Triradiate caecal fold:Is it a useful landmark for caecal intubation in colonoscopy?

AIM: To determine the frequency of identification of the triradiate fold during colonoscopy and evaluate its reliability as a marker of caecal intubation. METHODS: One hundred consecutive patients undergoing colonoscopy in a tertiary hospital colorectal unit from May to September 2013 were studied. Video documentation of the caecum was recorded and shown to consultant colorectal surgeons on the unit. Each reviewer was asked through a series of questions to independently identify the triradiate fold. The main outcome was the frequency of visualisation of the triradiate fold in the ***: The triradiate fold was seen on average in 18% of cases, but inter-observer agreement was poor. There were only four patients(4%) in which all reviewers agreed on the presence of a triradiate fold. In patients who had undergone previous appendicectomy, the appendiceal orifice was less frequently seen compared with patients who had not undergone ***: The triradiate fold is infrequently seen during colonoscopy and is therefore an unreliable landmark of caecal intubation.

体育教育中体现的"体育素养"认为学生无论具有任何能力都应终身保持体育运动,这一概念的重要性正在不断提高。本文旨在讨论运动教育这一现代教学模式的教学特征如何用于体育教育中以培养"体育素养",以及当前有哪些实证研究可证实这一观点。大量实证研究表明:与体育素养发展相关的属性能通过有效地应用这一模型在体育教育中实现(Whitehead,2000年)。运动教育具有鲜明的教学特色,能对体育素养各方面起积极作用,并可进一步使个人体育素养提高并通过自身在体育教育中得以体现。尽管如此,它仍然有自身的局限性,不能作为发展终身体育的"万能药"。如果这些体育素养的相关属性要将个人的动机和自信转化为其利用自身先天的潜能,并明显提升其生活质量,就必须将运动教育发展成为一门连接性的专门学科。

The lrys platform from BioNano Genomics overcomes the limitations of short fragment technologies to provide unprecedented insights into whole-genome biology,lrys is a single-molecule genome analysis system based on NanoChannel Array technology that linearizes extremely long DNA molecules for *** high-throughput platform automates massively parallel imaging of individual molecules of genomic DNA hundreds of kilobases in size to measure sufficient sequence uniqueness and long-range contiguity critical for unambiguous de novo assembly of complex ***-resolution genome maps assembled de novo retain the original context and architecture of the genome,making them extremely useful for sequence assembly scaffolding and structural variation detection applications. Specifically,these genome maps provide dense genome-wide anchor points for ordering and orienting sequencing contigs or scaffolds to greatly increase completion and accuracy of de novo *** variants and repeats are measured directly within long "reads" for comprehensive analysis of what has been dubbed "the inaccessible genome".Following an introduction to the platform and underlying single-molecule technology,several examples of real-world application will demonstrate its use in large complex genomes.

A Systematic Review of Proton Pump Inhibitors for the Prevention and Treatment of Preeclampsia and Gestational Hypertension

Background: Preeclampsia is a hypertensive disorder of pregnancy with a high rate of neonatal and maternal morbidity and mortality. The only definitive treatment is delivery. Through pre-clinical studies, proton pump inhibitors (PPIs), which are commonly and safely used in pregnancy, have been identified as potential therapeutic agents. Objective: To undertake a systematic review evaluating PPIs in the prevention and/or treatment of preeclampsia and gestational hypertension. Search strategy: Electronic databases were searched from inception to 2018. Search terms included preeclampsia, proton-pump inhibitors, pregnancy-induced hypertension, lansoprazole, rabeprazole, esomeprazole and omeprazole. Selection criteria: Studies were included if they were randomized control trials, case-control or cohort studies on human subjects. Case reports, review articles, opinion pieces and conference abstracts were excluded as well as studies with no or inappropriate control arms. Data collection and analysis: Only one eligible study was identified, so no analyses were able to be performed. Main results: There was only one clinical trial eligible for inclusion. This was a randomized clinical trial investigating the role of PPIs in the treatment of preterm preeclampsia in a high-risk, inpatient setting. This was a negative finding, with no change in the primary outcome of prolongation of gestation between the PPI group and placebo control. Conclusion: There is a scarcity of clinical trials published investigating the therapeutic potential of PPIs for preeclampsia and gestational hypertension. Of the one available study, PPIs were not found to prolong gestation among preterm preeclamptic patients compared to placebo control. In order to further elucidate the clinical potential of PPIs to prevent or treat preeclampsia, further trials are required.
