Characterization of optimal resting tension in human pulmonary arteries

AiM To determine the optimum resting tension(ORT) for in vitro human pulmonary artery(PA) ring *** Pulmonary arteries were dissected from disease free sections of the resected lung in the operating theatre and tissue samples were directly sent to the laboratory in Krebs-Henseleit solution(Krebs).The pulmonary arteries were then cut into 2 mm long *** rings were mounted in 25 m L organ baths or 8 m L myograph chambers containing Krebs compound(37 ℃,bubbled with 21% O_2:5% CO_2) to measure changes in isometric *** resting tension was set at 1-gram force(gf) with vessels being left static to equilibrate for duration of one *** contractile reactions to 40 mmol/L KCl were obtained from a resting tension of 1 *** reactions to 40 mmol/L KCl were then obtained from stepwise increases in resting tension(1.2,1.4,1.6,1.8 and 2.0 gf).RESULTS Twenty PA rings of internal diameter between 2-4 mmwere prepared from 4 *** human PA rings incrementing the tension during rest stance by 0.6 gf,up to 1.6 gf significantly augmented the 40 mmol/L KCl stimulated *** enhancement of active tension by 0.4 gf,up to 2.0 gf mitigate the 40 mmol/L KCl stimulated *** Myograph and the organ bath demonstrated identical conclusions,supporting that the radial optimal resting tension for human PA ring was 1.61 *** The radial optimal resting tension in our experiment is 1.61 gf(15.78 m N) for human PA rings.

Coronary Artery Bypass Surgery for Patients Presenting with Ventricular Arrhythmias: Propensity Matched Early and Late Outcome

Objectives: Patients with ischemic ventricular arrhythmia (iVA) in the form of fibrillation or tachycardia represent a surgical challenge. Evidence in the literature suggests that ventricular arrhythmia threatens survival even after cardiac surgery. We aim to review the results of our patients presenting with iVA with regard to short and long term outcome following cardiac surgery. Methods: This was a retrospective study of data entered prospectively into our cardiac surgical database between January 1999 and September 2015. A total of 9609 patients underwent Cardiac Surgery which included 54 patients after surviving iVA. The short- and long-term outcomes were compared to a propensity matched group. Actuarial survival was calculated using Kaplan Meier analysis. Results: The 54 study group patients were propensity matched on a 1:2 basis with a control group of non-iVA (n = 108). The baseline preoperative characteristics and risk factors were similar between the 2 groups and all cases underwent CABG only. Univariate analysis showed pacing postoperatively (33.3 vs 66.7%;p = 0.001) and postoperative ventricular arrhythmia (10 vs 22.2%;p = 0.039) to be significantly higher in the iVA group. Cox-multivariate analysis showed postoperative ventricular arrhythmia in either group (Hazard ratio = 1.5) to be the only significant factor to impact mortality (p 0.001). Long term survival was not significantly different between the two groups (10.4;Ci: 9.08 - 11.75 vs 9.3;Ci: 7.61 - 11.01 yrs, p = 0.3). Conclusion: Cardiac surgery on patients presenting with iVA can be performed safely yielding short and long term results equivalent to non-iVA cases. These patients should not be denied surgery with consideration of good long term outcome.

Does Chemo-Radiation Therapy influence Outcomes in Unresectable Locally Advanced State iV Rectal Cancer?

introduction: The median survival for patients with stage iV rectal cancer is approximately 20 months. Therapy should focus not only on improving survival but also on quality of life. The aim of our study was to determine if chemoradiation (C-RT) would improve palliation for metastatic unresectable locally advanced disease compared to patients receiving palliative chemotherapy alone (C) for stage iV cancer. Methods: Retrospective review of a prospectively maintained database at a single institution was carried out under iRB approval. From January 2004 to December 2008, 43 patients presenting with unresectable stage iV rectal cancer were identified with a median follow-up of 12 months. Patients with evidence of locally advanced disease or bulky disease received infusional 5-FU ± bevacizumab and 3D conformed mega voltage photon therapy (5400 cGy). Patients without evidence of bulky disease received either FOLFOX or FOLFiRi ± bevacizumab. Data on demographics, investigations, treatment, complications, metastasis, number of blood transfusions, days of hospitalization, and surgical intervention were analyzed using SPSS statistical software. p idered statistically significant. Results: There were 25 and 18 patients in the C and C-RT groups respectively. There was no difference in mean age, sex or overall survival. in the framework of liberalized deregulated electricity market, dynamic competitive environment exists between wholesale and retail dealers for energy supplying and management. Smart Grids topology in form of energy management has forced power supplying agencies to become globally competitive. Demand Response (DR) Programs in context with smart energy network have influenced prosumers and consumers towards it. in this paper Fair Emergency Demand Response Program (FEDRP) is integrated for managing the loads intelligently by using the platform of Smart Grids for Residential Setup. The paper also provides detailed modeling and analysis of respective demands of residential consumers in relation with economic load model for FEDRP. Due to increased customer’s partaking in this program the load on the utility is reduced and managed intelligently during emergency hours by providing fair and attractive incentives to residential clients, thus shifting peak load to off peak hours. The numerical and graphical results are matched for intelligent energy management scenario.

背景：美国国家联合委员会公布的高血压预防、诊断、评估及治疗第七次报告强调了老年（≥60岁）收缩性高血压（systolic hypertension，SH）的重要性。收缩性高血压是指收缩压≥140mmHg，舒张压〈90mmHg。目的：对老年SH患者临床治疗的文献进行系统回顾。资料来源：我们在MEDLiNE上检索了1996～2004年发表的英文文献，查找有关老年人SH的报告，尤其是随机临床试验结果。研究选择及数据提取：使用检索词hypertension（高血压）、systole／systolic（收缩或收缩性）、aged（老年），共筛选出1064项研究。数据综合：临床试验证据强烈支持对SBP≥160mmHg的老年SH患者进行治疗。目前还没有大规模试验在收缩压（SBP）处于140～159mmHg的老年人中评估降压治疗的价值，为这类患者提供的治疗建议主要是基于某些观察性研究，这些研究显示，随着SBP升高，患者心血管病的危险递增。研究强烈支持采用噻嗪类利尿剂和长效钙通道阻滞剂作为治疗SH的一线用药。结论：有证据强烈支持对SBP≥160mmHg的老年SH患者进行治疗。目前尚无充分证据支持将患者SBP控制到140mmHg，也没有充分证据支持对基线SBP处于140—159mmHg的患者进行治疗。决定治疗方案时应更加慎重地考虑患者的意愿和耐受性。
