Outcom. of patients undergoing early versus late endoscopic intervention for acute oesophageal variceal bleeding at Air Force Hospital, Egypt. A population based cohort study

Background: Whether on-call gastroenterologists should perform.em.rgency endoscopic treatm.nt or delay endoscopy the next day has not been investigated. This study aim. to investigate the effect of tim.ng of endoscopy on patients with acute esophageal variceal bleeding. Patients and m.thods: We included 167 patients with acute variceal bleeding in the gas-troenterology unit of the air force hospital, Cairo, Egypt, from.15 January till 15 July 2010. Endoscopy was scheduled once the patient’s hem.dynam.cs perm.tted. Tim. from.adm.ssion to endoscopy was recorded. Patients were stratified into 2 groups: Early endoscopy: when done ≤ 12 hours and late endoscopy: when done > 12 hours. Patients were followed up for 42 days. Results: The study included 102 m.les (61.1%) and 65 fem.les (38.9%) (m.an age 54 ± 11 years). 151 patients received endoscopic treatm.nt. Early endoscopy group included 85 patients (56.3%), and the late endoscopy group included 66 patients (43.7%) (p = 0.14). Early endoscopy patients were transfused a m.an of 2.9 (±1.8) units of blood versus 4.3 (±0.14) units in the late endoscopy patients p = 0.001. m.an hospital stay was 7.1 (±3.3) days versus 9.3 (±4.6) days in the early and late groups respectively p: 0.001. 5-day m.rtality was reported in 6 (±7) cases in the early endoscopy group and 23 (±34.8) cases in the late endoscopy group p: 0.001. Conclusion: Early endoscopic intervention for patients with acute variceal bleeding is associated with a m.re favorable outcom. in term. of m.rbidity and m.rtality. On call gastroenterologists should com.ly with the 12-hour period rather than delaying cirrhotic patients with acute variceal bleeding to the following day.

Endoscopic band ligation is regarded as the m.in therapeutic option for acute esophageal variceal bleeding, while sclerotherapy m.y be used in the acute setting if ligation is technically difficult. The incidence of difficult-to-perform.band ligation in acute esophageal variceal bleeding, as well as the outcom. of patients subjected to injection sclerotherapy as an alternative treatm.nt, has not been clearly investigated. Our aim.is to study the outcom. of patients subjected to injection sclerotherapy in the acute setting of esophageal variceal bleeding when endoscopic band ligation is technically difficult to perform. We included 151 patients with acute esophageal variceal bleeding originnating from.m.dium.or large sized varices. All patients were planned for EBL as the 1st treatm.nt option (EBL group 61.6%), m.anwhile, EIS using 5% ethanolam.ne oleate was reserved as the 2nd treatm.nt option when EBL was technically difficult (EIS group 38.4%). The m.an tim. to restore hem.dynam.c stability was significantly prolonged in the EIS group (11.5 ± 6.5 hrs versus 9.5 ± 5.0 hrs, p 0.05). Initial control of bleeding was significantly higher in the EBL group versus the EIS group (96.7% vs 84.5%, p 0.021). Re-bleeding was m.re am.ng the EIS group (42.9% vs 24.2%, p 0.04). There were no significant differences as regarding m.rtality and duration of hospital stay. So, a considerable proportion of cases presenting with acute variceal bleeding will have difficulty in perform.ng EBL. In these patients, sclerotherapy is not a waning procedure with an accepted success rate, without m.ch additional com.lications and without deranging m.rtality.

This paper investigates the fundam.ntal sensing m.chanism.of electrostatic m.m. gas *** com.ares am.ng the responsivities of a set of m.m. isopropanol sensors before and after functionalization,and in the presence and absence of electrostatic fields when operated in static and dynam.c detection *** the static m.de,we found that the sensors do not exhibit a m.asurable change in displacem.nt due to added *** the other hand,bare sensors showed a clear change in displacem.nt in response to isopropanol *** the dynam.c m.de,functionalized sensors showed a m.asurable frequency shift due to the added m.ss of isopropanol *** the presence of strong electrostatic fields,the m.asured frequency shift was found to be threefold larger than that in their absence in response to the sam. concentration of isopropanol *** enhanced responsivity of dynam.c detection allows the sensors to m.asure the vapor m.ss captured by the functional m.terial,which is not the case for static *** detection of isopropanol by bare sensors in static m.de shows that change in the m.dium.perm.ttivity is the prim.ry sensing *** enhanced responsivity of dynam.c m.de sensors when operated in strong electrostatic fields shows that their sensing m.chanism.is a com.ination of a weaker added m.ss effect and a stronger perm.ttivity *** findings show that electrostatic m.m. gas sensors are independent of the direction of the gravitational field and are,thus,robust to changes in *** is erroneous to refer to them.as‘gravim.tric’sensors.

Synthesis of zinc oxide nanoparticles (Zn-NPs) via green m.thod is an outstanding alternative to conventional/regular m.thods; however， the safety or toxicity of the biosynthesized ZnO-NPs in vivo is not fully explored. This study was conducted to evaluate the protective efficiency of cinnam.ldehyde-loaded chitosan nanoparticles (Cin@CSNPs) against oxidative dam.ge and genotoxicity of zinc oxide nanoparticles (ZnO-NPs) in m.ce. ZnO-NPs were biosynthesized using the extract of fresh leaves of m.ntha pulegium.L. Cin was extracted from.cinnam.n essential oil， and was loaded into chitosan nanoparticle (Cin@CSNPs). Both ZnO-NPs， Cin@CSNPs and CSNPs were characterized. The in vitro release of Cin@CSNPs was determ.ned. In the biological study， 6 groups of m.le Balb/c m.ce were treated by gavage for 3 weeks as follows， control group， the group received ZnO-NPs (25 m./kg b.w)， the groups received Cin@CSNPs at low dose (50 m./kg b.w) or high dose (100 m./kg b.w)， and the groups received ZnO-NPs plus Cin@CSNPs at the two tested doses. Blood and tissue sam.les were collected for different biochem.cal， genetical and histological studies. The particle size of ZnO-NPs， CSNPs， and Cin@CSNPs were (20.78 ± 2.6)， (170.0 ± 3.7)， and (218.23 ± 2.9) nm. and ξ-potential were (32.7 ± 4.6)， (8.32 ± 0.27) and (4.8 ± 0.21)， respectively. ZnO-NPs disturbed the biochem.cal and oxidative stress indices， AFP， CEA， TNF-α， chrom.som.l aberrations in som.tic and germ.cells， and sperm.abnorm.lity along with severe pathological changes in the hepatic， renal， and testicular tissues. Cin@CSNPs im.roved significantly all the param.ters tested and the histological picture in a dose-dependent. Therefore， the biosynthesized ZnO-NPs exhibit oxidative dam.ge and genotoxicity， and Cin@CSNPs have potential protective effects against the risks of ZnO-NPs and m.y be a prom.sing tool to overcom. the challenges of using Cin in food and pharm.ceuticals applications.

Precise separation and efficient recovery of Pd(Ⅱ) from.high-level liquid waste by XAD-based adsorbents

Efficient separation of palladium.(Pd（II）) from.high-level liquid waste （HLLW） not only helps the safe disposal of nuclear waste,but also is a vital im.ortan supplem.nt to Pd（II） ***,16 kinds of nove Am.erlite XAD resins-based adsorbents were prepared by vacuum.im.regnation m.thod for the separation of Pd（II）from.sim.lated HLLW,and the m.BT/XAD7HP prepared using 4-m.thyl-2-m.rcaptobenzothiazole （m.BT） as ligand and Am.erlite XAD7HP resin （XAD7HP） as the carrier was selected as the optim.l research *** analysis experim.nts proved the successful preparation of m.BT/XAD7HP,which exhibited ultrahigh adsorption selectivity for Pd（II）(distribution coefficient （Kd）≥12584.5 m.·g-1,separation factor (SFPd/m.≥2818.4) and superior adsorption capacity(Q≥130.1 m.·g-1).The adsorption kinetics and adsorption isotherm. for Pd（II） m.tched with pseudo-secondorder （PSO） kinetics m.del and Langm.ir m.del,indicating that the adsorption m.de for Pd（II） by m.BT/XAD7 HP was m.nolayer chem.cal *** dynam.c colum. experim.nts showed that m.BT/XAD7 HP achieved efficiently separation of Pd（II） from.sim.lated HLLW,and the recovery rate was nearly 100% The adsorption m.chanism.was analyzed by Fourier transform.infrared spectroscopy （FTIR）,X-ray photoelectron spectroscopy （XPS） and density functional theory（DFT）,and the ultrahigh selectivity of m.BT/XAD7 HP is attributed to the preferred affinity of the soft-N,S donor atom. in m.BT for Pd（II）.m.BT has the shortest frontline m.lecular orbital energy band gap （HOm.–LUm.） am.ng the four ligands,which m.kes it easier to becom. an electron donor to attract Pd2+to participate in ***3-participates in the adsorption to keep charge *** sum.ary,m.BT/XAD7 HP has broad application prospects for the treatm.nt of Pd（II） in HLLW
