Sirtuin 3(SIRT3)is a deacetylase that modulates proteins that control metabolism and protects against oxidative *** of SIRT3 activity has been proposed as a promising therapeutic target for ameliorating metabolic diseases and associated cardiac *** this study,we investigated the role of SIRT3 in inflammation and fibrosis in the heart using male mice with constitutive and systemic deletion of SIRT3 and human cardiac AC16 ***3 knockout mice showed cardiac fibrosis and inflammation that was characterized by augmented transcriptional activity of *** with this,SIRT3 overexpression in human and neonatal rat cardiomyocytes partially prevented the inflammatory and profibrotic response induced by TNF-α.Notably,these effects were associated with a decrease in the mRNA and protein levels of FOS and the DNA-binding activity of ***,we demonstrated that SIRT3 inhibits FOS transcription through specific histone H3 lysine K27 deacetylation at its *** findings highlight an important function of SIRT3 in mediating the often intricate profibrotic and proinflammatory responses of cardiac cells through the modulation of the FOS/AP-1 *** fibrosis and inflammation are crucial in the progression of cardiac hypertrophy,heart failure,and diabetic cardiomyopathy,our results point to SIRT3 as a potential target for treating these diseases.

Predictors for Massive Haemorrhage during Caesarean Delivery Due to Placenta Praevia

Objectives: To describe the incidence and predictive factors for massive haemorrhage during a caesarean delivery in women with placenta praevia. Methods: We performed a retrospective cohort study among consecutive women with a placenta praevia planned for a caesarean delivery between 2001-2011 in one academic centre and two teaching hospitals in the Netherlands. Massive haemorrhage was defined as ≥1000 mL blood loss during caesarean section. We used logistic regression analysis to assess which maternal and pregnancy characteristics were predictors for massive haemorrhage during caesarean delivery. Results: Of 54,794 deliveries, we identified 215 (0.39%) women with placenta praevia who underwent a Caesarean delivery, of whom 94 (44%) had massive haemorrhage at or after surgery. After univariable preselection, multivariable logistic regression indicated that higher maternal age (OR 2.09;95% CI 1.17 - 3.74), no Caucasian ethnicity (OR 1.73;95% CI 0.92 - 3.27), multiple pregnancy (OR 3.92;95% CI 0.72 - 21.28), lower systolic blood pressure during pregnancy (OR 1.03;95% CI 1.00 - 1.07) and the placenta located at the anterior wall (OR 2.21;95% CI 1.20 - 4.04) were independent predictors for massive haemorrhage during a caesarean delivery in women with placenta praevia. Predicted probabilities varied between 22% and 71%. The model has a reasonable discriminative ability and acceptable calibration. Conclusions: Women with placenta praevia are at high risk for massive haemorrhage during caesarean delivery. The risk profile can be used to identify extreme high-risk women in whom extensive preventive measures are justified.

Plants accumulate a vast array of secondary metabolites,which constitute a natural resource for *** corymbosa belongs to the Rubiaceae family,and has been used in traditional medicine to treat different diseases,including ***,the active metabolites of the plant,their biosynthetic pathway and mode of action in cancer are *** fill these gaps,we exposed this plant to eight different stress conditions and combined different omics data capturing gene expression,metabolic profiles,and anti-cancer *** results show that *** extracts are active against breast cancer cell lines and that ursolic acid is responsible for this ***,we assembled a high-quality genome and uncovered two genes involved in the biosynthesis of ursolic ***,we also revealed that ursolic acid causes mitotic catastrophe in cancer cells and identified three high-confidence protein binding targets by Cellular Thermal Shift Assay(CETSA)and reverse ***,these results constitute a valuable resource to further characterize the biosynthesis of active metabolites in the Oldenlandia group,while the mode of action of ursolic acid will allow us to further develop this valuable compound.

Split hand/foot malformation (SHFM), formerly known as ectrodactyly is a rare congénital anomaly, its incidence varies from 1/8.500 to 1/25.000 live birth. It mainly affects the development of the limbs, its clinical variability is standard, can present as an isolated feature or as a syndrome associated with other congenital anomalies. Our objective was to present the two cases of SHFM, and to review the literature on the clinical aspects and discuss a probable origin. The father went to school and is a driver because the malformations concerned only the fingers, were less severe, and did not prevent the realization of certain simple gestures of the daily life. On the other hand, the malformations of the fingers of the newborn were severe and the absence of the thumbs compromised the later prehension function. Also the association of a microglossia and a cleft palate contributed to a weight loss that justified hospitalization. The clinical presentation of split hands and feet is variable and the prognosis depends on the type of anomaly. Familial cases suggest a probable genetic origin. Genetic testing is necessary to establish genetic counseling.

颈内动脉狭窄（ICAS）是约30％的缺血性卒中的病因。在50-79岁人群中，约有4％～8％存在〉50％的颈内动脉狭窄。自然史研究和临床试验均已证实，患者的卒中风险随ICAS严重程度的增加而略微升高，尤其是当颈动脉直径减小超过80%时。多中心前瞻性随机试验已经发现，在治疗近期有症状的严重ICAS患者方面，颈动脉内膜切除术（CEA）比内科治疗更有效。然而，在无症状患者中进行CEA治疗的几项随机对照试验并不支持对大多数无症状患者实施CEA；同时，无创性筛查在这些患者中的作用仍然不清楚而且存在争论。此外，在避免非致残性卒中方面的益处是否在统计学上超过手术操作的总体成本和风险还不确定。临床医生一直致力于研究无症状ICAS患者的治疗决策。只有那些狭窄程度≥80％、病情稳定且预期存活至少5年的无症状ICAS患者才应该考虑进行CEA治疗，而且只能在具备已经证实围手术期并发症发生率很低（〈3％）的外科医师的医疗中心实施手术。本文对ICAS的患病率和自然史、CEA治疗无症状ICAS患者的证据、对ICAS进行筛查和监测的作用、评价ICAS的方法以及对执业医师的建议进行概述。
