The Impact of Intramuscular Depot Betamethasone Injection (Diprospan) on Patients with Fibromyalgia and Normal C-Reactive Protein Levels

Introduction: Intramuscular depot betamethasone treatment had resulted in a significant improvement among fibromyalgia patients with elevated C-reactive protein (C-RP) levels. Here, we wanted to evaluate the same regimen of treatment among fibromyalgia patients with normal C-RP levels. These patients represent the overwhelming majority of fibromyalgia patients. Patients and Methods: Consecutive patients with fibromyalgia attending the outpatient rheumatology clinic, with normal C-RP level and negative serology, who had failed different medical treatment, were asked to participate in our study. All patients have qualified the American College of Rheumatology (ACR) criteria from 2010. After consent, patients had an intra-muscular injection of 14 mg depot betamethasone at the gluteal area. Just prior to the injection, 1 week and 1 month later, patients were interviewed by phone and asked to answer the Fibromyalgia Revised Questionnaire (FIQR). Wilcoxon’s signed ranked test was used to compare the results 1 week and 1 month following the injection, compared to the base line scores. Results: Seventeen (17) patients completed the study. Favorable effects were seen regarding 13 out of 19 parameters one week following the injection, including functional parameters, mood and anxiety, tenderness to touch and intolerance to noise and light. No significant favorable effect was seen 1 month following the injection except for one parameter: ability of walking for twenty minutes. Conclusions: IM depot betamethasone injection had very limited and transient favorable effects on fibromyalgia patients with normal C-RP levels. Such a treatment is not a recommended modality of routine treatment, among fibromyalgia patients with normal C-RP levels.

Toward simple,generalizable neural networks with universal training for low-SWaP hybrid vision

Speed,generalizability,and robustness are fundamental issues for building lightweight computational *** we demonstrate generalizable image reconstruction with the simplest of hybrid machine vision systems:linear optical preprocessors combined with no-hidden-layer,"small-brain" neural networks. Surprisingly,such simple neural networks are capable of learning the image reconstruction from a range of coded diffraction patterns using two masks. We investigate the possibility of generalized or "universal training" with these small brains. Neural networks trained with sinusoidal or random patterns uniformly distribute errors around a reconstructed image,whereas models trained with a combination of sharp and curved shapes (the phase pattern of optical vortices) reconstruct edges more boldly. We illustrate variable convergence of these simple neural networks and relate learnability of an image to its singular value decomposition entropy of the image. We also provide heuristic experimental results. With thresholding,we achieve robust reconstruction of various disjoint datasets. Our work is favorable for future real-time low size,weight,and power hybrid vision:we reconstruct images on a 15 W laptop CPU with 15,000 frames per second:faster by a factor of 3 than previously reported results and 3 orders of magnitude faster than convolutional neural networks.

Constitutive equations for melts and concentrated solutions of linear polymers are derived as consequences of dynamics of a separate macromolecule. The model is investigated for viscometric flows. It was shown that the model gives a good description of non-linear effects of simple shear polymer flows: viscosity anomalies, first and second normal stresses, non-steady shear stresses.

Normalizing the Neural Correlates of Binge-Eating Disorder with Lisdexamfetamine: A Case Report and Literature Review

The brain regions associated with binge-eating disorder (BED) and those targeted by Lisdexamfetamine (LDX) overlap. The present paper reviews the psychobehavioural and neuropharmacological mechanisms at play in BED and compulsive eating behaviours, along with the mechanisms of action LDX is thought to play. We present one case of chronic BED and highlight the EEG scans pre- and post-LDX treatment. The normalization of the patient’s electroencephalogy (EEG) activity associated with BED after LDX supports theorized mechanisms of LDX action, further explaining its efficacy in treatment. A significant decrease in theta band power was observed in the patient after treatment with LDX. Specifically, this decrease was modulated by changes in the F8 electrode corresponding to the orbitofrontal cortex (OFC)—An area associated with deficits in inhibitory function, seen in addictions and BED. A better understanding of the psychobehavioural and neuropharmacological mechanisms involved in BED and LDX treatment has the potential to significantly enhance our knowledge of the underlying pathogenesis, potential prevention, and optimal treatment options for patients.

Longitudinal monitoring for the emergence of epidermal growth factor C797S resistance mutations in non-small cell lung cancer using blood-based droplet digital PCR

Validation of assays for the C797S mutation as a biomarker for osimertinib resistance is promising in guiding treatment decision-making for multidrug resistant non-small cell lung cancer. A newly developed droplet digital PCR (ddPCR) assay was used to retrospectively evaluate the emergence of the C797S mutation in six remnant plasma samples in this case report. It was found that the detected emergence of C797S clearly correlated with clinical signs of treatment resistance. Had these data been available to aid treatment selection in real time, there would have been hope for recaptured disease response and control instead of treatment cessation. The results of this study show that highly sensitive ddPCR methods can be used for the monitoring of emergent epidermal growth factor somatic variant mutations in circulation.

Comparison of Wheat Planting Methods and Residue Incorporation Under Saline-Sodic Soil

The present research was conducted to monitor the wheat productivity along with residue incorporation under saline-sodic soils by examining different planting methods at Zaidi Farm, Kakar Gill, Sheikhupura District, Punjab Province in 2007-2008. Randomized Complete Block Design （RCBD） with three replications was used having treatments： control （broadcast）, zero till wheat plantation, wheat plantation using happy seeder and wheat plantation on raised beds. It was observed that tillering was pretty higher （141 and 139 m2） under raised bed as well as happy seeder plantation as compared to zero tilled wheat and broadcast technique. A significant relation was detected among maximum straw and grain yield （4,898 and 1,752 kg-ha1） in raised bed followed by happy seeder planting method. The lowest grain yield was recorded in the broadcast method. Maximum net revenue earned by raised bed planting method （39,908 PKR） followed by happy seeder methodology （37,533 PKR）. The overall study suggests that raised bed and happy seeder wheat plantation are the superior planting methods.

Spatial-Aware Supervised Learning for Hyper-Spectral Image Classification Comprehensive Assessment

A comprehensive assessment of the *** mpervised learning algorithms for *** image （HSI） classification was presented. For this purpose, standard support vector machines （ SVMs ）, mudttnomial logistic regression （ MLR ） and sparse representation （SR） based supervised learning algorithm were compared both theoretically and experimentally. Performance of the discussed techniques was evaluated in terms of overall accuracy, average accuracy, kappa statistic coefficients, and sparsity of the solutions. Execution time, the computational burden, and the capability of the methods were investigated by using probabilistie analysis. For validating the accuracy a classical benchmark AVIRIS Indian pines data set was used. Experiments show that integrating *** context can further improve the accuracy, reduce the misclassltication error although the cost of computational time will be increased.

Soil Quality of a Semi-Arid Pasture Irrigated with Reverse Osmosis Wastewater—A Case Study from Northern New Mexico

Soil quality indicators were assessed in two adjacent fields in northern New Mexico near a reverse osmosis (RO) facility. One field had been cleared of native vegetation, sowed with a pasture mix and irrigated with saline RO wastewater (electrical conductivity (EC) of 2.73 dS/m) (WW) for two years. An adjacent field of non-irrigated, undisturbed native vegetation (NV) that received only natural rainfall was sampled for comparison and assumed to be representative of baseline values. Measurements included mean weight diameter (MWD), dry aggregates > 2 mm (D > 2 mm), dry aggregates < 0.25 mm (D < 0.25 mm), wet aggregate stability, permanganate oxidizable carbon (POXC), soil organic matter, EC, pH, sand, silt and clay contents, and chemical parameters (NO3-N, P, Ca, Mg, Na, Zn, Fe, Mn and Cu). The wastewater irrigated field had more favorable soil quality indicators than the non-irrigated field, presumably due to the pasture mix and irrigation. However, the EC is higher in the WW irrigated field and will affect long-term utilization of the land for cropping, unless good soil salinity management is implemented.
