Bacterial Blood Isolates in Children: Conventional vs. Bactec Automated Blood Culture System in a Tertiary Health Centre in Gombe, North East Nigeria

Background/Aim: Blood culture is critical in the diagnosis and treatment of blood stream infections (BSIs) especially in children. BSIs are among the most common cause of morbidity/mortality and blood culture has remained the gold standard for diagnosis. We sought to compare Blood Culture Isolates (BCI) from conventional and Bactec automated blood culture system (ABCS) among paediatric patients at the Federal Teaching Hospital Gombe (FTHG) Nigeria. Methods: BCI in children (0 - 18 years) by conventional method from 2008-2012 and Bactec Automated culture system from 2015-2020 were retrieved from the clinical microbiology laboratory register. Information analyzed included, age, sex, month, and year and blood culture isolates. Results: There were 5276 (56.9% males, 43.1% females) and 1169 (54% males, 46% females) Blood Culture Isolates by CM and ABCS respectively. Overall positive culture isolates were 9.7% (515/5276) in CM and 45.9% (536/1169) in ABCS (p = 0.01). Positivity rate in newborn was 13.3% (282/2114) by CM and 40.9% (219/263) by ABCS p = 0.01;under-5 was 10.5% (448/4253) vs. 37% (359/873) (p = 0.01);Gram positive 32.6% (172) vs. 65% (759) (p = 0.01;Gram negative 55% (2910) vs. 34% (397) (p = 0.01). Staph aureus 22% (114/515) by CM vs. 61.9% (332/536)) by ABCS (p = 0.01);Klebsiella 24.9% (128/515) by CM vs. 7.5% (40/536) p = 0.01) in ABCS, E. coli 8.9% (46/515) vs. 2.1% (11/536) p = 0.01;Proteus vs. 1.1% (6/515) by ABCS, Pseudomonas 3.3% (17/515) vs. 5.6% (30/536) p = 0.05, Alkaligenes 1% (5/515) vs. 8.2% (44/536) p = 0.01 and Citrobacter 1% (5/515) vs. 8.4% (45/536) p = 0.01. Conclusion: Blood culture yield was five times higher with Bactec compared with Conventional method.

Incidence &Risk Factors Associated with Carotid Disease in Patients Undergoing Coronary Artery Bypass Grafting Surgery

Objective: The purpose of this study was to evaluate the true incidence and the risk factors associated with carotid disease in the sitting of high risk patients undergoing coronary artery bypass graft (CABG) using carotid duplex scan and to find out if routine preoperative carotid duplex scan is needed among all these patients. Methods: This retrospective study included 402 consecutive patients who underwent bilateral carotid duplex scan admitted for CABG during the period from January 2006 to December 2008. We excluded patients in cardiogenic shock who were taken to operating room emergently. Results: The prevalence of associated risk factors showed diabetes mellitus recorded the highest (93.3%) whereas peripheral vascular disease the lowest (1.7%), hypertension (89.3%), dyslipidemia (72.6%), smoker (21.1%), left main disease (4.7%), and previous stroke (3%). Patients undergoing CABG has high incidence of carotid disease (68.7%) and severe stenosis is more in patients aged 60 and above (13.5%) versus (2.3%) in age 60, previous stroke and left main disease). Conclusion: This study showed that carotid screening is recommended for all patients who are undergoing CABG due to high incidence of carotid disease.

Congenital Heart Diseases Diagnosed on Transthoracic Echocardiography: Perspectives from the University of Maiduguri Teaching Hospital, Nigeria

In a retrospective study of 1224 transthoracic echocardiograms performed between January 2011 and December 2013, we evaluated the spectrum of congenital heart disease (CHD) diagnosed at a tertiary referral centre in Maiduguri, north-eastern Nigeria. Diagnosis of CHD was made in 88 (8.3%) subjects, comprising 23 (26.1%) adults and 65 (73.9%) aged less than 18 years. Forty six (52.3%) of those with CHD were females, while 42 (47.7%) were males. The frequencies of the CHD in decreasing order were: ventricular septal defect 23 (26.1%), tetralogy of Fallot (TOF) 14 (15.9%) and atrial septal defect (ASD) and atrioventricular septal defect (AVSD) were 11 (12.5%) each. One of the patients with AVSD had Ellis Van Creveldt syndrome. Six (6.8%) cases of patent ductus arteriosus (PDA) were diagnosed in those younger than 18 years, while all the 5 (5.7%) cases of Ebstein’s anomaly were diagnosed in adults. There were 6 (6.8%) cases of Eisenmenger syndrome involving three cases of AVSD, one case of ASD and two cases of Ebstein’s anomaly. Timely definitive cares for these patients are still lacking in Nigeria and many areas of sub-Saharan Africa. We recommend sensitization of all relevant clinicians to actively look for congenital heart defects. Pulse oximetry and postnatal echocardiographic new-born screening which were previously validated should be implemented at secondary and tertiary levels, and efforts should be made towards providing the needed care for patients with CHD.

Synthesis and Characterisation of Colorants Derived from 1,4-Diamino Anthraquinone Polyamides

In this study, the polycondensation products of a diamine anthraquinone (1,4-diamino anthraquinone) and some aliphatic diacids with different methylene numbers were synthesised. Their UV-visible spectroscopy and thermal properties were then characterised. The synthesis was achieved by reacting equimolar solutions of the amine and the acid halides. The structure and thermal properties of the products were characterised. FT-IR Spectra of all the three products revealed the formation of amide due to the presence of strong bands at 1565 and 3390 cm?1 assignable to C=O stretching and N-H vibrations. Differential Scanning Calorimetry revealed high melting exotherms for the products. Thermogravimetry revealed moderate thermal stabilities for Poly NAM and Poly NAS and low thermal stability for Poly NAA which indicated 5.9%, 7.1% and 61.2% weight losses at 285℃, 285℃ and 374℃ for the three products respectively. The wavelengths of maximum absorbance for the dye samples are 601.5, 591 and 589 nm respectively with absorptivity coefficients (a1cm1% ) of 11640000, 11610000 and 11560000 respectively.

Study the Biochemical Correlation of Insulin Resistance with HbA1c and Sex Hormones in NIDDM Patients/Meisan-Iraq

Objective: The association between type 2 diabetes mellitus and certain endocrine disorders has the paucity of information and limited data in the province of Meisan (southern of Iraq). Hence, in the present study, an attempt was made to investigate the levels of plasma HbA1c, serum insulin resistance and levels of serum sex hormones in men and women of control and type 2 diabetic patients. Also, the present study aimed to explored the correlation coefficient (r) between insulin resistance and level of HbA1c, FSH, LH, Tt, progesterone and estradiol in men and women patients with NIDDM. Methods: One hundred Iraqi volunteers with abdominal obesity were selected in the present study. Fifty diabetic patients with NIDDM (26 men, and 24 women) aged between (37) and (65) years and 50 healthy individuals (25 men, and 25 women) aged between (36) and (65) years considered as control group. Complete medical history was taken from each subject and complete clinical examinations were performed for all volunteers. Then, the two groups were matched for their fasting blood sugar, BMI, insulin hormone;c-peptide, HbA1c, sex hormones and insulin resistance parameters (HOMA2-IR, HOMA%B and HOMA%S) were calculated using HOMA2 calculator software. Results: We found that insulin resistance demonstrated negative correlations (p 0.05) with levels of serum follicle stimulating hormone (FSH), luteinizing hormone (LH), total testosterone, progesterone and estradiol. While, there was a positive and significant correlation (p 0.01) with value of HbA1c in men and women NIDDM patients. Conclusion: We conclude that an increase in the level of insulin resistance is associated with increased level of blood HbA1c and decreased in level of serum sex hormones (FSH, LH, Tt, E2 and progesterone) in men and women of diabetic group. Experimental Investigation on the Thermal Properties of Gypsum Plaster-Rice Husk Ash Composite

This experimental investigation aims at evaluating the thermal properties of rice husk ash (RHA)—filled gypsum plaster composite for potential applications, as insulating materials. The thermal conductivities of composites of gypsum plaster reinforced with RHA at 0%, 10%, 20%, 30%, and 40% volume fractions were determined experimentally using Lee’s disc method. The experimental results show that integrating RHA reduces the thermal conductivity of gypsum plaster and improves its insulation capacity. The results obtained from the experiments were compared with the Rule of Mixture Model, Maxwell Model, and Russell Model. It was observed that the thermal conductivities obtained from experiments and the theoretical models decreased with an increase in the volume fraction of RHA. The errors associated with the models with respect to experimental results are on the average of 28.7% for Mixture Rule, 31.6% for Russel Model, and 18.8% for the Maxwell Model. An agricultural waste like RHA can be beneficially used for the preparation of composites and, due to improved insulation capability, these composites can be used for applications such as insulation boards and sheathing, hardboard, ceilings of roofs, decorations, etc.

经济犯罪是指违法侵犯属于个人或社会的经济资源的经济活动,众所周知,经济犯罪会阻碍一国经济发展,从而产生一系列的恶性效应,对于发展中国家产生的负面影响更甚。近年来,苏丹的经济犯罪猖獗,给社会发展带来了诸多负面的影响。因此,研究苏丹的经济犯罪对苏丹正确认识此种犯罪从而重点打击意义重大。本文旨在揭示苏丹经济犯罪及此类犯罪对苏丹经济发展和稳定的影响并从全球的角度和法律的视角对苏丹的经济犯罪进行定义。而苏丹的经济犯罪不是一蹴而就的,它有着自己的产生背景和演变历史。除了涉及到苏丹过去和现在的经济犯罪数据,本文还以经济工业活动及其对人们和发展的影响为标准来划分经济犯罪的种类。苏丹的经济犯罪随着社会的发展表现为不同的类型,既有传统的经济犯罪类型也有新型的犯罪形式,而对这些犯罪进行类型化的划分有利于对于这些犯罪更好的进行打击。另外,这些经济犯罪并非凭空出现的,它的产生有很多方面的原因,诸如家庭的解体、媒体的出现和中途辍学与学校的低效教育作用等社会因素和贫穷、失业、移民等经济因素。此外,本文还探讨了经济犯罪对苏丹的发展和稳定的影响,以及经济犯罪如何导致了达富尔地区的武装冲突,并且揭示了经济犯罪对于比如国家运作和国家公共支出等产生的负面影响。最后,本文试图从苏丹经济犯罪产生的原因入手,有的放矢的提出了一些解决方案,建议和意见,旨在为杜绝或减少此类犯罪。

Kinetic and product studies of the reactions of NO3 with a series of unsaturated organic compounds

Rate coefficients for the reaction of N03 radicals with 6 unsaturated volatile organic compounds(VOCs) in a 7300 L simulation chamber at ambient temperature and pressure have been determined by the relative rate *** resulting rate coefficients were determined for isoprene,2-carene,3-carene,methyl vinyl ketone(MVK),methacrolein(MACR)and crotonaldehyde(CA),as(6.6±0.8)×10-13,(1.8±0.6)×10-11,(8.7±0.5)×10-12,(1.24±1.04)×10-16,(3.3±0.9)×10-15 and(5.7±1.2)×10-15 cm3/(molecule·sec),*** experiments indicate that NO3 radical reactions with all the studied unsaturated VOGs proceed through addition to the olefinic bond,however,it indicates that the introduction of a carbonyl group into unsaturated VOGs can deactivate the neighboring olefinic bond towards reaction with the NO3 radical,which is to be expected since the presence of these electronwithdrawing substituents will reduce the electron density in the π orbitals of the alkenes,and will therefore reduce the rate coefficient of these electrophilic addition *** addition,we investigated the product formation from the reactions of 2-carene and 3-carene with the NO3 *** identification of an epoxide(C10H16OH+),caronaldehyde(C10H16O2 H+) and nitrooxy-ketone(C10H16O4 NH+) was achieved using a proton transfer reaction time-of-flight mass spectrometer(PTR-TOF-MS) and a reaction mechanism is proposed.
