Changes in renal function and morphological variations of kidney diseases in rheumatoid arthritis patients

Objective:Rheumatoid nephropathy is one of the most severe extra-articular manifestations of rheumatoid arthritis(RA)associated with a very unfavorable prognosis.This study aimed to identify changes in renal function and morphological variations of kidney diseases in RA patients.Methods:The study enrolled patients(126 patients)between 18 and 55 years of age with a confirmed active RA of more than 12 months.Each patient underwent the following range of laboratory and instrumental research methods:general clinical analysis of blood and urine,performing urinalysis according to Nechiporenko method;determining daily proteinuria;determining the blood content of glucose,urea,creatinine,uric acid,total bilirubin,liver transaminase level,ionogram,lipidogram,and coagulogram;determining the blood content of rheumatoid factor,anti-streptolysin O,and C-reactive protein;and X-ray of the joints of hands and feet.Renal function was examined by estimating glomerular filtration rate,tubular reabsorption index,and renal functional reserve.For studying the morphological changes in the kidneys under ultrasound examination,renal biopsy was performed in 31 patients with RA with urinary syndrome(proteinuria more than 0.3 g per day and hematuria).Results:Nephropathy in RA is characterized by impaired renal function and manifested by an increased blood creatinine and a decrease in glomerular filtration rate and renal functional reserve.Among morphological variations of nephropathy at RA,mesangial proliferative glomerulonephritis prevails,accounting for 48.4%of patients.Other disorders include the secondary amyloidosis(29.0%of patients),tubulointerstitial nephritis(16.1%),membranous glomerulonephritis(3.2%),and focal-segmental glomerulosclerosis(3.2%).Conclusion:Kidney damage is a common systemic manifestation of RA with a long and active course,a major nephropathy trigger.

Autosomal reciprocal translocations represent exchanges of chromatin fragments between non-homologous chromosomes.Translocations are facilitated by the creation of quadrivalent structures during the first meiotic division,which are characterized by the length of the translocated and centric segments,asymmetry,and the presence of terminal breakpoints,all of which may impact segregation mode.Here,we report a rare case of multiple reciprocal translocations within a single family.This includes the evaluation of the translocations in each of the spouses and an analysis of their chromosome segregation patterns as determined by the constellation of universal characteristics in each of their quadrivalents.The obtained results will be of interest to fundamental biology,as they will expand the understanding of the factors affecting chromosome segregation during meiosis.

On the Influence of High Pressures and Boron Nitride on the Processes of Structure Formation and Microhardness of a High-Entropy Equiatomic Composition AlNiCoFeCr Alloy

The structure of equiatomic high-entropy AlNiCoFeCr alloy obtained by arc melting was investigated. The influence of high pressures (5, 8 and 11 GPa), quenching temperature (1650?C) and small additions of reinforcing agent-boron nitride (10% of the alloy volume) on the microstructure and microhardness of the alloy after quenching was studied. Depending on the conditions of thermobaric action, structures based on solid solution of the B2 type or mixed phases with structures of the Al, A2 or B2 types are formed in the AlNiCoFeCr alloy, which influences the alloy microhardness that varies in the range of 5 - 12.5 GPa. .

Adenocarcinomas of the gastrointestinal tract(esophagus,stomach,and colon)represent a heterogeneous group of diseases with distinct etiology,clinical features,treatment approaches,and prognosis.Studies are ongoing to isolate molecular genetic subtypes,perform complete biological characterization of the tumor,determine prognostic groups,and find predictive markers to the effectiveness of therapy.Separate molecular genetic classifications were created for esophageal adenocarcinoma[The Cancer Genome Atlas(TCGA)],stomach cancer(TCGA,Asian Cancer Research Group),and colon cancer(Colorectal Cancer Subtyping Consortium).In 2018,isolation of TCGA molecular genetic subtypes for adenocarcinomas of the gastrointestinal tract(esophagus,stomach,and colon)highlighted the need for further studies and clinical validation of subtyping of gastrointestinal adenocarcinomas.However,this approach has limitations.The aim of our work was to critically analyze integration of molecular genetic subtyping of gastrointestinal adenocarcinomas in clinical practice.

“Trojan terrorism”leaped to the place and in time as an exhalation of bewilderment at the terrorist attacks in London last year,as a challenge to political manipulation,and jumped as the continuation of the topic of migration(the fundamental problem of modernity),as a creation of an internal conflict that multiplies the external conflict,in the theme of systemic instability-theoretical reasonableness of Deception,which destroys the system of contracts as a condition of the world order.My report at the University of London(Birkbeck)on Feb.9,2018,which was read in English,still not the latest direct evidence of the use of this scandalous method of“Trojan terrorism”—the defamation of the whole country(Russia),plus violation of territorial integrity,including and military attack,another country that lies on the other side of the ocean(Syria),and which is accused of not living by the rules(the“Absolute”method by Caesar),the relevance of its problems beat real events in because the state of modern“utopianism”-the state of Utopus(USA),intended from the matrix of the British Empire,its tactics of violence-“occupy”(seizure of the“alien”)-has never changed,and the March-April events in international politics are just another notch on this dead python,which is called“democratic values”.As in the case of Marks’s“Capital”,the theory first followed,and then the practice of sabotage:a system of terrorist attacks,riots,revolutions,world and civil wars,and“Trojan terrorism”:bold attacks by international terrorists,sweeping away the principles and norms of any social treaty(Rousseau did not even dream of this sadness),the norms of law,ethics and morality,economic agreements(interstate agreements),including military demonstration of force,provocation of military conflicts,pirate antics,demarches of intimidation and blackmail,reviving the myth of the Trojan Horse and giving it the status of an“established order”:the norms that are accepted by the majority as“natural law”,as the natural course of things,contrary to common sense and u

Swinging Consciousness or Self-Organizing Chaos as a Resonant System

Consciousness is the gene of resistance that drives evolution by the evolutionary method-empathic Qualia, the subjective perturbation of “natural selection”, and the choice of which is not limited but stress-resistant to mistakes. Looking into the mirror of ourselves, we bring evolution itself to the mirror, which, without blinking, looks with the eye of a frightened gazelle-incredulously-temptingly. Swinging it with a blow to a blow, taking a lunge and reflecting as the shine of slided comet, translating outrage into joy, the most closed resonant system-consciousness works. It is important not to miss any “punch”(knock/sound/word) that is thrown into your consciousness, so that the sound of the universe (and it is she who sends impulses of understanding) turns into improvisation of your own thinking-becoming a landing from “personal Idaho state”, so that the “muscles” of consciousness pumping iron-training do not sleep like a big earthworm that digested fast food (alimentary, cultured), being content with very little and chafing to empty, in order to work, interpreting what is happening near and around you, and comprehend creatively. The cohesion of eidos is so great that thought literally gains weight-you not only get tired quickly in the process of any “thinking”, but you feel that literally the world with weights hangs in your pockets, throwing a couple of blocks with houses and household on your shoulders. And this is all a consequence of the cohesion and microcoupling of all perceptions that revolve in your mind, touching them democratically: with fluids, resonating with waves of meanings, retransmitting impulses of images, and swaddling with the resulting introjections as by current-already according to Maxwell. Let us leave the God of God, art-skillful, and consciousness-seesaw swing of meanings.
