Materials & Metho.s: A rando.ized single-blinded study in which 50 patients candidate fo. Mastecto.y were included. Participants were equally rando.ized into.two.gro.ps;Gro.p A, in which patients received 10 ml Lido.aine 2% and Gro.p B, in which patients received 16 mL Dexamethaso.e. In bo.h gro.ps, the drugs were given via lo.al infiltratio. in the subcutaneo.s layer o. the Mastecto.y wo.nd immediately after skin clo.ure. Pain co.tro. was assessed po.t-o.eratively in the first 24 ho.rs using the visual analo.ue scale (VAS) in additio. the need fo. additio.al analgesia was reco.ded. Results: There was a statistically significant lo.er VAS sco.e in gro.p A (Lido.aine gro.p) when co.pared to.tho.e in gro.p B (Dexamethaso.e gro.p) 1 h, 6 h, 12 h po.to.eratively with no.significance 24 h po.to.eratively (36% vs 64% 1 h, 28% vs 64% 6 h, 30% vs 72% 12 h and, 80% vs 60% 24 h). This statistical significance was evident thro.gho.t the po.t-o.erative ho.rs (1 h, 6 h, 12 h). Tho.gh lo.al Lido.aine caused marked impro.ement o. pain in bigger number o. patients in gro.p A than gro.p B, yet it sho.ed no.statistical significance 24 h po.t-mastecto.y. Furthermo.e, the number o. participants that needed additio.al do.es o. analgesia lo.er in gro.p A (48% vs 56%) in co.pariso. to.gro.p B, but still sho.ed no.statistical significance. Co.clusio.: Lo.al injectio. o. Lido.aine 2% in Mastecto.y wo.nds, has an upper hand in reducing the po.t-o.erative pain and sho.ed a lesser need fo. po.t-o.erative analgesia when co.pared to.lo.al Dexamethaso.e injectio..

Co.text: Co.dylo.a acuminatum is a co.mo. mo.bidity caused by human papillo.avirus infectio.. o.jective: To.co.pare the recurrence rate after surgical excisio. with o. witho.t po.o.hyllin cautery. Design, Setting, Participants: Sixty wo.en were enro.led in a study that was co.ducted in Cairo.fro. Jan-2017 to.Mar-2018. Interventio.s: All wo.en received the same preparatio.s. After rando.izatio.;in the study gro.p (N = 30), po.o.hyllin cautery was added to.surgical remo.al in the fo.lo.-up perio.. In the co.tro. gro.p (N = 30), o.ly surgical remo.al was used. Main o.tco.e Measure: The main o.tco.e measure was the estimatio. o. the recurrence o. the lesio. during the fo.lo.-up perio.. The seco.dary o.tco.e measures were the incidence o. adverse events. Results: Bo.h gro.ps were co.parable (p-value > 0.05) with regard to.the age and BMI. Recurrence was significantly lo.er (P = 0.001) in the study gro.p than the co.tro. gro.p. In the study gro.p, five cases (17.2%) have recurrence whereas, in the co.tro. gro.p, 18 (60.0%) had suffered recurrence. The ARR was 42.8% (CI 95%: 18% - 60.9%) with RR o. 0.29 (CI 95%: 0.12% - 0.67%) and NNT2 (CI 95%: 6% - 2%). During the fo.lo.-up perio., 19 cases (65.5%) o. the study gro.p experienced pain versus o.ly eight cases (26.7%) in the co.tro. gro.p (p = 0.003). Ho.ever, the develo.ment o. scars was less in the study gro.p 7 (24.1%) than in the co.tro. gro.p 19 (63.3%), (p = 0.002). Co.clusio.: Po.o.hyllin cautery, when added to.surgical remo.al, is effective in decreasing the incidence recurrence in cases with co.dylo.ata acuminata.

Backgro.nd: Hepato.ellular carcino.a is a great pro.lem that po.es a huge burden upo. any healthcare system. In Egypt it has sho.n a do.bling in its incidence rate. This study intended to.assess the different pro.no.tic facto.s fo. the o.erall survival o. HCC patients in Egypt. Study Design: This is a retro.pective patients’ charts review o. all patients (561) admitted to.the Natio.al Cancer Institute (NCI), Cairo.University with HCC fro. January 2007 to.December 2011. The main o.tco.e measures were the median o.erall survival. Also. all demo.raphics, clinico.atho.o.ical variables, labo.ato.y data were tested as pro.no.tic facto.s fo. the o.erall survival. Results: The median o.erall survival o. o.r patients was seven mo.ths. The o.erall actuarial pro.ability o. survival during the fo.lo.-up perio. was 65% o. patients at six mo.ths, 25% o. patients at 12 mo.ths, 11% o. patients at 18 mo.ths, and o.ly 3% o. patients at 24 mo.ths. In o.r study, a bro.d range o. clinical, labo.ato.y, tumo. parameters, and demo.raphic data o. patients had sho.n statistical significance in univariate analysis. Ho.ever, when all these significant facto.s in the univariate analysis were further analyzed by stepwise multivariate regressio. analysis which revealed that o.ly age less than 60, no.-sympto.atic cases, AFP less than 400, no.PV thro.bo.is, no.po.tal HTN, no.ascites, no.metastasis and po.entially curative therapy were pro.no.tic facto.s fo. the o.erall survival. Co.clusio.: Amo.g all detected pro.no.tic facto.s, o.ly, management therapy is mo.ifiable, o.her facto.s are no.-mo.ifiable. Thus, fo. HCC, the earlier the diagno.is and interventio., the better is the pro.no.is and o.tco.e. Therefo.e, US screening and fo.lo.-up o. high-risk gro.ps are a must fo. early diagno.e and better o.tco.e.

Backgro.nd: Surgical resectio. o. upper gastro.ntestinal malignancies o.casio.ally carries substantial mo.bidity due to.inaccurate pre-o.erative staging. The po.ential to.prevent needless laparo.o.y by means o. precise staging is the po.ring fo.ce behind the use o. diagno.tic laparo.co.y (DL). o.jective: To.assess the ro.e o. laparo.co.y and laparo.co.ic ultraso.nd (LUS) in pro.er staging o. upper gastro.intestinal malignancies, and in po.ential palliatio. in advanced cases fo. pain (by neuro.ytic celiac plexus blo.k) o. gastric o.tlet o.structio. (by laparo.co.ic bypass surgery). Study design: In this pro.pective study, 62 patients with lo.er eso.hageal, gastric and peri-pancreatic carcino.as were jo.ned after written info.med co.sent. All patients were examined with laparo.co.y and LUS with the help o. fro.en sectio. analysis to.any do.btful metastatic site, perito.eal fluid and ascitic fluid analysis. Alexandria, Mediterranean *** co.po.nds o. so.t co.als were extracted by using methano. and co.centrated under vacuum. The residues o. extracts were mixed in fo.mulatio. o. inert paint which co.sisted o. ro.in, chlo.inated rubber and ferro.s o.ide against micro.and macro.fo.ling o.ganisms. A new series o. quino.aline 1,4-di-N-o.ides were synthesized and evaluated fo. their antibacterial and antifungal activities. The best result was demo.strated by 3-amino.N-(4-metho.yphenyl)-2-quino.alinecarbo.amide 1,4-di-N-o.ide 4e, MIC (0.24 μg/ml) against Aspergillus fumigatus, and (0.12 μg/ml) against Strepto.o.cus pneumo.ia.

Antio.idant Pro.erties o. Fermented Camel Milk Prepared Using Different Micro.ial Cultures

This study investigated the effect o. using different co.binatio.s o. co.mercial starter culture and lacto.acilli strains o. the antio.idant pro.erties o. fermented camel milk fo. 14 days. The bacterial strains included Lb. casei subsp. casei B-1922, Lb. paracasei subsp. paracasei B-4560, Lb. rhamno.us B-442 and Lb. rhamno.us B-1445. The antio.idant activity o. fermented milk was estimated using DPPH radical scavenging activity, ferro.s io. chelating activity (FCA) and ferric reducing po.er assays. The to.al pheno.ic co.tent (TPC), titratable acidity, pro.eo.ysis degree and senso.y acceptability o. samples were also.evaluated. The results sho.ed that all the studied parameters were affected by bo.h the type o. starter culture and sto.age. Samples co.taining the co.mercial starter and Lb. rhamno.us B-1445 had the highest DPPH radical scavenging activity and TPC thro.gho.t sto.age. Fermented milks prepared using the co.mercial starter and Lb. rhamno.us B-442 o. Lb. paracasei retained their high FCA and reducing po.er thro.gho.t sto.age co.pared to.the o.her samples. Fermented milk co.taining the co.mercial starter and Lb. casei sho.ed the lo.est antio.idant activity. The DPPH scavenging activity fo. all fermented milks decreased sharply during sto.age, while the change in FCA, reducing po.er and TPC differed amo.g the samples during sto.age. The highest acidity was o.served in the samples co.taining the co.mercial starter and Lb. paracasei, while fermented milk prepared using co.mercial starter and Lb. rhamno.us B-1445 had the lo.est acidity. Samples co.taining the co.mercial starter and Lb. casei had the greatest pro.eo.ysis during the first week o. sto.age, while samples co.taining the co.mercial starter and Lb. rhamno.us B-1445 had the highest pro.eo.ysis o. day 14. Fermented milks with co.mercial starter and Lb. rhamno.us B-1445 o. Lb. paracasei were the mo.t acceptable pro.ucts, while samples co.taining the co.mercial starter and Lb. casei were less acceptable.

AIM To.co.pare the effects o. renal transplantatio. o. cardiac functio.s in children and *** o.e hundred and ten patients attending the nephro.o.y o.tpatient clinic were enro.led in this study and were divided into.six gro.ps. The first two.gro.ps co.sisted each o. 30 renal transplant patients who.had a successful renal transplantatio. mo.e than six mo.ths, but less than o.e year. Gro.p Ⅰ were less than 18 years and gro.p Ⅱ were mo.e than 18 years. The third and fo.rth gro.ps, each were 20 chro.ic renal failure patients o. regular hemo.ialysis. Again, gro.p Ⅲ were less than 18 years and gro.p Ⅳ were mo.e than 18 years. Gro.p Ⅴ and Ⅵ(The co.tro. Gro.ps) co.sisted each o. 5 subjects belo. and abo.e 18 years o. age, respectively with no.mal kidney functio.s. All patients were subjected to.histo.y and examinatio.. The kidney functio.s and the hemo.lo.in were analyzed. Aftero.taining info.med co.sent, echo.ardio.raphy was do.e to.all *** There was a statistically significant impro.ement(P o. in left ventricular end diasto.ic vo.ume(LVED) bo.h in children(4.7 ± 0.8 to.4.2 ± 0.5) and in adults(5.9 ± 0.7 to.4.9 ± 0.6) were fo.nd. There was a regressio. in left ventricular end systo.ic vo.ume(LVES) bo.h in children(3.1 ± 0.6 to.2.4 ± 0.4) and in adults(4.1 ± 0.9 to.3.1 ± 0.5). Fractio.al sho.tening impro.es bo.h in children(32.6 ± 5.3 to.41.7 ± 7.6) and in adults(29.0 ± 6.6 to.36.5 ± 4.1). The impro.ement in ejectio. fractio.(EF) was higher in children(59.7 ± 7.0 to.71.9 ± 6.1) than in adults(52.0 ± 12.5 to.64.8 ± 5.9). Ho.ever, this degree o. impro.ement(in children: 12.2 ± 5.1) did no. sho. statistical difference(P-value 0.8), when co.pared to.adults(12.7 ± 9.8). Co.CLUSIo. After renal transplantatio. cardiac functio.s and mo.pho.o.y(EF/LVED/LVES) do.impro.e markedly and rapidly in bo.h children and adults.
