Background: The capabi.i.y of obtai.i.g ti.sue samples for hi.tologi. exami.ati.n duri.g EUS has theoreti.al advantages over cytology alone. The objecti.e was to evaluate the feasi.i.i.y and the yi.ld of EUS-gui.ed FNA ti.sue acqui.i.i.n (EUS-FNTA) by usi.g hi.h negati.e pressure sucti.n. Methods: The study desi.n i. a prospecti.e, observati.nal pi.ot study set at a terti.ry referral center. Twenty-seven pati.nts wi.h a soli. mass amenable to sampli.g wi.h EUS were i.cluded i. the study. FNA wi.h a 22-gauge needle was used for a total of 5 passes. An addi.i.nal pass wi.h the same needle was performed by applyi.g conti.uous hi.h negati.e pressure sucti.n usi.g the Alli.nce i. i.flati.n system. The mai. outcome measurements were the rate of ti.sue acqui.i.i.n and the di.gnosti. accuracy of EUS-FNTA. Observati.ns: Ti.sue samples were obtai.ed i. 26 of the 27 pati.nts (96%). Mali.nancy was detected i. 20 of the 26 bi.psy speci.ens obtai.ed by FNTA and i. 20 of the 27 FNA speci.ens. i. 3 pati.nts, EUS-FNTA fai.ed to di.close mali.nancy, whi.h i. two of the pati.nts was di.gnosed by FNA. Conversely, EUS-FNTA di.gnosed a recurrent mali.nant thymoma and a schwannoma i. two FNA-negati.e pati.nts. i. 3 pati.nts wi.h both FNTA and FNA negati.e for mali.nancy, a defi.i.i.e di.gnosi. could not be establi.hed. Overall, di.gnosti. accuracy was 76.9%for both EUS-FNTA and EUS-FNA. When combi.ed, a correct di.gnosi. was achi.ved i. 84.6%of the pati.nts. i.munostai.i.g of the retri.ved ti.sue allowed characteri.ati.n of the pri.ary tumor i. 5 cases and the di.gnosi. of a schwannoma and two neuroendocri.e tumors. Li.i.ati.ns of the study were small sample si.e and a pi.ot study. Conclusi.ns: EUS-FNTA has a hi.h yi.ld for the retri.val of core ti.sue samples. Further studi.s i. whi.h EUS-FNTA i. performed before FNA and wi.h vari.ble number of passes are needed to better defi.e i.s di.gnosti. role and performance characteri.ti.s.

Background and Study Ai.s: Endoscopi. mucosal resecti.n and photodynami. therapy are exci.i.g, mi.i.ally i.vasi.e curati.e techni.ues that represent an alternati.e to surgery i. pati.nts wi.h Barrett’s esophagus and hi.h-grade dysplasi. or i.tramucosal adenocarci.oma. However, there i. lack of uni.ormi.y regardi.g whi.h stagi.g method should be used pri.r to therapy, and some i.vesti.ators even questi.n whether stagi.g i. requi.ed pri.r to ablati.n. We report our experi.nce wi.h a protocol of conventi.nal endoscopi. ultrasound stagi.g pri.r to endoscopi. therapy. Pati.nts and Methods: A total of 25 consecuti.e pati.nts wi.h a di.gnosi. of hi.h-grade dysplasi. or i.tramucosal adenocarci.oma i. Barrett’s esophagus who had been referred to the Uni.ersi.y of Chi.ago for stagi.g i. preparati.n for endoscopi. therapy between March 2002 and November 2004 were i.cluded i. the study. All 25 pati.nts underwent repeat di.gnosti. endoscopy and conventi.nal endosonography wi.h a radi.l echo endoscope. Any suspi.i.us lymph nodes that were detected were sampled usi.g endoscopi. ultrasound-gui.ed fi.e-needle aspi.ati.n. Results: Baseli.e pathology i. the 25 pati.nts (mean age 70, range 49- 85) revealed hi.h-grade dysplasi. i. 12 pati.nts and i.tramucosal carci.oma i. 13 pati.nts. Fi.e pati.nts were found to have submucosal i.vasi.n on conventi.nal endosonography. Seven pati.nts had suspi.i.us adenopathy, si. regi.nal (N1) and one metastati. to the celi.c axi. (M1a). Fi.e-needle aspi.ati.n confi.med mali.nancy i. fi.e of these seven pati.nts. Based on these results, fi.e pati.nts (20% ) were deemed to be unsui.able candi.ates for endoscopi. therapy. Conclusi.ns: By detecti.g unsuspected mali.nant lymphadenopathy, conventi.nal endosonography and endoscopi. ultrasound wi.h fi.e-needle aspi.ati.n dramati.ally changed the course of management i. 20% of pati.nts referred for endoscopi. therapy of Barrett’s esophagus wi.h hi.h-grade dysplasi. or i.tramucosal carci.oma. Endoscopi. treatment of Barrett's esophagus:From metaplasi. to i.tramucosal carci.oma

The annual i.ci.ence of adenocarci.oma ari.i.g from Barrett’s esophagus (BE) i. approxi.ately 0.5%. Through a process of gradual transformati.n from lowgrade dysplasi. to hi.h-grade dysplasi. (HGD), adenocarci.oma can develop i. the setti.g of BE. The cli.i.al i.portance of appropri.te i.enti.i.cati.n and treatment of BE i. i.s vari.us stages, from i.testi.al metaplasi. to i.tramucosal carci.oma (i.C) hi.ges on the dramati.ally di.ferent prognosti. status between early neoplasi. and more advanced stages. Once a pati.nt has symptoms of adenocarci.oma, there i. usually locally advanced di.ease wi.h an approxi.ate 5-year survi.al rate of about 20%. Esophagectomy has been the gold standard treatment for BE wi.h HGD, due to the suspected ri.k of harbori.g occult i.vasi.e carci.oma, whi.h was tradi.i.nally esti.ated to be as hi.h as 40%. i. recent years, the paradi.m of BE early neoplasi. management has recently evolved, and endoscopi. therapi.s (endoscopi. mucosal resecti.n, radi.frequency ablati.n, and cryotherapy) have entered the cli.i.al forefront as acceptable non-surgi.al alternati.es for HGD and i.C. The goal of endoscopi. therapy for HGD or i.C i. to ablateall BE epi.heli.m (both dysplasti. and non-dysplasti.) due to ri.k of synchronous/metachronous lesi.n development i. the remai.i.g BE segment.

Ai.: To perform a quali.y control(QC) revi.w of endoscopi. ultrasound(EUS) wi.h emphasi. on current consensus establi.hed quali.y i.di.ators. METHODS: A nati.nal quali.y control study of EUS was performed wi.h expanded i.ternati.nal compari.on. Ten di.ferent healthcare i.sti.uti.ns i. i.rael parti.i.ated i. coordi.ati.n wi.h Uni.ersi.y of Chi.ago Medi.al Center. Each i.raeli.center provi.ed ten pati.nt reports, compared wi.h twenty reports from Uni.ersi.y of Chi.ago Medi.al Center. Quali.y i.di.ator forms were prepared wi.h secti.ns to be completed before, duri.g, and after EUS. Physi.i.n compli.nce to all li.ted i.di.ators wasevaluated. Quali.y i.di.ators were evaluated pri.r to, duri.g, and after performi.g EUS. RESULTS: One hundred di.ferent EUS procedural reports were analyzed. The mean pati.nt age was 59 years old. i.di.ati.ns for referral were mostly for pancreati. or bi.i.ry reasons. QC showed several strongly reported areas, i.cludi.g i.di.ati.ns for EUS(97%), anesthesi. gi.en(94%), peri.rocedural pancreati. evaluati.n(87%), and an overall summary of the EUS exami.ati.n(82%). i.termedi.tely reported areas i.cluded pati.nts' perti.ent past medi.al hi.tory(71.7%), evaluati.n of the bi.i.ry tree(63%), and provi.i.g medi.al gui.ance about potenti.l procedural adverse events, i.cludi.g pancreati.i. and bleedi.g(52%). Half of the reports(50%) di. not i.clude a systemi. organ evaluati.n. Other areas, i.cludi.g systemati. reporti.g of screened organs(36%), descri.ti.n of fi.e needle aspi.ati.n(15%), tumor descri.ti.n vi. tumor-nodemetastasi.(5%), and li.ti.g of adverse events(0%) were largely lacki.g from procedural documentati.n. CONCLUSi.N: Documenti.g speci.i. EUS quali.y i.di.ators i.cludi.g li.ti.g post-procedural recommendati.ns may i.prove the quali.y and effi.i.ncy of future EUS exami.ati.ns and subsequent pati.nt follow-up.

OBJECTi.ES: The ai.s of thi. study were to determi.e whether pattern of pati.nts presenti.g wi.h colorectal cancer (CRC) i. the last few years di.fers si.ni.i.antly from that previ.usly reported i. Australi., and to relate the trends, i. present, to use of hormone replacement therapy (HRT). METHODS: We exami.ed demographi. and pathologi.al characteri.ti.s of 145 consecuti.e CRC pati.nts (65 females) treated i. our i.sti.uti.n i. calendar years 2006-2007. Compari.ons were made wi.h data on 12536 CRC pati.nts obtai.ed from the Australi.n Associ.ti.n on Cancer Regi.tri.s (AACR) for the year 2003, most recent avai.able. Prescri.i.g data for HRT were obtai.ed from the Australi.n Commonwealth Department of Health and Agei.g. RESULTS: The di.tri.uti.n of colon, si.moi. and rectal cancers i. our seri.s was 40%, 24.8% and 35.2%, respecti.ely, whi.h di.fers si.ni.i.antly from 65%, 8.1% and 26.9% i. the AACR data (p i.ni.i.antly younger (65.4 ± 12.1 vs. 69.5 ± 12.3 years), especi.lly females (63.0 ± 12.7 vs. 70.3 ± 13.0 years;p i.n of female pati.nts aged i.ni.i.antly hi.her (30.8% vs. 13.8% and 41.5% vs. 21.4%, respecti.ely). Younger pati.nts have more aggressi.e and advanced cancers. i. Australi. HRT use decli.ed si.ce 2001 and fell by a half i. 2006. CONCLUSi.NS: i. the changi.g CRC pattern of greatest concern i. a si.ni.i.antly hi.her proporti.n of younger pati.nts, especi.lly females, wi.h hi.her prevalence of more advanced and aggressi.e cancers, coi.ci.ent wi.h decreased prescri.i.g of HRT. These fi.di.gs may have i.portant i.pli.ati.ns for refi.i.g screeni.g and preventi.e strategi.s and on demand for radi.therapy servi.es.
