Is convenience really king? Comparative evaluation of catastrophic costs due to tuberculosis in the public and private healthcare sectors of Viet Nam: a longitudinal patient cost study

background In Viet Nam, tuberculosis (TB) represents a devastating life-event with an exorbitant price tag, partly due to lost income from daily directly observed therapy in public sector care. Thus, persons with TB may seek care in the private sector for its flexibility, convenience, and privacy. Our study aimed to measure income changes, costs and catastrophic cost incurrence among TB-affected households in the public and private *** Between October 2020 and March 2022, we conducted 110 longitudinal patient cost interviews, among 50 patients privately treated for TB and 60 TB patients treated by the National TB Program (NTP) in Ha Noi, Hai Phong and Ho Chi Minh City, Viet Nam. Using a local adaptation of the WHO TB patient cost survey tool, participants were interviewed during the intensive phase, continuation phase and post-treatment. We compared income levels, direct and indirect treatment costs, catastrophic costs using Wilcoxon rank-sum and chi-squared tests and associated risk factors between the two cohorts using multivariate *** The pre-treatment median monthly household income was significantly higher in the private sector versus NTP cohort (USD 868 vs USD 578;P=0.010). However, private sector treatment was also significantly costlier (USD 2075 vs USD 1313;P=0.005), driven by direct medical costs which were 4.6 times higher than costs reported by NTP participants (USD 754 vs USD 164;P<0.001). This resulted in no significant difference in catastrophic costs between the two cohorts (Private: 55% vs NTP: 52%;P=0.675). Factors associated with catastrophic cost included being a single-person household [adjusted odds ratio [(aOR=13.71; 95% confidence interval (CI): 1.36–138.14;P=0.026], unemployment during treatment (aOR=10.86; 95%CI: 2.64–44.60;P<0.001) and experiencing TB-related stigma (aOR=37.90; 95%CI: 1.72–831.73;P=0.021).Conclusions Persons with TB in Viet Nam face similarly high risk of catastrophic costs whether treated in the

Electro-hydraulic actuators(EHA)have recently played a signiﬁcant role in modern industrial applications,especially in systems requiring extremely high *** can be explained by EHA’s ability to precisely control the position and force through advanced sensors and innovative control *** of the promising approaches to improve control accuracy for EHA systems is applying classical to modern control algorithms,in which the proportional–inte-gral–derivative(PID)algorithm,fuzzy logic controller,and a hybrid of these methods are popular *** this paper,we developed a novel version of the fuzzy control algorithm and linear feedback control method,namely fuzzy lin-ear feedback control,to improve the control *** achieve the highest performance,weﬁrst designed a mathematical EHA model based on the Matlab/Simulink software packages thanks to the selected parameters,which are similar to a real EHA ***,we respectively applied PID,fuzzy PID(FPID),and fuzzy linear feedback control(FLFC)before comparing them to have a full view of the outstanding advantages of the proposed *** simulation results showed that the proposed FLFC algorithm is approximately 99%and 77%super-ior in performance to the PID and feedback control algorithms,respectively.

Effects of NaOH Concentrations on Properties of the Thermal Power Plant Ashes-Bricks by Alkaline Activation

The use of the thermal power plant ashes including fly ash(FA) and bottom ash(ba) for producing unfired building bricks(UBB) using sodium hydroxide(NaOH) solution as an alkaline activator was investigated. A low applied forming pressure of 0.5 MPa and various NaOH concentrations of 5, 8, 10, and 12 M were used for the preparation of brick samples with different solution-to-binder(S/B) ratios of 0.35 and 0.40. The bricks were subjected to various test programs with reflecting the effect of both NaOH concentrations and S/B ratios on the brick’s properties. The compressive strength, unit weight, ultrasonic pulse velocity, and thermal conductivity of bricks increased with increasing NaOH concentration, whereas the contrary trend was found with increasing S/B ratio. Also, the water absorption of bricks was observed to reduce with increasing NaOH concentration and decreasing S/B ratio. As the results, the combined utilization of both low forming pressure and coal power plant ashes can produce the UBBs with low unit weight, low heat conductivity, and acceptable strength and water absorption rate as stipulated by TCVN 6477-2016. Furthermore, the outcomes of chemical analysis and microstructure observation also demonstrate that a high concentration of the Na OH promoted the geopolymerization process. Notably, the use of NaOH solution of either 10 M or above is recommended for the production of UBBs, which are classified as grade M5.0 or higher.

To the Editor:Recent randomized controlled trials,as well as high-quality systematic reviews and meta-analyses,strongly support the use of corticosteroids in patients with coronavirus disease 2019(COVID-19)who require oxygen support.[1]The use of pulse cor-ticosteroid therapy has been reported in case series and studies,showing significant improvement in patients and a reduction in mortality rate.[2]In this letter,we report the mortality rate among patients with severe COVID-19 receiving pulse corticos-teroids with methylprednisolone(500 mg/day)for≥3 days and≥24 h before intubation(median duration of therapy:3 days)compared with standard of care.
