervals allows us to assume that there were no differences between the three methods. Infective endocarditis and thoracic aortic disease: A review on forgotten psychological aspects

AIM To summarize the current evidence on psychological issues in thoracic aortic disease(TAD) and infective endocarditis(IE) setting. METHODs We performed a narrative review about psychological issues in adults with IE and TAD. Through the electronic databases, Pub Med and Psyc InFO, we searched full manuscripts in English and published until september 1, 2014. REsUlTs We found sixteen studies exploring psychological issues in patients with IE(six studies) and in TAD(ten papers). Psychological issues assessed were quality of life, depression, anxiety and posttraumatic stress disorder. Quality of life was explored in IE(four papers) and in TAD(eight papers). Depression and anxiety were analyzed in TAD only(five papers). Post-traumatic stress disorder was assessed in IE(one study). Quality of life was found impaired in three of four studies about IE and in three of eight studies about TAD. Posttraumatic stress disorder was present in 11% and was associated with lower levels of quality of life in IE patients. In TAD patients, anxiety and depression levels after different invasive interventions did not differ. COnClUsIOn sixteen studies report about psychological issues in IE and TAD. Most of them explore quality of life and to a less extent anxiety and depression.

What is the prognosis of emphysematous pyelonephritis associated with extended-spectrum beta-lactamases producing microorganisms?

Objective:To describe the microbiological characteristics in emphysematous pyelonephritis(EPn),demonstrate the frequency of extended-spectrum beta-lactamase(EsBl)microorganisms,and determine if these microorganisms are associated with the prognosis of patients with ***:We conducted a retrospective study in patients with a diagnosis of EPn in a tertiary care hospital of the northeast region of Mexico during the period from January 2011 to January *** variables were analyzed to determine association with the presence of EsBl-producing *** significance was set with pl of 63 patients were included;55(87.3%)of them were females,with a median age of 55(interquartile range:45-65)*** management was indicated in 38.1%;42.9%were treated with ureteral stent;12.7%with open or percutaneous drainage;15.8%with early nephrectomy;and 9.5%with delayed *** mortality was 13(20.6%)cases;23(36.5%)cases required admission to the intensive care *** most frequent microorganism isolated was Escherichia coli(n=34,53.9%).EsBl microorganisms were found in 31.7%of the *** significant association of EsBl was found with admission to the intensive care unit,or with increased ***:To our knowledge,this is the first study that evaluates EsBl microorganisms as a prognostic factor in *** factors associated with a poor prognosis in patients with EPn have been *** microbiological factors,specifically EsBl-producing bacteria,do not seem to influence in the prognosis of these patients.
