Short-term outcomes after the loading phase of intravitreal bevacizumab and subthreshold macular laser in non-center involved diabetic macular edema

AiM： To compare the effectiveness of intravitreal bevacizumab and subthreshold macular photocoagulation （SMP）, for the treatment of non-center involved diabetic macular edema （non-Ci DME）. METHODS： Prospective, randomized, controlled clinical study included patients with type 2 diabetes, non-Ci DME and best-corrected visual acuity （BCVA） of 0.30 logMAR or better. Each eye was randomized into three groups： group 1, monthly intravitreal bevacizumab; group 2, single SMP; group 3, single SMP and monthly bevacizumab. Main outcome measures were BCVA, and macular thickness measured with optical coherence tomography as macular central subfield thickness （CST）, macular area of greater thickness （MAGT） and total macular volume （TMV）. Results were analyzed after 3mo. RESULTS： A total of 32 eyes were included. Group 3 improved in BCVA （0.19±0.16 to 0.12±0.14 logMAR; P=0.041） and in TMV （7.90±0.57 to 7.65±0.73 mm3; P=0.025）. Group 1 improved in MAGT （325±26.26 to 298.20±44.85 μm; P=0.022） and TMV （7.79±0.57 to 7.50±0.56 mm3, P=0.047）. Group 2 didn’t show significant improvement of any variable. CONCLUSiON： The loading phase of bevacizumab as monotherapy or combined with SMP is superior to SMP as monotherapy in providing short-term visual and anatomical improvement in non-Ci DME.

Analisys of immunohistochemical Prognostic Markers in Canine Mammary Cancer and its Relation to Postsurgical Survival

Background: Several animal models, including dogs, have been useful to compare the pathogenesis of mammary neoplasm in humans, showing biological parallelism in the growth and development of breast cancer. The causes of cancer could be attributed to change in several tumor suppressor genes. The relationship between molecule associated to senescence and clinical prognosis of patients affected by mammary cancer is little known. Beyond a collection of data, the major interest of the present study was to carry out a clinical follow-up of patients affected by these tumors, through association with new molecular markers by immunohistochemical technic. Upon completing the study, 15 patients survived, while 45 died. in the case of malignant neoplasms, 40 patients died because of the illness. The type of surgery most used by veterinarian surgeons was the simple lumpectomy, followed by the regional mastectomy. Sentinel node was removed by surgery only when clearly affected. Result: Markings against steroid hormones were positive. Regarding the markings against HER2 and Ki-67, they were negative in all cases. The markings against P53 and CD31 were all positives. Markings against molecules associated with cellular senescence were all positives. No statistical differences were found in immunomarcation for the different antigens used as clinical prognosis factors in mammary neoplasms. Conclusions: According to the study conditions, the survival of patients affected by breast tumors is directly related to diagnosis and malignancy histological grade, but not to animal breed, number of affected glands or patient reproductive status. On the other hand, immunohistochemical markings were not related to the patient prognosis. For this reason, it is important to highlight the persistance of a high percentage of mammary neoplasm cases clinically diagnosed with poor results on patient survival. Factors Predictive of Right internal Jugular Vein Cross-Sectional Area Change in Response to Trendelenburg Positioning

Background/Purpose: The right internal jugular vein (RiJV) is the most commonly accessed central venous site in the cardiac operating room. The Trendelenburg position is frequently used to increase the cross-sectional area (CSA) of the RiJV to facilitate its cannulation. However, the extent of change of RiJV CSA in response to Trendelenburg positioning in anesthetized patients and its predictive factors remain unknown. Methods: Thirty-seven patients presented for the cardiac surgery, and 20 ASA i and ii surgical patients without a history of cardiac disease (control) were studied. After induction of anesthesia, RiJV CSA was measured both at supine level position and in 10-degree Trendelenburg using vascular ultrasonography. Central venous pressure was measured in cardiac surgery patients only, since the patients in control group did not require invasive lines placement. Results and Conclusions: Body-surface area, central venous pressure, type of surgery and ejection fraction did not show any correlation with the degree of RiJV CSA change. RiJV dilation in response to Trendelenburg was significantly less pronounced, and more variable, in female patients.

Affective and Behavioural Dysfunctions in Patients with Subcortical Stroke

Objective: To investigate the association of ischaemic and haemorrhagic strokes with the site of subcortical focal cerebrovascular injuries with affective, behavioural and cognitive dysfunctions in the acute phase and after three months. Sample: 58 patients with focal cerebrovascular injuries;mean age 61.5 ± 13.5, 72.4% male. Control group: 20 healthy subjects, mean age 67.1 ± 7.6, 50% male. Results: Significant differences were observed (p = 0.006) between the acute phase and three months later on the apathy items of the Neuropsychiatric inventory (NPi) in patients with subcortical stroke. Significant differences were also observed on the affective scale of the BRMS (p = 0.004), the behaviour scale of the Maudsley Obsessive-Compulsive inventory (MOCi) (p = 0.000), and on the Mini Mental State Examination (MMSE) (p = 0.006). No significant differences were observed on the Hamilton Rating Scale-Depression (HRSM) (p = 0.101). Significant differences were found according to the infarct type: patients with haemorrhagic lesions had higher raw scores on the HRS-D than those with ischaemic lesions (p = 0.024). Conclusions: The performance of patients with subcortical stroke on affective, behavioural and cognitive scales improves after three months. Patients with haemorrhagic lesions are more likely to be depressive than those with ischaemic lesions.

Low and High Risk Human Papillomavirus in the Oral Mucosa of Mexican Women with Genital Papillomavirus

Background: Human papillomaviruses (HPV) are implicated in cervical cancer and, recently in oral cancer. in Mexico, there are few studies on oral cancer, therefore the interest in identifying the HPV frequency of low and high risk in samples of the oral and cervical cavities, and determining some risk factors. Objective: To determine the frequency of high and low risk HPV infection in the oral cavity of women with cervical HPV, and to correlate the infection site with risk factors. Materials and Methods: Eighteen female patients between 24 and 53 years, with antecedents of genital HPV infection were included. Both samples of oral cavity and cervix were obtained. DNA extraction from the epithelial cells was performed using the Qiagen kit. PCR was done and the amplicon was observed in 2% agarose gels stained with ethidium bromide. A correlation of HPV infection and risk factors was done. Results: HPV-DNA was detected in the 67% of both samples. The frequency of oral and cervix low risk HPV-DNA was 50%, while high risk HPV-DNA in oral cavity was detected in 17%, and 39% in the cervix. The study of the risk factors involved in HPV infection showed that the participants had the habits of smoking 39%;alcohol drinking 28%;and 78% oral sex. Conclusion: The results showed a high frequency of HPV (67%) infection in the oral and genital mucosas, suggesting that patient’s habits could contribute to the infection;the presence of HPV in the oral mucosa may act as reservoir for new HPV infections.

Rheological Behaviour, Synthesis and Performance of Smart Thermal Barrier Coating Systems Based on Hollow Alumina

The stability of three water-based slurries containing different loads of Al microparticles was measured by laser scattering. The slurries displayed a Newtonian behaviour and the evolution of viscosity over nine days was found to be constant. Ground surfaces of nickel were also wetted similarly irrespective of the Al content in the slurries (30, 40 or 50 wt%) thereby demonstrating that the 1/10 PVA/H2O water based slurries are adequate for spraying. After deposition and annealing of the as-sprayed coatings, a thermal barrier coating system was formed with a top coat of hollow alumina spheres, an intermediate thermally grown oxide and a bottom aluminized bond coat. The coating system is shown to display very low thermal conductivity and remarkable oxidation resistance at high temperatures.

Cymbidium sp. has several ornamental, medicinal and cosmetic uses. Essential oils were obtained through extraction by hydrodistillation of flower and pedicellate ovary. The chromatographic profile was obtained by thin layer chromatography (TLC), combined with gas chromatography-mass spectrometry (GC/MS), for the identification and relative quantification of various substances in three samples from the orchid (flower, pedicellate ovary and aqueous residue). With TLC, five compounds were identified in the flower, with the most abundant being terpineol, linalool and zingiberene, and five in the pedicellate ovary, with borneol, cineole, and β-bisabolene being in the greatest concentration, while for the aqueous residue, the most abundant was geraniol. Using GC/MS, 25 volatile components associated with the flower were identified, of which the most abundant were linalool (22.92%), 4-methyl-phenol (15.07%) and p-Menth-1-en-8-ol (12.32%). in pedicellate ovaries, 13 components were identified, with the most abundant being 4-methyl-phenol (31.24%), bicycle [4.4.0] dec-1-ene, 2-isopropyl-5-methyl-9-methylene-(17.74%) and 2-hexanone (10.24%), while in the aqueous residue 18 components were found, with the most abundant being 4-methyl-phenol (18.71%) and 2- cyclohexen-1-ol (14.60%).
