Comparative Study of transvesical and Retropubic Prostatic Adenomectomies in the Urology-Andrology Department of Ignace Deen University Hospital Center

Objective: to compare the results of transvesical and retropubic prostatic adenomectomies at the Urology-Andrology Department of Conakry University Hospital. Methodology: this was a prospective multi-operator analytical study lasting 6 months. It included 70 patients operated on for benign prostatic hypertrophy (BPH), divided into 2 groups of 35 according to the retropubic or transvesical technique. the data was collected using a questionnaire and entered into the Epi info application. the analysis was initially descriptive and then univariate. to identify confounding factors, a multivariate analysis was performed. the application conditions of each test were checked before their application. A p-value Results: Upper adenomectomies represented 53.6% of surgeries performed for BPH. the average age was 73.2 years. the mean prostate volume was 117 ± 51.6 cc and the mean total PSA was 9.2 ± 11.4 ng/ml. the majority of patients had an ASA1 (60%) and ASA2 (37.1%) score. Spinal anesthesia, a double-current silicone probe with hinged hinge 22, and drainage of the prevesical space were carried out in all patients, i.e. 100%. the average overall blood loss was 229 ± 98.2 ml. It was lower in the group of patients operated via the retropubic route (187 ± 60.4 ml). the same was true for the duration of intervention (49.1 ± 7.17 vs 55.4 ± 7.9 min), the catheter wearing time (5.14 ± 0.97 vs 9.29 ± 2 .9 days) and the length of hospitalization (=6.26 ± 1.15 vs 12.1 ± 5.74 days). Conclusion: the retropubic technique has advantages over the transvesical one. these advantages are translated by a gain in the duration of the intervention, blood loss, catheter wearing time and length of hospitalization.

Short, Medium, and Long-term Stroke Mortality in Libreville and Associated Factors

Strokes are common around the world and especially in sub-Saharan Africa. they are responsible for severe sequelae and the majority of deaths. In Gabon, no study on stroke mortality has been conducted. Objective: to determine short, medium, long-term mortality and factors associated with long-term stroke mortality. Method: Our study took place in the Neurology Department of the University Hospital Center of Libreville (UHCL). It was a historical cohort study with descriptive and analytical purposes covering the period from June 1 to August 31, 2018 and taking into account hospitalized patients from January 1, 2013 to December 31, 2017. We conducted a comprehensive systematic recruitment of patients with stroke, 18 years old and more, and had been agreed to give information. We included all patients meeting the inclusion criteria. the data was gathered using Epi-Info 7 software. the CHI-2 test was used for the comparison of frequencies and the Student’s test, for comparison of means. Multivariate analysis with logistic regression allowed us to look for factors associated with long-term mortality. A result was statistically significant for a p 0.05 value. Results: At 3 months, 28 patients (18.1%) died, at 6 months thirty-one patients or 20.1% died. At 5 years old, fifty-three patients or 34.4% had died. the factors associated with long-term mortality, if the lost of sight were all alive were tobacco (p = 0.01) and stroke (p = 0.008). If all those who were lost to sight had died, no factor was associated with 5-year mortality. Conclusion: Stroke mortality must not be taken for granted, it can be underestimated because of the large number of lost sight. Measures must be put in place to strengthen post-stroke monitoring. .

Management of Acute Myeloblastic Leukaemia (AML) treated with Intensive Chemotherapy: Experience in a Single Centre

Introduction: Acute myeloblastic leukaemia (AML) is a haematological malignancy with a poor prognosis, despite significant therapeutic progress. this study presents the results of AML management in Mali according to GFAOP recommendations. Methodology: this was a retrospective, cross-sectional study. It included patients aged 0 - 15 years treated in the paediatric oncology unit for AML and followed up between January 2016 and December 2020. Results: During the study period, 85 cases of acute leukaemia were diagnosed in the paediatric oncology unit (including 51 cases of ALL), of which 34 cases of AML were included in this study. the majority were boys (59%). the mean age was 8 years, with extremes of 18 months and 15 years. the mean time to diagnosis was 68 days. In 79% of cases, patients were referred by 1st or 2nd level hospitals. Anaemia was observed in 91% of cases, an infectious syndrome in 68%, haemorrhage in 56% and a tumour syndrome in 85%. the haemogram showed hyperleukocytosis in 15% of cases, thrombocytosis in 22% and severe anaemia in 73%. Death occurred in 85% of cases, most often in the context of sepsis or haemorrhage. Conclusion: AML is probably underestimated in Mali and diagnosis delayed, which may be explained by patient-related factors (lack of knowledge, financial constraints) and a cumbersome referral system. these results suggest the need to implement an appropriate diagnostic and therapeutic strategy, with strong involvement of the political authorities.

Introduction: the level of knowledge and attitude of health professionals about breast cancer are important determinants. General objective: to study the knowledge, attitude and practice of health professionals of the hospital of the district of the commune IV on breast cancer. Methodology: this was a cross-sectional, prospective, descriptive, qualitative and quantitative study. the study period was from 01 October 2022 to December 2022. Results: In one week of survey, 110 health workers (intern, obstetrician nurse, general practitioners and specialists) received the survey sheet, 80 health workers informed it, a participation rate of 73%. the male sex was most represented at 63.75% with a sex ratio of 1.76. the average age was 39.59 years with extremes of 22 and 61 years. the clinical signs evoked by the participants were: breast nodule (81.25%), followed by breast discharge (48.75%). In relation to risk factors: interns and obstetrician nurses had no good knowledge, 51.72% of general practitioners had good knowledge, 43.48% of specialist doctors had good knowledge, and 5.88% of the wise had good knowledge. Conclusion: Breast cancer is a common pathology around the world, health professionals are at the forefront of the fight against breast cancer, and this fight cannot be effective without trained personnel.

Profile of Autoimmune Polyendocrinopathies at the Medical Clinic II of the Abass Ndao Hospital: About 40 Cases

Introduction: Autoimmune polyendocrinopathies (AP) represent a group of rare concomitant pathologies, making them underdiagnosed. the objective was to study their profile at the Medical Clinic II of the Abass Ndao Hospital. Patients and Methods: this was an observational, descriptive and analytical study, lasting 24 months, from January 1, 2020 to December 31, 2022. We assessed the epidemiological, clinical and paraclinical characteristics of the patients and classified the APs found. Results: We included 40 patients divided into type III (38 cases) and IV (2 cases). A female predominance was noted with a sex ratio of 0.21. the mean age was 38.6 years. A family history of component diseases of autoimmune polyendocrine syndrome (APS) was found in 62.5%. Goiter (80%) was the main clinical sign present. All 38 patients with ISAP-3 had autoimmune thyroiditis, including 29 cases of Graves’ disease (72.5%) and 9 cases of Hashimoto’s thyroiditis (22.5%). they were associated with either type 1 diabetes (57.9%), Biermer’s disease (21.1%), vitiligo + alopecia (18.4%), lupus (2.6%). the 2 patients with AP-4 had Biermer’s disease associated with either Addison’s disease or type 1 diabetes. Management depended on the pathologies present and their possible complications. the immunological phenomena were also controlled. Conclusion: this series is globally similar to the literature. the polymorphous character of the clinical pictures requires a better collaboration between specialists leading to a clinical and holistic synthesis.

Introduction: High blood pressure is a major public health problem worldwide due to its frequency and cardiovascular complications. Adherence to treatment for chronic diseases is a global problem. the aim was to study therapeutic adherence in hypertensive patients followed in ambulatory. Materials and Methods: this was a cross-sectional, descriptive study with prospective recruitment that took place from July 1 to December 31, 2022 (6 months) in the cardiology department of the university hospital of Kati. the variables studied were sociodemographic data, cardiovascular risk factors, comorbidities, the possession of insurance and compliance (the Girerd questionnaire was used to assess adherence). Results: A total of 1182 patients were consulted, including 887 for hypertension, a frequency of 75%. Fifty-six patients were included in the study. the average age was 58.18 ± 13.25 years with extremes of 30 and 80 years. there was a female predominance (75%) with a sex ratio of 0.3. the majority of patients lived in urban areas (89.3%). Out-of-school patients accounted for 44.6%, more than half of patients or 55.4% had no income, patients with medical coverage accounted for 67.9% of cases. the main risk factors were physical inactivity (25%) followed by smoking 14.3%. More than 71% of patients had a compliance problem and the main reasons were forgetting to take the drug with 73.2%, followed by delayed treatment of 50% and drug discontinuation of 28.6%. Conclusion: Compliance is a real challenge and a major public health issue. this study allowed us to find a real problem of compliance in our hypertensive patients. there was a statistically significant relationship between drug adherence and forgetting to take the drug and drug discontinuation.

Effects of Chemical treatment on the Physical Properties of typha

Plant-based concretes are produced from plant aggregates and a binder.Plant fibers are mainly composed of saccharides(sugars)and these sugars can decrease the concentration of Ca2+ions in the cement pore solution and delay the formation of hydration products.to improve the interfacial bond between fibers and matrix a chemical treatment is widely used.this study investigates the effect of sodium hydroxide treatment on physical and hygroscopic properties of typha aggregates.In particular,a 5%sodium hydroxide solution is used to treat these aggregates and their bulk and absolute densities,porosity,water content and water absorption are evaluated accordingly.Results indicate that bulk and absolute densities increase after treatment from 56.44 kg/m^(3) to 122.57 kg/m^(3) and 541.93 kg/m^(3) to 555.17 kg/m^(3),respectively.NaOH treatment reduces porosity of typha from 89.58%to 77.92%and decreases water content from 1.4%to 1%.the treatment with sodium hydroxide reduces substantially the water absorption of the aggregates.

Appendicitis bilharzia is a very rare condition and we report 3 cases of this pathology that sense clinical and biological similarities with bacterial appendicitis. the etiological diagnosis was exclusively histopathological, this allowed to highlight eggs of Schistosome in the appendicular wall in the three patients. the surgical treatment was supplemented by a specific medical treatment based on praziquantel. the surgical suites were simple for 2 patients, complicated by superficial parietal suppuration in a patient.

Results of the First 10 Cases of Coronary Bypass Surgery in Senegal

this is a review of the first 10 coronary artery bypass surgeries performed by the local team. the mean age was 62 years old [45 - 74]. the patients were predominantly male, with a M/F ratio of 4:1. Cardiovascular risk factors were mainly myocardial infarction (MI) (60%), hypertension (50%), obesity (40%) and diabetes (30%), with at least two risk factors per patient. Angina was the main symptom (80%). the average time from presentation to surgery was 8 months. the mean Euroscore 2 was 2.92 ± 1.65 [1.33 - 6.60]. Coronary angiography revealed an average of 2 lesions per patient, with 3-vessel involvement in 70% of cases: the Interventricular artery (IVA) (100%), the right coronary artery (90%) and the circumflex artery (70%). On echocardiography, the mean Left ventricular ejection fraction (LVEF) was 59% [33% - 76%]. All patients underwent median sternotomy with bypass grafting. the average duration of the cardiopulmonary bypass was 150 min [46 - 275 min];that of aortic clamping, 120 min [43 - 232 min]. the grafts used were internal thoracic artery (ItA) in 100% of cases (80% on the left and 20% on the right), and the great saphenous vein (GSV) in 60% of cases (50% on the left and 10 on the right). Double bypass was performed in 60% of cases, single bypass in 30% and triple bypass in 10%. the bypasses were performed on the IVA (100%), the middle lateral of the circumflex (30%) and the bisector (20%). the average time to extubation was 11 hours and the length of stay in the intensive care unit was 7 days [03 - 17 days]. One patient had a reoperation on Day 0 post-op. the average hospital stay was 13 days [06 - 27 days]. Complications occurred in nine of the patients (90%), with a predominance of infectious and neurological complications. Overall operative mortality was 3%, all in intensive care.

Introduction: the severity of Sars-Cov-2 infection is associated with the development of acute respiratory distress syndrome (ARDS). the progression to ARDS appears to be driven by a major inflammatory mechanism potentially sensitive to corticosteroids. Observation: this article describes the case of an elderly patient was admitted to emergency departments for intense asthenia, accompanied by motor diarrhoea, dyspnoea with desaturation in ambient air, in a context of strong suspicion of infection linked to COVID-19. the article also reviews the existing literature on the diagnosis and treatment of this severe form of the disease. Conclusion: Corticosteroids, and in particular dexamethasone, have been shown to be effective in the management of patients with COVID-19, an oxygen-releasing disease.
