Ataxic gait following total gastrectomy for gastric cancer

A 58-year-old woman, who had undergone total gastrectomy for early gastric cancer 9 years previously, visited the outpatient clinic complaining of progressive difficulty in walking for 15 d. Laboratory examinations showed macrocytic anemia and a decreased serum vitamin B12 concentration and increased serum concentrations of folate, vitamin E and copper. Magnetic resonance imaging showed multifocal high signal intensities along the posterior column of the cervical and thoracic spinal cord. Treatment consisted of intramuscular injections of vitamin B12 for 7 d, which increased her serum level of vitamin B12 to normal. This was followed by weekly intramuscular injections of vitamin B12 for another 2 wk and oral administration of vitamin B12 three times per day. After comprehensive rehabilitation for 4 wk, she showed sufficient improvements in strength and ataxic gait, enabling her to return to her normal daily activities.

Though the WiMedia supports higher data rates than other WPAN technologies,a WiMedia device cannot reach other devices that are separated from it by more than 10 m,which is the typical limited transmission range of the WiMedia *** this work,we propose a multi-hop QoS routing protocol to enable WiMedia devices to transmit real-time data to devices that are located out of the transmission *** proposed routing protocol is a hybrid algorithm,which mixes the table-driven and on-demand routing algorithms,searching one or more routes according to the number of hops to a destination *** MAC is potentially capable of learning the existence of neighbor devices by using a beacon *** utilizing the neighbors' information,all devices can create routing entries for devices within 2-hops *** devices beyond the 2-hop range,the newly designed on-demand routing algorithm is applied to multi-hop *** a routing entry for a destination device is not listed in the routing table,the source device sends a request packet to 2-hop range devices which could be found in the routing *** every device maintains routing entries for 2-hop range devices in the routing table,the request packet is replied in advance before its arrival at the destination ***,to decide the optimal route for a destination device,the number of medium access slots(MASs),received signal strength indicator(RSSI)and hop count are utilized to establish a QoS-enabled routing *** perform ns-2 simulation to investigate the performance of the proposed routing protocol with AODV and *** simulation results show that the proposed protocol has better throughput and lower overhead than other protocols.

Beta-nerve growth factor(β-NGF) is known to be a major leading cause of neuronal plasticity. To identify the possible action mechanisms of β-NGF gene therapy for sciatic nerve recovery, experimental dogs were randomly divided into control, pyridoxine, and pyridoxine + β-NGF groups. We observed chronological changes of morphology in the dorsal root ganglia in response to pyridoxine toxicity based on cresyl violet staining. The number of large neurons positive for cresyl violet was dramatically decreased after pyridoxine intoxication for 7 days in the dorsal root ganglia and the neuron number was gradually increased after pyridoxine withdrawal. In addition, we also investigated the effects of β-NGF gene therapy on neuronal plasticity in pyridoxine-induced neuropathic dogs. To accomplish this, tyrosine kinase receptor A(TrkA), βIII-tubulin and doublecortin(DCX) immunohistochemical staining was performed at 3 days after the last pyridoxine treatment. TrkA-immunoreactive neurons were dramatically decreased in the pyridoxine group compared to the control group, but strong TrkA immunoreactivity was observed in the small-sized dorsal root ganglia in this group. TrkA immunoreactivity in the dorsal root ganglia was similar between β-NGF and control groups. The numbers of βIII-tubulin-and DCX-immunoreactive cells decreased significantly in the pyridoxine group compared to the control group. However, the reduction of βIII-tubulin-and DCX-immunoreactive cells in the dorsal root ganglia in the β-NGF group was significantly ameliorated than that in the pyridoxine group. These results indicate that β-NGF gene therapy is a powerful treatment of pyridoxine-induced neuropathic damage by increasing the TrkA and DCX levels in the dorsal root ganglia. The experimental protocol was approved by the Institutional Animal Care and Use Committee(IACUC) of Seoul National University, South Korea(approval No. SNU-060623-1, SNU-091009-1) on June 23, 2006 and October 9, 2009, respectively.

Parameter identification of Pasternak foundation models(PFM)is never satisfactory,which discourages the application and popularization of *** the present study,an energy-based model to predict the dynamic foundation coefficients was proposed using the vibration kinetic energy and potential energy of a Pasternak foundation-rigid plate *** the basis of the Pasternak foundation,the relationship among the natural frequency,dynamic foundation coefficients,rigid plate configuration,and vibrating soil equivalent mass per unit area was *** obtain the natural frequencies of the Pasternak foundation-rigid plate system,dynamic tests were *** two or more dynamic test results of various rigid plates on a foundation,a set of equations of dynamic foundation coefficients was set up to directly identify the foundation coefficients and equivalent mass per unit area of vibrating *** feasibility of the proposed method was verified by comparing it with the outdoor and indoor test results and finite element analysis *** the proposed method is used to obtain the dynamic parameters,PFM can be generalized and applied more widely in engineering practice.
