Krabbe disease or globoid cell leukodystrophy(GLD;MIM#245200)is a rare and fatal lysosomal storage disease with an autosomal recessive mode of inheritance that results from the deficiency of galactocerebrosidase(GALC;E.C.3.***.1.46),a lysosomal enzyme encoded by the GALC gene.1 GALC breaks down galactosylceramide,a cerebroside located mainly in the myelin *** in GALC cause the accumulation of a cytotoxic metabolite,galactosylsphingosine or psychosine,which can be toxic to oligodendrocytes and Schwann cells.2 The failure to digest galactosylceramide triggers the formation of multi-nucleated globoid cells,causing severe demyelination,axonopathy,and neuronal death.3 The reported frequency of Krabbe disease is 1 in 100,000 live births with symptoms including irritability,loss of motor ability,spasticity,ataxia,visual dysfunction,seizures,andcognitive impairment.

The treatment of shavings,trimmings and splits of leather waste from tanneries has a potential to generate value-added products. In this study enzymatic treatment of leather waste was performed. This method utilizes alkaline protease produced by Bacillus subtilis in our laboratory by submerged fermentation. Optimum conditions of pH,time duration,temperature and concentration of enzyme were determined for maximum degradation of leather waste. The amount of degradation was measured by the release of amino acid hydroxyproline. Amino acid composition in the hydrolysate obtained by the enzyme hydrolysis was determined. This relative simple biotreatment of leather waste may provide a practical and economical solution.

Fission cross sections strongly depend on the ratio of the level density parameter in fission to neutron emission, af/an . In this work, a cascade-exciton model implemented in the code CEM95 has been used to observe this effect for proton induced fission cross sections of tungsten, lead and bismuth. The method was employed using different level density parameter ratios for each fission cross section calculation. The calculated fission cross sections are compared with the available experimental data in the literature. It has been observed that a change of the ratio of the level density parameter, af/an , is necessary with the incident energy of the proton, to best estimate the fission cross sections in CEM95.

Predictive Factors of No Reflow during Primary Angioplasty

Introduction: No reflow during primary angioplasty is associated with a poor prognosis despite the reopening of the culprit coronary. The aim of our work was to determine the predictive factors of no reflow. Methodology: Single-center retrospective analytical study from June 2000 to December 2016 that included patients presenting with STEMI took care of by primary angioplasty. No reflow was defined according to angiographic criteria: a TIMI flow Results: The prevalence of no reflow was 24%. In univariate analysis mean age, diabetes,hypertension, tachycardia, hypotension, killip stage 4 left ventricular failure, hyperglycemia > 11, renal failure, left ventricular dysfunction, tritruncal status, common trunk involvement, initial TIMI flow at 0, significant thrombotic load, delay to angioplasty > 6 hours, and predilation were all correlated with no reflow with a p 75 years [OR = 6.02, 95% CI 1.4 - 27, p = 0.014], tachycardia [OR = 4.3, 95% CI 1.6 - 7.4, p = 0.037], delay to angioplasty > 6 hours [OR = 1.3, 95% CI 1.1 - 2.1, p = 0.003] and high thrombotic load [OR = 1.5, 95% CI 1.3 - 3.2, p = 0.02] were independent predictors of no reflow. Conclusion: No reflow is associated with a poor short-term prognosis. Its care requires knowledge of predictive factors, prevention and treatment.

Influence of primary tumor location and resection on survival in metastatic colorectal cancer

BACKGROUND Patients with right sided colorectal cancer are known to have a poorer prognosis than patients with left sided colorectal cancer, whatever the cancer stage. To this day, primary tumor resection(PTR) is still controversial in a metastatic, non resectable *** To explore the survival impact of PTR in patients with metastatic colorectal cancer(mCRC) depending on *** We retrospectively collected data from all consecutive patients treated for mCRC at the Centre Georges Francois Leclerc Hospital. Univariate and multivariate Cox proportional hazard regression models were used to assess the influence of PTR on survival. We then evaluated the association between PTL and overall survival among patients who previously underwent or did not undergo PTR. A propensity score was performed to match *** Four hundred and sixty-six patients were included. A total of 153(32.8%) patients had unresected synchronous mCRC and 313(67.2%) patients had resected synchronous mCRC. The number of patients with right colic cancer, left colic cancer and rectal cancer was respectively 174(37.3%), 203(43.6%) and 89(19.1%). In the multivariate analysis only PTL, PTR, resection of hepatic and or pulmonary metastases and the use of oxaliplatin, EGFR inhibitors or bevacizumab throughout treatment were associated to higher overall survival rates. Survival evaluation depending on PTR and PTL found that PTR improved the prognosis of both left and right sided mCRC. Results were confirmed by using a weighted propensity *** In mCRC, PTR seems to confer a higher survival rate to patients whatever the PTL.

A clinical perspective on mucoadhesive buccal drug delivery systems

Intermittent energy restriction(IER) defined as periods of energy restriction interspersed with normal energy intake. As a common alternative form of intermittent energy restriction, Ramadan fasting is a religious practice where practitioners of the Muslim faith undergo a yearly ritual in which no food or beverages are consumed between sunrise to sunset for 29-30 days. Our previous finding has demonstrated that Ramadan fasting could leads to shifts in human gut microbiota structured by changing dietary composition. Together with gut microbiota, metabolites also jointly participate the gut function in nutritive, immunologic, and protective in host metabolism. However, limited human studies have been conducted to characterize whether diet-related behaviors, such as fasting(intermittent energy restriction) can influence fecal metabolites, given consideration of ethnic difference. In this study, we aim to study the changes of metabolites in fecal of two ethnic groups before and after fasting by metabolomics, search for potential biomarkers and related metabolic pathways, and thus figure out whether Ramadan fasting altered the metabolites in Chinese and Pakistani individuals with consideration of diet changes. The study will provide new ideas for potential influence of intermittent energy restriction on human health. Methods 34 healthy adult participants, living in the city of Lanzhou and distributed across two ethnic groups(16 Chinese and 18 Pakistani) were recruited in this study. B survey and fecal samples were collected from the two groups before and after Ramadan fasting. LC-MS metabolomics data analysis was used to detect the metabolites from fecal samples. Unsupervised principle component analysis and(orthogonal) partial least-squares-discriminant analysis were employed to identify differential metabolites between different groups. Mucoadhesion can be defined as a state in which two components, of which one is of biological origin, are held together for extended periods of time by the help of interfacial forces. Among the various transmucosal routes, buccal mucosa has excellent accessibility and relatively immobile mucosa, hence suitable for administration of retentive dosage form. The objective of this paper is to review the works done so far in the field of mucoadhe- sire buccal drug delivery systems （MBDDS）, with a clinical perspective. Starting with a brief introduction of the mucoadhesive drug delivery systems, oral mucosa, and the theories of mucoadhesion, this article then proceeds to cover the works done so far in the field of MBDDS, categorizing them on the basis of ailments they are meant to cure. Additionally, we focus on the various patents, recent advancements, and challenges as well as the future prospects for mucoadhesive buccal drug delivery systems.
