Performance Analysis on a Dual External Cavity Tunable Laser ECTL Source

This paper presents the diffraction effects on the performance of a dual external cavity tunable laser source, whose external cavities are constructed by micro electro mechanical systems (mEmS). One of the main problems in these structures is the optical diffraction as the emitting surface of the laser diode is usually quite limited in the transverse directions. The emitted beam diffracts rapidly in the air and only a small amount of light is coupled back to the source that usually limits the tuning range of the source. Device characteristics such as tuning range, wavelength shift and sensitivity are evaluated. New expression is used and multiple reflections inside external cavities are considered. The simulation results have shown that single external cavity has limited tuning range. It is shown that multiple reflections have significant effect in our model. To get a better engineering for the dual ECTL dimensions, diffraction effects must be taken into account.

Outcome of patients undergoing early versus late endoscopic intervention for acute oesophageal variceal bleeding at Air Force Hospital, Egypt. A population based cohort study

Background: Whether on-call gastroenterologists should perform emergency endoscopic treatment or delay endoscopy the next day has not been investigated. This study aims to investigate the effect of timing of endoscopy on patients with acute esophageal variceal bleeding. Patients and methods: We included 167 patients with acute variceal bleeding in the gas-troenterology unit of the air force hospital, Cairo, Egypt, from 15 January till 15 July 2010. Endoscopy was scheduled once the patient’s hemodynamics permitted. Time from admission to endoscopy was recorded. Patients were stratified into 2 groups: Early endoscopy: when done ≤ 12 hours and late endoscopy: when done > 12 hours. Patients were followed up for 42 days. Results: The study included 102 males (61.1%) and 65 females (38.9%) (mean age 54 ± 11 years). 151 patients received endoscopic treatment. Early endoscopy group included 85 patients (56.3%), and the late endoscopy group included 66 patients (43.7%) (p = 0.14). Early endoscopy patients were transfused a mean of 2.9 (±1.8) units of blood versus 4.3 (±0.14) units in the late endoscopy patients p = 0.001. mean hospital stay was 7.1 (±3.3) days versus 9.3 (±4.6) days in the early and late groups respectively p: 0.001. 5-day mortality was reported in 6 (±7) cases in the early endoscopy group and 23 (±34.8) cases in the late endoscopy group p: 0.001. Conclusion: Early endoscopic intervention for patients with acute variceal bleeding is associated with a more favorable outcome in terms of morbidity and mortality. On call gastroenterologists should comply with the 12-hour period rather than delaying cirrhotic patients with acute variceal bleeding to the following day.

Intragastric Balloon (BIB) for the management of Obesity, Report on the First Egyptian Experience

Introduction: The bioenteric balloon (BIB) is widely applied in the management of obesity. Aim of the work: This is a report on the efficacy and safety of BIB®device in the first 101 Egyptian cases. methods: Patients referred for BIB between January 2011 till December 2012 were evaluated retrospectively. BIB was inserted for 6 months according to predetermined inclusion and exclusion criteria. Results: In 101 cases, 17 cases were males (16.8%) and 84 cases were females (83.2%), mean age 33.2 (±10.44) years, mean BmI 35.9 (±4.65) kg/m2. None of the patients exhibited balloon migration or perforation. The balloon was removed upon request in 3 patients due to exaggerated intolerance (2.9%). The mean weight loss was 15.5 kg (±4.67), mean EWL 64.12% (±23.48%). The mean BmI at extraction was 29.7 kg/m2 (±4.48) with a BmI loss of 6.2 kg/m2 (±2.0). 87 patients (88.7%) achieved target EWL (32.1% of excess weight), and 87 patients lost >12.2% of their basal weight (88.7%). 70 patients achieved BmI loss >5.7 kg/m2 (71.4%). Conclusion: BIB achieves acceptable success with minimal complications. In further long term, prospective studies are needed to evaluate obesity related comorbidities when using such modality and to compare it to other available devices.

Bacterial infections post-living-donor liver transplantation in Egyptian hepatitis C virus-cirrhotic patients: A singlecenter study

AIm To determine risk factors, causative organisms and antimicrobial resistance of bacterial infections following living-donor liver transplantation(LDLT) in cirrhotic *** This prospective study included 45 patients with hepatitis C virus-related end-stage liver disease who underwent LDLT at Ain Shams Center for Organ Transplant, Cairo, Egypt from January 2014 to November 2015. Patients were followed-up for the first 3 mo after LDLT for detection of bacterial infections. All patients were examined for the possible risk factors suggestive of acquiring infection pre-, intra-and post-operatively. Positive cultures based on clinical suspicion and patterns of antimicrobial resistance were identified. RESULTS Thirty-three patients(73.3%) suffered from bacterial infections; 21 of them had a single infection episode, and 12 had repeated infection episodes. Bile was the most common site for both single and repeated episodes of infection(28.6% and 27.8%, respectively). The most common isolated organisms were gramnegative bacteria. Acinetobacter baumannii was the most common organism isolated from both single and repeated infection episodes(19% and 33.3%, respectively), followed by Escherichia coli for repeated infections(11.1%), and Pseudomonas aeruginosa for single infections(19%). Levofloxacin showed high sensitivity against repeated infection episodes(P = 0.03). Klebsiella, Acinetobacter and Pseudomonas were multi-drug resistant(mDR). Pre-transplant hepatocellular carcinoma(HCC) and duration of drain insertion(in days) were independent risk factors for the occurrence of repeated infection episodes(P = 0.024).CONCLUSION mDR gram-negative bacterial infections are common post-LDLT. Pre-transplant HCC and duration of drain insertion were independent risk factors for the occurrence of repeated infection episodes.

Image processing can be used to extract meaningful optical results from ***,from images of plasmonic structures,we combined convolutional neural networks with recurrent neural networks to extract the absorption spectra of *** provide the data required for the model,we performed 100,000 simulations with similar setups and random *** designing this deep network,we created a model that can predict the absorption response of any structure with a similar *** used convolutional neural networks to collect spatial information from the images,and then,we used that data and recurrent neural networks to teach the model to predict the relationship between the spatial information and the absorption *** results show that this image processing method is accurate and can be used to replace time-and computationally-intensive numerical *** trained model can predict the optical results in less than a second without the need for a strong computing *** technique can be easily extended to cover different structures and extract any other optical properties.

Living donor liver transplantation for high model for endstage liver disease score:What have we learned?

AIm: To assess the impact of model for end-stage liver disease(mELD) score on patient survival and morbidity post living donor liver transplantation(LDLT). mETHODS: A retrospective study was performed on 80 adult patients who had LDLT from 2011-2013. Nine patients were excluded and 71 patients were divided into two groups; Group 1 included 38 patients with a mELD score mELD score > 20. Comparison between both groups was done regarding operative time, intra-operative blood requirement, intensive care unit(ICU) and hospital stay, infection, and patient ***: Eleven patients died(15.5%); 3/38(7.9%)patients in Group 1 and 8/33(24.2%) in Group 2 with significant difference(P = 0.02). mean operative time, duration of hospital stay, and ICU stay were similar in both groups. mean volume of blood transfusion and cell saver re-transfusion were 8 ± 4 units and 1668 ± 202 m L, respectively, in Group 1 in comparison to 10 ± 6 units and 1910 ± 679 m L, respectively, in Group 2 with no significant difference(P = 0.09 and 0.167, respectively). The rates of infection and systemic complications(renal, respiratory, cardiovascular and neurological complications) were similar in both groups. CONCLUSION: A mELD score > 20 may predict mortality after LDLT.

Annexin A2 as a biomarker for hepatocellular carcinoma in Egyptian patients

AIm To investigate the clinical utility of serum annexin A2(ANXA2) as a diagnostic marker for early hepatocellular carcinoma(HCC).mETHODS This study was performed in HCC Clinic of Ain Shams University Hospitals, Cairo, Egypt and included: Group 1: Fifty patients with early stage HCC(Barcelona Clinic Liver Cancer stage A); Group 2: Twenty five patients with chronic liver disease; and Control Group: Fifteen healthy, age-and sex-matched subjects who were seronegative for viral hepatitis markers. The followinglaboratory investigations were done: Viral hepatitis markers [hepatitis B surface antigen and hepatitis C virus(HCV) antibodies], HCV RNA in HCV antibody-positive patients, serum alpha fetoprotein(AFP), and serum ANXA2 *** In this study, 88% of HCC patients(n = 44) were HCVpositive, while HBV infection represented only 8% of all HCC patients(n = 4); and two patients were negative for both viral markers. A highly significant difference was found between patients with HCC and chronic liver disease as well as controls with regard to serum ANXA2 levels(130, IQR 15-240; 15, IQR 15-17; and 17, IQR 15-30 ng/m L, respectively). The area under the curve of ANXA2 was 0.865; the cut-off value was established to be 18 ng/mL with a diagnostic sensitivity of 74% and a specificity of 88%, while the sensitivity and specificity of AFP at the cut-off value of 200 ng/dL were 20% and 100%, *** Serum ANXA2 may serve as a biomarker for the early detection of HCC.

选用Fe、Cu、Al、Pb、SiO_2固体颗粒作为机油污染物,在模拟的发动机缸套-活塞环往复运动实验台上,研究边界润滑条件下固体污染物含量和尺寸对机油摩擦学性能的影响。实验结果表明:机油中的固体颗粒物导致摩擦因数和摩擦功损失增加,同时增加摩擦副接触面的表面粗糙度;摩擦因数、摩擦功损失、磨损量均随污染物含量、颗粒物尺寸的增加而增加,其中铁粒子污染物影响最大,铜次之,铅的影响最小;固体颗粒物会降低发动机性能,增加燃油消耗,为降低固体颗粒物对机油的影响,必须提高机油过滤精度。

明视负向反应(PhNR)和图形视网膜电图(PERG)的结果作为评估青光眼的诊断指标。10例对照组和15例早期青光眼患者接受了完整的眼科检查,包括视力测量,眼压(IOP)测量,眼底检查和视野检查。同时进行图形视网膜电图和全视野视网膜点图纪录。青光眼组的平均偏差和图形平均标准偏差显著较低(P<0.001,P<0.01)。PERG N95,PhNR,b波以及PhNR/b波的幅值显著较低(P<0.001)。眼压升高降低了PERG与PhNR的振幅。
