Objective： Over 50% of the world populations are infected with Helicobacter pylori （H. pylori）. most subjects are asymptomatic; however, in 1994, H. pylori has been categorized as group I carcinogen. The aim of the study was to investigate the relationship between H. pylori infection and gastric cancer. methods： Thirty gastric cancer patients （GCs） and 30 gastritis patients were enrolled in the study. H. pylori was cultured on non-selective and selective medias, infection density was assessed by quantitative culture. Antibiotic sensitivity testing was performed. PCR was done for the H. pylori 16S rRNA gene in addition to cagA, vacA and iceA genes. Results： H. pylori could be cultured from 100% of specimens obtained from all patients. The density of H. pylori was higher in cancer cases than in gastritis patients. The 16S rRNA was detected in all GC patients （100%） while it was only detected in 70% of gastritis patients. The cagA gene was found in 53.3% vs 13.3% of GC and gastritis patients, respectively. The vacA gene was present in all GC patients （by at least one of its alleles） while it was only found in 33.3% of gastritis patients. The vacA slml combination was the most predominant genotype in GC patients, while m2 was the commonest allele in gastritis patients （10%）. The iceA gene was found in 86.7% vs 40% of GC and gastritis patients, respectively. Simultaneous presence of multiple H. pyiori strains was proved, both phenotypically and genotypically. Conclusion： The development of GC is linked to infection with H. pylori harboring certain virulence genes. Higher infection density of H. pylori was found in GC patients. Co-existence of more than one strain of H. pylori in the same patient occurs in both malignant and benign lesions.

AIm:To assess the effectiveness of hepatitis B virus(HBV) vaccination program among fully vaccinated ***:A national community based crosssectional study was carried out in 6 governorates representing Egypt. A total of 3600 children aged from 9 mo to 16 years who were fully vaccinated with HBV vaccine during infancy were recruited. Face to face interviews were carried out and sera were evaluated for hepatitis B surface antigen(HBsA g),anti-HBV core antibodies(total) and quantitative detection of hepatitis B surface antibody using enzyme linked immunoassays techniques. Samples positive to HBs Ag/anti-HBV core antibodies were subjected to quantitative HBV-DNA detection by real time polymerase chain reaction with 3.8 IU/L detection limit. RESULTS:Sero-protection was detected among 2059 children(57.2%) with geometric mean titers 75.4 ± 3.6 IU/L compared to 3.1 ± 2.1 IU/L among nonseroprotected children. multivariate logistic analysis revealed that older age and female gender were the significant predicting variables for having non seroprotective level,with adjusted odds ratio 3.3,9.1and 14.2 among children aged 5 to mpared to those mong girls compared to boys with P mong children aged ≥ 7 years(0.59%) compared to 0.07% among younger children with odds ratio equal to 8.4(95%CI:1.1-64.2) and P mong girls(0.48%) than boys(0.29%) with P > 0.05. C ON C LU S I ON :T he E gy pt ian c ompuls or y H B V vaccination program provides adequate protection. Occult HBV infection exists among apparently healthy vaccinated children. Adherence to infection control measures is mandatory.

This paper presents a robust multi-stage security solution based on fusion,encryption,and watermarking processes to transmit color healthcare images,*** presented solution depends on the features of discrete cosine transform(DCT),lifting wavelet transform(LWT),and singular value decomposition(SVD).The primary objective of this proposed solution is to ensure robustness for the color medical watermarked images against transmission *** watermark embedding,the host color medical image is transformed into four sub-bands by employing three stages of *** resulting low-frequency sub-band is then transformed by employing three stages of DCT followed by SVD ***,a fusion process is used for combining different watermarks into a single watermark *** single fused image is then ciphered using Deoxyribose Nucleic Acid(DNA)encryption to strengthen the ***,the DNA-ciphered fused watermark is embedded in the host medical image by applying the suggested watermarking technique to obtain the watermarked *** main contribution of this work is embedding multiple watermarks to prevent identity *** the presence of different multimedia attacks,several simulation tests on different colormedical images have been *** results prove that the proposed security solution achieves a decent imperceptibility quality with high Peak Signal-to-Noise Ratio(PSNR)values and high correlation between the extracted and original watermark ***,the watermark image extraction process succeeds in achieving high efficiency in the presence of attacks compared with related works.

目的　定量评价二尖瓣球囊瓣膜成形术对肺高压和心输出量的即刻和远期影响。方法　应用频谱多普勒和心导管技术测定了４７例二尖瓣球囊瓣膜成形术（ＭＢＶＰ）患者术前和术后长期随访的血流动力学改变。结果ＭＢＶＰ后即刻和随访期较术前心脏指数显著增加。肺动脉高压状态显著降低（Ｐ＜０．０１）。结论　ＭＢＶＰ可显著改善二狭患者术后即刻和长期的血流动力学；频谱多普勒是无创定量评价和随访ＭＢＶＰ疗效的可靠技术。
