BACKGROUND Life-long removal of gluten from the diet is currently the only way to manage celiac disease(CeD). Until now, no objective test has proven useful to objectively detect ingested gluten in clinical practice. Recently, tests that determine consumption of gluten by assessing excretion of gluten immunogenic peptides(GIP) in stool and urine have been developed. Their utility, in comparison with conventional dietary and analytical follow-up strategies, has not been fully *** To assess the performance of enzyme-linked immunosorbent assay(ELISA) and point-of-care tests(PoCTs) for GIP excretion in CeD patients on gluten-free diet(GFD).METHODS We conducted an observational, prospective, cross-sectional study in patients following a GFD for at least two years. Using the Gastrointestinal Symptom Rating Scale questionnaire, patients were classified at enrollment as asymptomatic or symptomatic. Gluten consumption was assessed twice by 3-d dietary recall and GIP excretion(by ELISA in stool and PoCTs(commercial kits for stool and urine) in two consecutive samples. These samples and dietary reports were obtained 10 day apart one from the other. Patients were encouraged to follow their usual GFD during the study *** Forty-four patients were enrolled, of which 19(43.2%) were symptomatic despite being on a GFD. Overall, 83 sets of stool and/or urine samples were *** out of 44 patients(25.0%) had at least one positive GIP test. Diabetic Patients Develop Greater Intensity of Postoperative Pain than Non-Diabetics after Open Cholecystectomy: A Pilot Study

Background: Pre-clinical and clinical studies have shown that inflammatory pain intensity is increased under diabetes condition. Open cholecystectomy (OC) is a surgical procedure with predictable postoperative pain. However, the comparison of postoperative pain due to open cholecystectomy in diabetic and non-diabetic patients remains unknown. The research question to answer was whether diabetic patients undergoing OC development greater intensity of postoperative pain than non-diabetic patients. Methods: The study was conducted between June 2016 and February 2018 at the Regional Hospital of High Specialty “Dr. Juan Graham Casasús” of Villahermosa, Tabasco, Mexico. Seventy patients in two groups of 35 patients each scheduled for OC under general anesthesia were studied. Pain was assessed using the 11-point numerical rating scale (NRS). The primary endpoint was to know NRS pain scores after awaking of general anesthesia. Secondary outcomes included the time of onset of pain and comparing NRS scores between diabetic and non-diabetic patients undergoing OC. Results: Diabetic patients reported significantly greater intensity pain than non-diabetic patients. The mean overall pain score in the diabetic and non-diabetic patients was 7.2 ± 0.3 and 5.3 ± 0.3 (P = 0.0002), respectively. Furthermore, 60% of diabetic patients had severe pain (NRS ≥ 8) compared to 20% of non-diabetics (P = 0.006). The time to onset postoperative pain was about 35 minutes in both groups (P = 0.876). Conclusions: Diabetic patients undergoing OC have greater intensity postoperative pain and also more frequency of patients with severe pain scores compared with non-diabetic patients. Therefore, analgesic treatment in those patients should consider this point in order to provide a satisfactory postoperative analgesia.

Impact of Organic and Mineral Fertilization in Pecan Nut on Production,Quality and Antioxidant Capacity

The pecan nut is considered one of the most important dried fruits in the world by its classification as healthy food. The nutritional balance in the harvest is crucial for its impact on the production, quality and content of bioactive compounds. This research was carried out in the city of Aldama, Chihuahua (Mexico), where doses of mineral and organic fertilization were tested in L25 Taguchi structure with 6 factors and 5 levels by factor: nitrogen (N) 0 - 240 kg·ha-1, phosphate (P2O5) 0 - 120 kg·ha-1, potassium (K2O) 0 - 100 kg·ha-1, calcium (CaO) 0 - 400 kg·ha-1, liquid humus 0 - 3600 L·ha-1 and solid humus 0 - 8000 kg·ha-1. An average yield of 2.2 t·ha-1 was obtained;145 nuts per kilogram, 60% of the edible part of the nut. The total phenolic content was 225.9 mg gallic acid g-1 and the antioxidant capacity was 180.9 mg Trolox g-1. It is concluded that the factor with the greatest impact on yield, quality and antioxidant capacity in pecan nut was nitrogen. It was also found that the combination of mineral and organic fertilization helps to maintain the production and quality ranges of the nut. In turn, these factors contribute to the increase in the phenolic strength and antioxidant capacity. The optimal fertilization dosis to obtain the maximum levels in production of 3.2 t·ha-1;decrease the number of nuts per kilogram to 135, increase the percentage of the edible part of the nut to 61.8%, the increase in the total phenolic content to 318.6 mg of gallic acid g-1 and in the antioxidant capacity with 187.2 mg of Trolox g-1, was 184 kg·ha-1 of N, 107.4 kg·ha-1 of P2O5, 50 kg·ha-1 of K2O y 2777 L·ha-1 of liquid humus. Finally, the mineral fertilization complemented with organic fertilization is considered a good fertilization strategy for pecan trees and its possible benefits to health and the environment.

Background: A left ventricular aneurysm is a mechanical complication of a myocardial infarction that frequently develops on the anterior and apical wall. Survival of 3 to 5 years is 27% and 12%, respectively. Our objective is to report 3 cases of ventricular aneurysms in atypical locations and analyze the survival of these patients. Material and Methods: Three patients with suspicion of ventricular aneurysm after acute myocardial infarction who sought?attention at our institution were included. All underwent transthoracic echocardiograms (TTE), cardiac magnetic resonance (CMR), nuclear cardiology (NC),?coronary angiotomography and cardiac catheterization to assess the location and characteristics of the aneurysms, left ventricular systolic function and the?anatomical and functional condition of the coronary arteries. Results: The mean age of studied patients was 58.33 ± 10.37 years. The locations of the ventricular aneurysms were lateral, inferior and septal. The patients received optimal medical treatment to control heart failure and were discharged in stable condition without early mortality. The mean follow-up was 6.33 years (intervals: 2 - 9) and during this period the patients were in NYHA functional class I/II. Conclusion: Left ventricular aneurysm is a late mechanical complication of an infarction that can develop in an atypical location. Diagnosis is achieved using non-invasive techniques such as TTE, CMR, NC, and coronary angiotomography. No mortality occurred among the patients during medium and long-term follow-up.

Effect of the Substitution of Sand by Rubber of Waste Tires on the Mechanical Properties of Hydraulic Concrete and Exposure to Gamma Radiation

For a long time and until now, rubber is the most used material for the manufacture of tires for motor vehicles. Unfortunately, once the tire meets its life cycle, the remaining rubber cannot be recycled, so the tires are discarded in collection centers and often in clandestine dumps. This represents a serious environmental problem because, in one case, these waste tires become breeding grounds for insects and wildlife that is harmful to humans. In the second case, the tires are burned, releasing highly damaging gases into the atmosphere. On the other hand, concrete is worldwide the construction material par excellence. It is basically composed of cement, gravel and sand. Mixing these three components in different proportions, their mechanical strength in compression can be increased. However, due to its fragile nature, concrete, once a crack is formed, it rapidly advances by fragmenting the material and producing its rapid collapse. In the present work, in order contribute to the care of the environment as well as to modify the fracture mode of the concrete, rubber particles obtained from waste tires were used as sand substitute in hydraulic concrete. In addition, rubber modified samples concrete were lately exposed to 70 kGy of gamma radiation in order to study the effects of this radiation on the mechanical deformation of concrete. The results showed a decrease in the mechanical properties of the concrete with rubber particles with respect to the traditional concrete itself. However, such decreases were offset by the fact that samples with rubber addition do not collapses as fast as the free rubber samples. The acquired data pave the way for research with great benefits, such as the use of recycled tires in concrete for its fracture mode modification in a beneficial way, as well as a possible decrease in the cost of concrete.

Metabolic syndrome occurrence in university students from México City: The binomium HDL/waist circumference is the major prevalence factor

Objective: Metabolic Syndrome (MetS) is the leading cause to develop type 2 diabetes worldwide. We examined associations of MetS components early in life, and their use as risk factors of acquiring MetS. Method: We used an international definition of MetS. Subjects were categorized into “Healthy”/“Not Healthy”, altered parameters are low HDL-cholesterol, large waist circumference (WC), hypertriacylglycerolemia, hypertension, and hyperglycemia, in 32 combinations (2^5) with two values (altered/not altered). MetS was identified with three or more altered parameters. Results: A total of 3424 students (ages 17 - 24 years) participated in the survey, and 2475 were “Not Healthy” showing at least 1 parameter altered;from them 49.6% showed low blood HDL either alone or combined, 38.2% had altered waist circumference either alone or combined;while 18.1% showed hypertriacylglycero-lemia either alone or combined. Hypertension and hyperglycemia were the lowest in frequency. Conclusion: We propose that the binomium HDL/ Waist Circumference is the main prevalence factor to develop MetS in the asymptomatic young population, followed by hypertriacylglycerolemia which together define MetS;while hypertension and hyperglycemia seem to occur later in MetS.

Breast cancer is the number one cause of can- cer deaths among Hispanic women in the United States, and in Mexico, it recently became the primary cause of cancer deaths. This malign- nancy represents a poorly understood and un- derstudied disease in Hispanic women. The ELLA Binational Breast Cancer Study was es- tablished in 2006 as a multi-center study to as- sess patterns of breast tumor markers, clinical characteristics, and their risk factors in women of Mexican descent. We describe the design and implementation of the ELLA Study and provide a risk factor comparison between women in the U.S. and those in Mexico based on a sample of 765 patients (364 in the U.S. and 401 in Mexico). Compared to women in Mexico, U.S. women had significantly (p < 0.05) lower parity (3.2 vs. 3.9 mean live births) and breastfeeding rates (57.5% vs. 80.5%), higher use of oral contraceptives (60.7% vs. 50.1%) and hormone replacement therapy (23.3% vs. 7.6%), and higher family history of breast cancer (15.7% vs. 9.0%). Re- sults show that differences in breast cancer risk factor patterns exist between Mexico and U.S. women. We provide lessons learned from the conduct of our study. Binational studies are an important step in understanding disease pat- terns and etiology for women in both countries.
