Epidemiology and Clinical Features of Mycobacterium avium Complex Pulmonary Disease in a Brazilian Amazon Cohort

The incidence of Mycobacterium avium complex pulmonary disease (MACPD) has been globally increasing. The present study aims to provide a comprehensive analysis of clinical characteristics and treatment response among patients in the Amazon region of Brazil and conduct a retrospective cohort study at a prominent referral hospital in the State of Par, Brazil, from 2012 to 2020. The M. avium group represented 58.9% of cases, followed by M. intracellulare (35.7%), M. colombiense (3.6%), and M. chimaera (1.8%). The majority of patients (73.2%) were female and with an average age of 63 years. Primary clinical manifestations included persistent cough and sputum production. The Charlson comorbidity index (CCI) averaged 2.47, with 36.3% of patients having a CCI score equal to higher than 3. Notably, over 70% of patients had received treatment for pulmonary tuberculosis before initiating treatment for MAC infection. The fibrocavitary radiologic form was the most prevalent (55.4%), frequently exhibiting a bilateral distribution (53.6%). Antimicrobial susceptibility revealed a significant prevalence of MAC resistance to drugs in the therapeutic regimen. Despite this, most of the patients experienced clinical improvement (50%). In conclusion, this study highlights a higher prevalence of MAC infections among middle-aged women, with a history of pulmonary tuberculosis treatment and presenting the fibrocavitary radiological form was predominant.

Beyond total mesorectal excision: The emerging role of minimally invasive surgery for locally advanced rectal cancer

This work describes the use of TiO_(2)nanotubes-based electrodes(TNT)modified with Cu_(2)O nanostructures and gold nanoparticles for the photoelectroreduction of CO_(2)to produce value-added compounds.A thin layer of polydopamine was used as both an adherent agent and an electron transfer mediator,due to itsπ-conjugated electron *** highest production yield was achieved using a TNT@PDA/Nc/Au40%electrode,with Faradaic efficiencies of 47.4%(110.5μM cm^(-2))and 27.8%(50.4μM cm^(-2))for methanol and methane,*** performance of the photoelectrodes was shown to be Cu_(2)O facet-dependent,with cubic structures leading to greater conversion of CO_(2)to methanol(43%)and methane(27%),compared to the octahedral morphology,while a higher percentage of metallic gold on the nanostructured Cu_(2)O surface was mainly important for *** functional theory(DFT)calculations supported these findings,attributing the superior photoelectrocatalytic performance of the TNT@PDA/Nc/Au40%electrode for CH4generation to the formation of an OCH3intermediate bonded to Au *** using isotope-labeling and analysis by gas chromatograph-mass(GC-MS)demonstrated that13CO_(2)was the source for photoelectrocatalytic generation of13CH3OH and13CH313CH2OH.

Retrofitting with vegetation recent building heritage applying a design tool--the case study of a school building

Severaf researches show the environmental and microclfmatic benefits of the （ntegration of vegetation in architecture; however the potentialities of vertical and horizontal greening systems to retrofit buildings are still not much investigated. The retrofitting project of the Barsanti Institute of Camogli （Genoa, Italy） is presented, a building dated back to the sixties with serious architectural and efficiency problems, located in a considerable Landscape area. The development and application of a design tool （process tree）, for horizontal and vertical greened surfaces, allows to evaluate the potentialities of vegetation to retrofit and to relate the encountered efficiency problems and the climate characteristics with the choice of plant species, system, and technology more suitable for the specific situation （of which environ- mental and economic impact are also evaluated） and to define a design approach for the systematic consideration of the many parameters involved. ~ 2013. Perioperative fluid management in major hepatic resection: an integrative review

BACKGROUND: Fluid intervention and vasoactive pharmacological support during hepatic resection depend on the preference of the attending clinician, institutional resources, and practice culture. Evidence-based recommendations to guide perioperative fluid management are currently limited. Therefore, we provide a contemporary clinical integrative overview of the fundamental principles underpinning fluid intervention and hemodynamic optimization for adult patients undergoing major hepatic resection. DATA SOURCES: A literature review was performed of MEDLINE, EMBASE and the Cochrane Central Registry of Controlled Trials using the terms 'surgery', 'anesthesia', 'starch', 'hydroxyethyl starch derivatives', 'albumin', 'gelatin', 'liver resection', 'hepatic resection', 'fluids', 'fluid therapy', 'crystalloid', 'colloid', 'saline', 'plasma-Lyte', 'plasmalyte', 'hartmann's', 'acetate', and 'lactate'. Search results for MEDLINE and EMBASE were additionally limited to studies on human populations that included adult age groups and publications in English. RESULTS: A total of 113 articles were included after appropriate inclusion criteria screening. Perioperative fluid management as it relates to various anesthetic and surgical techniques is ***: Clinicians should have a fundamental understanding of the surgical phases of the resection, hemodynamic goals, and anesthesia challenges in attempts to individualize therapy to the patient's underlying pathophysiological condition. Therefore, an ideal approach for perioperative fluid therapy is always individualized. Planning and designing large-scale clinical trials are imperative to define the optimal type and amount of fluid for patients undergoing major hepatic resection. Further clinical trials evaluating different intraoperative goal-directed strategies are also eagerly awaited.

The purpose of this paper is to analyze the determinants of mergers and acquisitions in the oil and gas *** particular,it aims to identify the distinctive financial characteristics of acquiring and target ***,the previous literature has not studied in isolation the oil and gas industry dynamics,and therefore this paper contributes in increasing the knowledge of this sector under a corporate finance *** study uses a quantitative deductive approach in order to arrive to empirical *** observation sample comprises 342 deals that took place solely in the oil and gas industry throughout the period 2008-2018.A binomial logistic regression model is employed in order to determine which firm characteristics are statistically significant for both acquiring and target *** research has found that:（1）overvalued companies are likely to become acquirers,while undervalued firms are likely to be target companies;（2）companies with high availability of free cash flow are likely to become acquirers;（3）large companies are more likely to become acquirers,while small firms are more likely to be targets;（4）acquiring companies are more likely to have low leverage,while target companies present a higher leveraged capital *** this paper does not take into account the different oil price environments oil and gas companies could face,it is recommended for future studies to examine whether firm characteristics change during periods where oil prices are relatively high or ***,a multinomial regression model could be used to take into account other takeover features that could impact the results,such as type（friendly/hostile）and direction（horizontal/vertical）.

Utility of endoscopic ultrasound, cytology and fluid carcinoembryonic antigen and CA 19-9 levels in pancreatic cystic lesions

AIM: To assess the diagnostic accuracy of endoscopic ultrasound (EUS), fluid tumor markers and cytology in distinguishing benign from (pre)malignant pancreatic cystic lesions. METHODS: 46 consecutive patients, referred to a gastroenterologist and surgeon for a symptomatic or incidental pancreatic cyst, were reviewed. EUS, cytology, and carcinoembryonic antigen (CEA) and carbohydrate antigen (CA 19-9) levels were compared with the final diagnosis, based on surgical pathology and/or imaging follow-up of at least 12 mo. Cysts were classified as benign (pseudocyst, serous cystadenoma) or malignant/ pre-malignant (mucinous cystic neoplasm). Receiver- operator characteristics (ROC) curve analysis was performed. RESULTS: The mean age was 56 years; 29% were male and median cyst diameter was 3 cm. Final outcome was obtained in 41 (89%) patients. Twenty-three (56%) of these 41 had surgical pathology. Twenty-three (56%) had benign lesions and 18 (44%) had malignant/pre- malignant lesions. Sensitivity, specificity and positive and negative predictive value of EUS alone to distinguish benign from malignant/premalignant pancreatic cystic lesions were 50%, 56%, 36% and 54% and for cytology were 71%, 96%, 92% and 85%, respectively. The corresponding values for the ROC-derived ideal cutoffswere 75%, 90%, 75%, 90% for CA 19-9 (> 37 U/mL) and 70%, 85%, 79% and 78% for CEA (> 3.1 ng/mL). Subgroup analysis of those with surgical pathology yielded almost identical performance and cutoffs. CONCLUSION: Cytology and cyst fluid tumor marker analysis is a very useful tool in distinguishing benign from (pre)malignant pancreatic cystic lesions.

We estimated water-use efficiency and potential photosyn- thetic assimilation of Holm oak （Quercus ilex L.） on slopes of NW and SW aspects in a replicated field test examining the effects of intensifying drought in two Mediterranean coppice forests. We used standard tech- niques for quantifying gas exchange and carbon isotopes in leaves and analyzed total chlorophyll, carotenoids and nitrogen in leaves collected from Mediterranean forests managed under the coppice system. We pos- tulated that responses to drought of coppiced trees would lead to differ- ential responses in physiological traits and that these traits could be used by foresters to adapt to predicted warming and drying in the Mediterra- nean area. We observed physiological responses of the eoppiced trees that suggested acclimation in photosynthetic potential and water-use effi- ciency：（1） a significant reduction in stomatal conductance （p〈0.01） wasrecorded as the drought increased at the SW site; （2） foliar 813C increased as drought increased at the SW site （p〈0.01）; （3） variations in levels of carotenoids and foliar nitrogen, and differences in foliar morphology were recorded, and were tentatively attributed to variation in photosyn- thetic assimilation between sites. These findings increase knowledge of the capacity for acclimation of managed forests in the Mediterranean region of Europe.

Is Greening the Building Envelope a Sustainable Design Practice？

Nowadays, the number of products and also of architectural projects, qualified as ＂sustainable＂, is ever more growing. Following this trend, architectural design may exploit the use of （eco） materials and components to promote a project as sustainable even without considering its impact on the environment; This can be defined as architectural greenwash. This paper considers the case of green envelopes with the aim to evaluate if these can be defined as sustainable design practice. Green envelopes provide environmental advantages together with the suggestion to a green aesthetics par excellence. In fact, the integration of vegetation in urban fabric plays an important role in improving ecological and environmental conditions of （and in） cities, although systems can have a high environmental burden. This paper aims to answer the question： Is greening the building envelope a sustainable design practice？ To do so, the paper provides a literature review which includes the main research developed regarding environmental burden, benefits and LCA （life cycle assessment） calculations of green envelopes.
