AIM： To investigate factors predicting failure of percutaneous endoscopic gastrostomy （PeG） to eliminate gastroesophageal reflux （GeR）. MeTHODS： Twenty-nine consecutive mechanically ventilated patients were investigated. Patients were evaluated for GeR by pH-metry pre-PeG and on the 7th post-PeG day. endoscopic and histologic evidence of reflux esophagitis was also carried out. A beneficial response to PeG was considered when pH-metry on the 7th post-PeG day showed that GeR was below 4%. ReSULTS： Seventeen patients responded （ReSP group） and 12 did not respond （N-ReSP） to PeG. The mean age, sex, weight and APACHe II score were similar in both groups. GeR （%） values were similar in both groups at baseline, but were significantly reduced in the ReSP group compared with the N-ReSP group on the 7th post-PeG day [2.5 （0.6-3.8） vs 8.1 （7.4-9.2, P 〈 0.001）]. Reflux esophagitis and the gastroesophageal flap valve （GeFV） grading differed significantly between the two groups （P = 0.031 and P = 0.020, respectively）. Histology revealed no significant differences between the two ***： endoscopic grading of GeFV and the presence of severe reflux esophagitis are predisposing factors for failure of PeG to reduce GeR in mechanically ventilated patients.

This paper addresses two issues that concern the electricity market participants under the european day-ahead market(DAM)framework,namely the feasibility of the attained schedules and the non-confiscation of cleared *** address the first issue,new resource-specific orders,i.e.,thermal orders for thermal generating units,demand response orders for load responsive resources,and energy limited orders for storage resources,are proposed and incorporated in the existing european DAM clearing *** address the second issue,two approaches which lead to a non-confiscatory market are analyzed:①discriminatory pricing with side-payments(***);and②non-discriminatory pricing excluding out-ofmoney orders(european paradigm).A comparison is performed between the two approaches to investigate the most appropriate pricing rule in terms of social welfare,derived revenues for the sellers,and efficiency of the attained *** proposed model with new resource-specific products is evaluated in a european test system,achieving robust *** feasibility of the attained schedules is demonstrated when using resource-specific orders compared with block ***,the results indicate the supremacy of discriminatory pricing with side-payments compared with the current european pricing rule.

The Fe–Sn-based kagome compounds attract intensive attention due to its attractive topological transport and rich magnetic *** experimental data,first-principles calculations,and Calphad assessment,thermodynamic and topological transport properties of the Fe–Sn system were *** functional theory(DFT)calculations were performed to evaluate the intermetallics’finite-temperature heat capacity(C_(p)).A consistent thermodynamic assessment of the Fe–Sn phase diagram was achieved by using the experimental and DFT results,together with all available data from previous ***,we report that the metastable phase Fe_(3)Sn was introduced into the current metastable phase diagram,and corrected phase locations of Fe_(5)Sn_(3)and Fe_(3)Sn_(2)under the newly measured corrected temperature ***,the anomalous Hall conductivity and anomalous Nernst conductivity of Fe_(3)Sn were calculated,with magnetization directions and doping considered as perturbations to tune such transport *** was observed that the enhanced anomalous Hall and Nernst conductivities originate from the combination of nodal lines and small gap areas that can be tuned by doping Mn at Fe sites and varying magnetization direction.

Water molecules are oriented dipoles joined by hydrogen bonds. When water is heated, this structure collapses (i.e., the entropy increases). When water is re-cooled to a lower temperature, the previous structure is not re-formed immediately. Sometimes, when the re-cooling is performed within a freezer, there is not enough time for the structure to re-form because of the high cooling rate. The entropy reduction curve as a function of the temperature, S = f(T), shows retardation (a lag) relative to the entropy growth curve. Water that has been heated and re-cooled to the initial temperature shows greater entropy than that before it was heated. This means that, while its molecules now have the same kinetic energy, their thermal motion after heating is less oriented with respect to the structure mentioned above. After re-cooling, random collisions are more likely, owing to this the temperature decreases more quickly.

Advances in the neurobiology of growth factors, neural development, and prevention of cell death have resulted in a heightened clinical interest for the development of protective and regenerative neuromodulatory strategies for the cavernous nerves （CNs）, as therapies for prostate cancer and other pelvic malignancies often result in neuronal damage and debilitating loss of sexual function. Nitric oxide released from the axonal end plates of these nerves within the corpora cavernosa causes relaxation of smooth muscle, initiating the haemodynamic changes of penile erection as well as contributing to maintained tumescence; the loss of CN function is primarily responsible for the development of erectile dysfunction （eD） after pelvic surgery and serves as the primary target for potential neuroprotective or regenerative strategies. evidence from pre-clinical studies for select neuromodulatory approaches is reviewed, including neurotrophins, glial cell line-derived neurotrophic factors （GDNF）, bone morphogenic proteins, immunophilin ligands, erythropoetin （ePO）, and stem cells.

AIM： To investigate the influence of experimental obstructive jaundice and exogenous bombesin （BBS） and neurotensin （NT） administration on the expression of the tight junction （TJ）-protein claudin-4 in intestinal epithelium of rats. MeTHODS： Forty male Wistar rats were randomly divided into five groups： Ⅰ = controls, Ⅱ = sham operated,Ⅲ = bile duct ligation （BDL）,Ⅳ = BDL＋BBS （30μg/kg per d）, V = BDL＋NT （300μg/kg per d）. At the end of the experiment on d 10, endotoxin was measured in portal and aortic blood. Tissue sections of the terminal ileum were examined histologically and immunohistochemically for evaluation of claudin-4 expression in intestinal epithelium. ReSULTS： Obstructive jaundice led to intestinal barrier failure demonstrated by significant portal and aortic endotoxemia. Claudin-4 expression was significantly increased in the upper third of the villi in jaundiced rats and an upregulation of its lateral distribution was noted. Administration of BBS or NT restored claudin-4 expression to the control state and significantly reduced portal and aortic endotoxemia. CONCLUSION： experimental obstructive jaundice increases claudin-4 expression in intestinal epithelium,which may be a key factor contributing to the disruption of the mucosal barrier. Gut regulatory peptides BBS and NT can prevent this alteration and reduce portal and systemic endotoxemia.
