Comparison of HbAlc in Chinese patients with type 1 or type 2 diabetes randomized to twice daily insulin lispro low mix 25 or twice daily human insulin mix 30/70

Background Glycemic control prevents onset and progression of diabetes-related long-term complications. The objective of this study was to demonstrate that twice daily insulin lispro low mix 25 is noninferior to twice daily human insulin mix 30/70 in achieving glycemic control as measured by hemoglobin Alc （HbAlc）, from baseline to endpoint, in patients with type 1 or 2 diabetes. Methods In this phase IV, crossover, open-label, multicenter study, 117 Chinese patients with diabetes were randomly assigned to one of two treatment sequence groups. One group received 12-week treatment with twice daily human insulin mix 30/70 followed by 12-week treatment with twice daily insulin lispro low mix 25, while the other group received the reverse treatment sequence. HbAlc, baseline-to-endpoint change in HbAlc, proportion of patients achieving target HbAlc ≤ 7% and 〈 6.5%, fasting blood glucose, and daily insulin doses were measured for each period. Safety and tolerability were also assessed. Results A statistically significant reduction （P≤0.0001） of HbAlc was achieved after each treatment （human insulin mix 30/70： mean HbA1c=7.91% （95% CI： 7,67%, 8.15%）; insulin lispro low mix 25： mean HbA1c=7.96% （95% CI： 7.72%, 8.20%））. The 95% Cl （-0.20, 0.10） of the difference between the two treatments satisfied the prespecified noninferiority margin of 0.3% （lower limit of 95% CI 〉 -0.3%）. No statistically significant differences between treatments were observed for any of the secondary efficacy measures. The incidence of treatment-emergent adverse events and hypoglycemia between the two treatments and treatment sequence groups was similar. Three serious adverse events were reported （human insulin mix 30/70 group： 2 patients （1.7%, hypoglycemic coma and cardiac failure）; insulin lispro low mix 25 group： 1 patient （0.9%, stroke））. All serious adverse events were resolved and no patients died during the study. Conclusion The results support noninferiority of twice daily insulin lispro low

Reduction of artifacts in dental cone beam CT images to improve the three dimensional image reconstruction

Cone-beam CT (CBCT) scanners are based on volumetric tomography, using a 2D extended digital array providing an area detector [1,2]. Compared to traditional CT, CBCT has many advantages, such as less X-ray beam limitation, and rapid scan time, etc. However, in CBCT images the x-ray beam has lower mean kilovolt (peak) energy, so the metal artifact is more pronounced on. The position of the shadowed region in other views can be tracked by projecting the 3D coordinates of the object. Automatic image segmentation was used to replace the pixels inside the metal object with the boundary pixels. The modified projection data, using synthetically Radon Transformation, were then used to reconstruct a new back projected CBCT image. In this paper, we present a method, based on the morphological, area and pixel operators, which we applied on the Radon transformed image, to reduce the metal artifacts in CBCT, then we built the Radon back project images using the radon invers transformation. The artifacts effects on the 3d-reconstruction is that, the soft tissues appears as bones or teeth. For the preprocessing of the CBCT images, two methods are used to recognize the noisy black areas that the first depends on thresholding and closing algorithm, and the second depends on tracing boundaries after using thresholding algorithm too. The intensity of these areas is the lowest in the image than other tissues, so we profit this property to detect the edges of these areas. These two methods are applied on phantom and patient image data. It deals with reconstructed CBCT dicom images and can effectively reduce such metal artifacts. Due to the data of the constructed images are corrupted by these metal artifacts, qualitative and quantitative analysis of CBCT images is very essential.

Novel approach for the automated detection of allergy test using spectral imaging

This paper proposes a novel approach for the automatic detection of allergy test (allergy lesion). A hyperspectral microscope system was used to image the test samples which were diagnosed by dermatologist. It was found that allergy of different levels, and healthy skin cells show absorption spectra, which are sufficiently characteristic and yet reproducible enough to allow for differentiation when using a spectroscopic system. Principal components analysis was used to extract relevant features that could be used for classification from these spectra. Preliminary results indicate that the different types of allergy cells can be reliably distinguished by these features. We conclude that hyperspectral microscopic analysis is a promising approach for improving and automating the diagnosis of allergy test as well as another skin lesions.

BACKGROUND Trans-anal endoscopic microsurgery(TEM)enables a good visualization of the surgical field and is considered the method of choice for excision of adenomas and early T1 rectal *** rectum and retro-rectal space might be the origin of uncommon neoplasms,benign and aggressive,certain require radical trans-abdominal surgery,while others can be treated by a less aggressive *** this study we report outcomes in patients undergoing TEM for rare and non-adenomatous rectal and retro-rectal lesions over a period of 11 *** Between January 2008 to December 2019 a retrospective analysis was completed for all patients who underwent TEM for non-adenomatous rectal lesion or retro-rectal mass in our *** were discharged once diet was well tolerated and no complications were *** were evaluated at 3 wk post operatively,then at 3-mo intervals for the first 2 years and every 6 mo depending on the nature of the final *** examination and rectoscopy were performed during each of the follow-up *** Out of 198 patients who underwent TEM during the study period,18 had non-adenomatous rectal or retro-rectal *** age was 47 *** mean size of the lesions was 2.9 mm,with a mean distance from the anal margin of 7.9 *** surgical time was 97.8 *** were no intra-operative neither late post-operative *** length of stay was 2.5 *** patient follow-up duration was 42 *** TEM allows for reduced morbidity given its minimally invasive *** should be familiar with the technique but careful patient selection should be *** can be used safely for uncommon rectal and selected retro-rectal lesions without compromising *** believe that it should be reasonably considered as one of the surgical methods when treating rare lesions.

Modeling of the 3D spatio-temporal thermal profile of joule-class Yb3+-based laser amplifiers

Thermal profile modification of an active material in a laser amplifier via optical pumping results in a change in the material’s refractive index,and causes thermal expansion and stress,eventually leading to spatial phase aberrations,or even permanent material *** this purpose,knowledge of the 3D spatio-temporal thermal profile,which can currently only be retrieved via numerical simulations,is critical for joule-class laser amplifiers to reveal potentially dangerous thermal features within the pumped active *** this investigation,a detailed,spatio-temporal numerical simulation was constructed and tested for accuracy against surface thermal measurements of various endpumped Yb^3+-doped laser-active *** measurements and simulations show an excellent agreement and the model was successfully applied to a joule-class Yb3+-based amplifier currently operating in the POLARIS laser system at the Friedrich-Schiller-University and Helmholtz-Institute Jena in Germany.

EFL for Survival Outside the L 1 Environment： An Empirical Study of Arab High School Learners

Teaching English as a foreign language （EFL） has been growing throughout the Arab world and hence English hasbecome an obligatory subject in the school curriculum in most Arab countries. However, teaching EFL to Arablearners outside the L! environment becomes more challenging. In such a setting, Arab EFL learners use mostlyspoken English for survival, i.e., English acts as a genuine lingua franca outside the classroom and this requireswell-qualified teachers as well as a professional educational environment inside the classroom. For survival, thespeaking skill becomes inevitable. The study attempts to investigate the factors that might affect Arab high schoollearners＇ EFL speaking performance in Poland. The data collected come from two sources： a questionnaire that wasdistributed to Arab EFL high school learners （N = 30） studying at the Libyan private school and Dar al salaamprivate school of Iraq in Warsaw and semi-structured interviews with instructors （N = 3）. Both quantitative andqualitative approaches have been employed. The findings reveal that students encounter problems in EFL speakingskill due to three main factors： teacher-related variables, curricular-variables, and learner-variables.
