Development of a Web-Based GIS of Flood Zones in the Municipality of Ouagadougou in Burkina Faso

This document describes the creation of an informative Web GIS aimed at mitigating the impacts of flooding in the municipality of Ouagadougou, in Burkina Faso, a region that is highly sensitive to climate change. Burkina Faso, which is undergoing rapid urbanization, faces major natural threats, particularly flooding, as demonstrated by the severe floods of 2009 that caused loss of life, injury, structural damage and economic losses in Ouagadougou. The aim of this research is to develop a web map highlighting the municipality’s flood-prone areas, with a view to informing and raising awareness of flood risk reduction. Using the Leaflet JavaScript mapping library, the study uses HTML, CSS and JavaScript to implement web mapping technology. Data on Ouagadougou’s flood zones is generated by a multi-criteria analysis combining Saaty’s AHP method and GIS in QGIS, integrating seven (7) parameters including hydrography, altitude, slope, rainfall, soil types, land use and soil moisture index. QGIS processes and maps the themes, PostgreSQL with PostGIS serves as the DBMS and GeoServer functions as the map server. The Web GIS platform allows users to visualize the different flood risks, from very low to very high, or the high-risk areas specific to Ouagadougou. The AHP calculations classify the municipality into five flood vulnerability zones: very low (24.48%), low (27.93%), medium (23.01%), high (17.11%) and very high (7.47%). Effective risk management requires communication and awareness-raising. This online mapping application serves as a tool for communication, management and flood prevention in Ouagadougou, helping to mitigate flood-related natural disasters.

Clinical and Bacteriological Profile of Infections in Sickle Cell Children in Two Referral Hospitals in Niamey, Niger

Introduction: Infections are significant causes of mortality in sickle cell children in resource-limited countries. This study aimed to determine the clinical profile and bacterial ecology of infections in children with sickle-cell disease in two referral hospitals in Niamey. Patients and methods: A retrospective descriptive study was conducted from January 2018 to July 2020 in two referral hospitals in Niamey. All children aged one (1) to 15 years with sickle cell disease admitted for suspected infection, including at least one bacterial culture, were studied. Bacteriological analysis was performed using the appropriate culture media, using BactAlert (Reference 4700003 BTA3D60 BioMérieux). Results: Over 36-months, 350 children with a mean age of 10.9 months were admitted. The sex ratio was 1.2. The SS electrophoretic profile was the most common (93.4%). Immunization status was up to date in 66% of patients. Fever was the common reason for consultation (55.1%). Infection was confirmed in 62 patients (17.7%). The primary diagnoses were bacterial gastroenteritis (24.2%) and urinary tract infection (19.4%). Blood cultures were isolated from Salmonella typhi (13.0%) and Escherichia coli (8.7%). Klebsiella spp (7.1%) and Escherichia coli (5.0%) were detected in cytobacteriological examination of urine. Salmonella typhi (23.5%) and Escherichia coli (5.9%) were isolated on coproculture. Conclusion: Bacterial ecology appears not different from that usually observed in sickle-cell children. Salmonella and Escherichia coli were predominant.

Epidemiological Aspects of Osteoarticular and Rheumatological Diseases Related to Diabetes in Niger

Type 1 or 2 diabetes can be complicated by numerous non-specific joint conditions. Sometimes revealing, these affections are frequent but often remain unrecognized. However, they are the cause of a significant functional handicap. The objective of this work is to describe the osteoarticular rheumatological disorders in diabetics consulting or hospitalized in the rheumatology and internal medicine departments of the National Hospital of Niamey and the Magori Polyclinic. This was a cross-sectional study at the Rheumatology Department in collaboration with the Internal Medicine Department within the National Hospital of Niamey and Polyclinic Magori over a period of 7 months (December 2018 to July 2019). One hundred (100) diabetic patients with osteoarticular diseases were collected, 75% of whom were women with a female/male sex ratio of 0.33. The average age was 56 years with extremes of 21 and 90 years. Forty-three percent (43%) of the patients had diabetes for more than 10 years, with an average age of 13 years. The average BMI was 28.08 ± 5.42 kg/m2. Poor glycemic control was found in the majority of patients (92%). Gonarthrosis was the most common disease associated with diabetes (89%). In the context of joint stiffening syndrome, the most frequent problem was a protruding finger, found in 4% of patients, followed by retractile capsulitis in 3% of the 100 cases. Infectious complications were dominated by septic arthritis in 4% of patients. The presence of one or more degenerative complications of diabetes (nephropathy, retinopathy, nephropathy) was the most frequent factor associated with osteoarticular diseases in our diabetic patients. Sexual Violence in the Zinder Region, about 441 Cases in Reference Motherhoods

Background: Niger is one of the countries that have the highest fertility rate in the world, with 7, 6 children per woman. Zinder is the most populated region of Niger with 5 million inhabitants that is 1/5 of Niger population. The demographic impact that has affected Niger the last 10 years: unemployment poverty and drug use, spawned violence of all kinds in Zinder region (physical and sexual violence). Sexual abuse is a major risk for poor psychiatric and physical outcomes. As sexual violence has serious consequences on the life and future of women and the community and it concerns our field of intervention, we initiated this study whose objective was to determine the socio-epidemiological and lesional aspects, the consequences and appropriate solutions of sexual violence in the Zinder. Methodology: This was a retrospective collection study of all cases of alleged sexual violence received over a 12-year period from 1 January 2009 to 30 June 2021 in Zinder’s referral maternity wards. Results: During the study period, 441 cases of sexual violence were collected;an annual average of 36 cases (441/12). The frequency is 1.29% on the 34,186 consultations. The mean age was 12.31 years with age extremes of 3 to 43 years. Students accounted for 25.39%. Street vendors were found in 224 cases or 50.79%. The predominant reason for admission is rape in 64.63%. The defloration was 52.15%. Pregnancy was found in 118 cases or 26.75%. HIV serology was positive in 1 victim. School dropout was 26.75%. Conclusion: The victims of sexual violence were minors, mostly street vendors and school children. The lack of means of repression at the time allowed the expansion of this violence. The consequences of sexual violence were severe. Advocacy has improved the phenomen on of sexual violence in Zinder.

Abdominal Pregnancy with Hemorrhagic Placenta: About a Case at the Mother and Child Health Center of Zinder

Background: In developing countries maternal mortality and morbidity are most often due to the late to consult, poverty and ignorance. Added to this is, the under-medicalization of the health system. The objective of study is to report a case of late consultation and its consequences and make recommendations to improve: case of abdominal pregnancy with hemorrhagic placenta at the Health and Mother Center (CSME) in Zinder/Niger. Clinical Observation: We describe a case of abdominal pregnancy with hemorrhagic placenta in a 24-year-old woman, without profession, resident in the city of Zinder with a low economic level, consulted for abdomino-pelvic pain. She had a notion of amenorrhea for 5 months without any consultation made. She was pale, feverish and had abdominal pain. Ultrasound revealed a 23-week-old fetus and an empty uterus. The diagnosis was abdominal pregnancy. A laparotomy was performed and a The morbidity was hysterectomy and anemia. The postoperative period course was complicated with anemia. The patient was cured on the 9th day with a good vital prognosis. Conclusion: Abdominal pregnancy occurred in a patient who presented a risk factor. Ultrasound was the key diagnostic test. Surgery was the treatment of choice and the prognosis depends on the earliness of the consultation. Community awareness and early consultation with health services can improve the prognosis of abdominal pregnancy.

Socio-Demographic Factors Associated with Psychoactive Substance Use among Commercial Motorcycle Operators in Kano, Nigeria

Background: Psychoactive substance use was common among commercial motorcycle operators. Studies had shown that there were some socio-demographic factors which were commoner among commercial motorcycle operators who used psychoactive substances. However, this was not explored among commercial motorcycle operators in Kano, North Western Nigeria. Aim: The aim of the study was to determine the socio-demographic factors associated with psychoactive substance use among motorcycle operators in Kano, Northwestern Nigeria. Methods: It was a cross-sectional study. Instrument used for data collection included socio-demographic questionnaire and World Health Organisation Student Drug Use Questionnaire. Data analysis was done using Statistical Package for Social Sciences (SPSS) 16th edition. Results: Commercial motorcycle operators who used substances were found to be of younger age group (X2 = 10.9, P 2 = 4.3, P = 0.034) and had a father who used tobacco (X2 = 4.28, P = 0.04). Participants who were single were more likely to use inhalants (X2 = 21.06, P =0.001), opiates (X2 = 11.82, P = 0.012), cannabis (X2 = 5.12, P = 0.028) and stimulants (X2 = 5.12, P = 0.028) compared to those who were married. Conclusion: This study found that being of younger age, being single, and having a father who smoked were significantly associated with psychoactive substance use. Participants who were singles were more likely to use inhalants, opiates, cannabis and stimulants. No relationship was observed between the age of the respondents and specific substance use. Legislations should be enacted to reduce the rate of psychoactive substance use among motorcycle operators and general population at large.

A Comparison of Quality of Life, Medication Side-Effect and Adherence among Schizophrenia Patients on Conventional versus Atypical Antipsychotics

Quality of life (QoL) is becoming a widely accepted schizophrenia management outcome. But it is still not very clear if there are any significant differences between the conventional and atypical antipsychotics in terms of QoL improvement among people with schizophrenia (PWS). It is also imperative that antipsychotic drug-related factors, such as medication adherence and side-effect, which could directly or indirectly affect the QoL of PWS, are determined and compared among PWS on different classes of the drugs. Data were collected on Socio-demographic Characteristics, Quality of Life and Medication adherence using Socio-demographic and Schizophrenia Clinical Characteristics questionnaire, World Health Organization Quality of Life (WHOQoL)-Brief, and Morisky Medication Adherence Scale (MMAS) respectively from 250 respondents attending a tertiary health center’s Psychiatric clinic in Kano, Nigeria. Although PWS on the two classes of antipsychotic drugs showed inequalities in different aspects and domains of QoL, as well as in the levels of adherence and side-effects, the differences were all insignificant. However, presence of drug side effects was significantly associated with lower health-related QoL in the conventional antipsychotics group (p = 0.001), and lower score in the physical domain of QoL in the atypical antipsychotics group (p = 0.044). Medication adherence was found to be associated with better scores in different domains of QoL in both groups of PWS. There are no significant differences in terms of QoL, medication side-effect and adherence among PWS on the two classes of antipsychotics. However, drug side-effects and adherence were significantly and respectively associated with lower and higher scores in different domains of QoL in both groups.

Misoprostol: An Effective Agent for Cervical Ripening and Labor Induction: A 2-Year Review in a Tertiary Center

Background: Misoprostol a PGE1?analogue has increasingly been used for cervical ripening in preparation for induction of labour with variable outcome for both mother and baby. Objectives: To determine the effectiveness of Misoprostol in cervical ripening and labour outcome in Aminu Kano Teaching Hospital Kano. Study Design: A study of all patients who had cervical ripening for induction of labour using Misoprostol at AKTH Kano, Nigeria, between 1st Jan 2012-31st Dec 2013. Socio-demographic data of these patients including number of doses inserted before ripening is achieved, duration, course and outcome of labour were documented. Result: Four hundred and two women were admitted for cervical ripening. 365 women met the inclusion criteria out of the 358 folders retrieved, giving a retrieval rate of 98%. Despite this six folders did not contain enough information for the study and were excluded leaving us with a sample size of 352. They had a successful cervical ripening with an average of 2 insertions. Spontaneous onset of labour without further intervention was observed in 266 women (75.5%). However, 86 women (24.5%) were induced with oxytocin following cervical ripening. The mean duration of labour among those that had spontaneous onset of labour following Misoprostol insertion was 9.8 hours. 96% of the patients had vaginal delivery while 4% had Caesarean section. Conclusion: Misoprostol was associated with favorable outcome following cervical ripening with a low caesarean section rate.

A Prospective Study on the Incidence and Predictors of Postpartum Depression among Pregnant Women Attending an Antenatal Clinic in Kano, Northern Nigeria

Depressive disorder is a major contributor to years lived with disability (YLD) globally. It is estimated that 13% of all women experience the disorder in the first year postpartum. Postpartum depression (PPD) has significant implications to the physical and mental wellbeing of both the mother and her baby. Only non-depressed pregnant women (score of ≤ 6 on Edinburgh Postnatal Depression Scale) were recruited into this study. Data collection was done at 2 different stages;at recruitment stage (during the third trimester of pregnancy) and at the follow up stage (4th - 6th week postpartum). During the recruitment stage, intimate partner violence (IPV) and level of social support perception, among the women, were assessed using Hurt Insult Threaten Scream (HITS) and Multidimensional Scale of Perceived Social Support (MSPSS) questionnaires respectively. While at the follow up stage, the Obstetric questionnaire and Mini International Neuropsychiatric Interview (MINI-7) were used respectively to obtain obstetric-related data and to diagnose for occurrence of depression. The incidence of PPD was found to be 16.34%. Several factors such as level of education, husband’s socioeconomic status, stressful life events, low perception of social support, obstetric instrumentation and not having a baby of preferred gender were significantly associated with PPD. Positive predictors of PPD included not practicing breast feeding (OR = 186.72, 95% CI = 8.32 - 4188.35), family history of mental illness (OR = 4.95, 95% CI = 1.04 - 23.81) and pregnancy lasting beyond 40 weeks (OR = 7.26, 95% CI = 1.51 - 34.88). High incidence of PPD with numerous associated but largely modifiable risk factors call for more proactive measures, such as PPD screening among pregnant women.

Determination of Effect of Home-Based Oral Chloroquine Treatment on Haematological Indices of <i>P. falciparum</i>Malaria in Children under 5 Years in Jos Metropolis

Background: The incidence of P. falciparum malaria is characterized by high rates of morbidity and mortality in under 5 children;a trend reportedly prevalent in tropical and subtropical countries including Nigeria, and recently observed in Jos metropolis, has to date defied all constructive, preventive and drug therapy intervention measures and consequently continues to constitute a serious public health problem in this most vulnerable group. Objective: The aim of this study was to determine certain haematological indicators of malaria parasite infection;their role in the clinical manifestation of P. falciparum malaria and effect of first line oral chloroquine treatment in children under 5 years attending Jos University Teaching hospital and OLA Hospital in Jos metropolis. Method: This is a cross-sectional study of 93 malaria and non-malaria children, age 1 - 59 months attending Jos University Teaching Hospital (JUTH), Jos and OLA hospital, Jos, North Central Nigeria. Malaria diagnosis was carried out using microscopical examination of Leishman’s stained thick and thin blood films and complete blood count was done using Beckman Coulter Analyzer. Results: The mean percentage lymphocyte value of chloroquine treated children (39.28% ± 7.45%) was significantly lower than the control (66.38% ± 2.27%). The mean granulocyte value of chloroquine treated children (51.07% ± 6.40%) was significantly higher than the control (26.69% ± 2.43%). Red Blood Cell (RBC) counts (4.01 ± 0.21 × 106/μL), Haemoglobin concentration (9.60 ± 0.51 g/dl) and Haematocrit (30.97% ± 1.43%) of chloroquine treated children were significantly higher than corresponding values in untreated malarious children, but not significantly different from values obtained in non-malaria control children. The RBC counts (2.92 ± 0.39 × 106/μL), Haemoglobin concentration (7.23 ± 1.01 g/dl) and Haematocrit (23.70% ± 3.37%) obtained for untreated malarious children were significantly lower than corresponding values in
