AIM:To explore the feas.bility of dual camera caps.le (DCC)s.all-bowel(s.)imaging and to examine if two cameras.complement each other to detect more s. ***:Forty-one eligible,cons.cutive patients.underwent DCC s. *** experienced inves.igators.examined the videos.and compared the total number of detected les.ons.to the number of les.ons.detected by each camera *** tolerability was.as.es.ed us.ng a ***:One patient was.*** cameras.detected 68 pos.tive findings.POs.in 20(50%)*** of them were detected by the"yellow"camera,48 by the"green"and 28 by both cameras.44%(n=22)of the"yellow"camera’s.POs.were not detected by the"green"camera and 42%(n=20)of the"green" camera’s.POs.were not detected by the"yellow"*** two cas.s.only one camera detected s.gnificant *** participants.had 216 findings.of unknown s.gnificance(FUs..The"yellow","green"and both cameras.detected 171,161,and 116 FUs.res.ectively;32%(n=55)of the"yellow"camera’s.FUs.were not detected by the"green"camera and 28%(n=45)of the"green"camera’s.FUs.were not detected by the "yellow"*** were no complications.related to the examination,and 97.6%of the patients.would repeat the examination,if ***:DCC s. examination is.feas.ble and well *** two cameras.complement each other to detect more s. les.ons.

Des.ite the effectivenes. of endos.opic retrograde cholangiopancreatography（ERCP） for the treatment of choledocholithias.s. various.complications.have been des.ribed. We herein report the firs. cas. of prolonged pos.ERCP jaundice due to toxicity of the contras. agent Iobitridol（~174;XENETIX, Guerbet, Rois.y Cd G Cedex, France） in a patient who underwent ERCP with s.hincterectomy and common bile duct s.one removal. While clinical improvement and normalization of aminotrans.eras.s.and choles.atic enzymes.after the procedure, an unexplained increas. of direct bilirubin was.noticed. A s.cond ERCP was.performed one week later, excluding pos.ible remaining choledocholithias.s. Nevertheles., s.rum direct bilirubin increas.d further up to 15 mg/d L. Other potential caus.s.of direct hyperbilirubinemia were ruled out and patient＇s.liver biops. was.compatible with drug-induced liver toxicity. Additionally, the caus.-res.lt time connection between the us. of Iobitridol and bilirubin increas. indicated the pos.ibility of a toxic effect related to the repeated us. of the particular contras. agent. Iobitridol, a contras. agent, can induce prolonged direct hyperbilirubinemia.

Although colonos.opy has.been proven effective in reducing the incidence of colorectal cancer through the detection and removal of precancerous.les.ons. it remains.an imperfect examination, as.it can fail in detecting up to almos. one fourth of exis.ing adenomas. Among reas.ns.accounting for s.ch failures. is.the inability to meticulous.y vis.alize the colonic mucos. located either proximal to haus.ral folds.or anatomic curves. including the hepatic and s.lenic flexures. In order to overcome thes. limitations. various.colonos.ope attachments.aiming to improve mucos.l vis.alization have been developed. All of them-trans.arent cap, Endocuff, Endocuff Vis.on and Endorings.are s.mply mounted onto the dis.al tip of the s.ope. In this.review article, we introduce the rationale of their development, pres.nt their mode of action and dis.us. in detail the effect of their implementation in the detection of les.ons.during colonos.opy.

BACKGROUND Endocuff-a plas.ic device with flexible projections.mounted on the dis.al tip of the colonos.ope, promis.s.improved colonic mucos. *** To elucidate the effect of Endocuff on adenoma detection rate(ADR), advanced ADR(AADR) and mean number of adenomas.per colonos.opy(MAC).METHODs.Literature s.arches.identified randomized-controlled trials.evaluating Endocuffas.is.ed colonos.opy(EAC) vs.conventional colonos.opy(CC) in terms.of ADR,AADR and MAC. The effect s.ze on s.udy outcomes.was.calculated us.ng fixed or random effect model, as.appropriate, and it is.s.own as.relative ris.(RR) [95%confidence interval(CI)] and mean difference(MD)(95%CI). The rate of device removal in EAC arms.was.als. *** We identified nine s.udies.enrolling 6038 patients. All s.udies.included mixed population(s.reening, s.rveillance and diagnos.ic examinations.. s.ven and two s.udies.evaluated the firs. and the s.cond-generation device, res.ectively. EAC was.as.ociated with increas.d ADR compared to CC [RR(95%CI): 1.18(1.05-1.32); Ι~2 = 71%]; EAC benefits.more endos.opis.s.with ADR ≤ 35% compared to thos. with ADR > 35% [RR(95%CI): 1.37(1.08-1.74); Ι~2 = 49% vs.1.10(0.99-1.24); Ι~2= 71%]. In terms.of AADR and MAC, no difference was.detected between EAC and CC [RR(95%CI): 1.03(0.85-1.25); Ι~2 = 15% and MD(95%CI): 0.30(-0.17-0.78);Ι~2 = 99%]. s.bgroup analys.s.did not s.ow any difference between the two device generations.regarding all three endpoints. In EAC arms. the device had to be removed in 3%(95%CI: 2%-5%) of the cas.s.mainly due to tortuous.s.gmoid or pres.nce of diverticula along *** EAC increas.s.ADR compared to CC, es.ecially for endos.opis.s.with lower ADR. On the other hand, no s.gnificant effect on AADR and MAC was.detected.

AIM: To retros.ectively compare previous.day vs.s.litdos. preparation in terms.of bowel cleanlines. and polyp detection in patients.referred for polypectomy. METHODs. Fifty patients.underwent two colonos.opies. one diagnos.ic in a private clinic and a s.cond for polypectomy in a Univers.ty Hos.ital. The latter procedures.were performed within 12 wk of the index ones. Examinations.were accomplis.ed by two experienced endos.opis.s. different in each facility. Twenty-s.ven patients.underwent s.reening/s.rveillance colonos.opy, while the res. were s.mptomatic. Previous.day bowel preparation was.utilized initially and s.litdos. for polypectomy. Colon cleans.ng was.evaluated us.ng the Aronchick s.ale. We meas.red the number of detected polyps. and the polyp mis. rates.***: Excellent/good preparation was.reported in 38 cas.s.with previous.day preparation(76%) vs.46 with s.lit-dos.(92%), res.ectively(P = 0.03). One hundred and twenty-s.x polyps.were detected initially and 169 s.bs.quently(P s.126 polyps.were diminutive(P s.29 s.all(P = 0.048) and 13 vs.14 equal or larger than 10 mm. The mis. rates.for total, diminutive, s.all and large polyps.were 25.4%, 30.1%, 13.7% and 6.6%, res.ectively. Multivariate analys.s.revealed that s.lit-dos. preparation was.s.gnificantly as.ociated(OR, P) with increas.d number of polyps.detected overall(0.869, P s.ON: s.lit-dos. preparation improved colon cleans.ng, enhanced polyp detection and unmas.ed s.gnificant polyp mis. rates.

Background:Endos.opic ultras.und(EUs.-guided tis.ue acquis.tion repres.nts.the choice of methods.for s.s.ected lymph nodes.LNs.located next to the gas.rointes.inal *** s.udy aimed to compare the pooled diagnos.ic performance of EUs.guided fine-needle biops.(EUs.FNB)and fine-needle as.iration(EUs.FNA)for LNs.***:We s.arched PubMed/MedLine and Embas. databas.s.through Augus. *** outcome was.diagnos.ic accuracy;s.condary outcomes.were s.ns.tivity,s.ecificity,s.mple adequacy,optimal his.ological core procurement,number of pas.es.and advers. *** performed a pairwis. meta-analys.s.us.ng a random-effects.*** res.lts.are pres.nted as.odds.ratio(OR)or mean difference along with 95%confidence interval(CI).Res.lts.We identified nine s.udies.1,276 patients.in this.*** thes. patients.66.4%were male;the median age was.67 *** accuracy was.not s.gnificantly different between the two approaches.OR,1.31;95%CI,0.81–2.10;P=0.270).The accuracy of EUs.FNB was.s.gnificantly higher when being performed with newer end-cutting needles.OR,1.87;95%CI,1.17–3.00;P=0.009)and in abdominal LNs.OR,2.48;95%CI,1.52–4.05;Ps.of s.mple adequacy was.obs.rved between the two approaches.OR,1.40;95%CI,0.46–4.26;P=0.550);however,his.ological core procurement and diagnos.ic s.ns.tivity with EUs.FNB were s.gnificantly higher than thos. with EUs.FNA(OR,6.15;95%CI,1.51–25.07;P=0.010 and OR,1.87;95%CI,1.27–2.74,P=0.001).The number of needle pas.es.needed was.s.gnificantly lower in the EUs.FNB group than in the EUs.FNA group(mean difference,-0.54;95%CI,-0.97 to -0.12;P=0.010).Conclus.ons.EUs.FNA and EUs.FNB perform s.milarly in LN s.mpling;however,FNB performed with end-cutting needles.outperformed FNA in terms.of diagnos.ic accuracy.

Endos.opic ultras.nography (EUs. was.introduced 25 years.ago aiming at better vis.alization of the pancreas.compared to trans.bdominal ultras.nography. This.update dis.us.es.the current evidence in 2010 concerning the role of EUs.in the clinical management of patients.with pancreatic dis.as.. Major indications.of EUs.are:(1) Detection of common bile duct s.ones.(e.g. in acute pancreatitis.; (2) Detection of s.all exo-and endocrine pancreatic tumours. and (3) Performance of fine needle as.iration in pancreatic mas.es.depending on therapeutic cons.quences. EUs.s.ems.to be les. us.ful in cas.s.of chronic pancreatitis.and cys.ic pan-creatic les.ons. Moreover the cons.ant improvement of computed tomography has.limited the role of EUs.in pancreatic cancer s.aging. On the other hand,new therapeutic options.are available due to EUs.s.ch as.pancreatic cys. drainage and celiac plexus.neurolys.s.offering a new field in which new techniques.may aris.. s. the main goal of this.review is.to determine the exact role of EUs.in a number of pancreatic and biliary dis.as.s.
