Targeted therapies in gastric cancer and future perspectives

Advanced gastric cancer(AGC) is associated with a high mortality rate and, despite multiple new chemotherapy options, the survival rates of patients with AGC remains poor. After the discovery of targeted therapies, research has focused on the new treatment options for AGC. In the last two decades, many targeted molecules were developed against AGC. Currently, two targeted therapy molecules have been approved for patients with AGC. In 2010, trastuzumab was the first molecule shown to improve survival in patients with HER2-positive AGC as part of a first-line combination regimen. In 2014, ramucirumab was the second targeted molecule to improve survival rates and was suggested as treatment for patients with AGC who had progressed after firstline platinum plus fluoropyrimidine with or without anthracycline chemotherapy. Ramucirumab was the first targeted therapy acting as a single agent in patients with advanced gastroesophageal cancers. Although these two molecules were introduced into clinical use, many other promising molecules have been tested in phase Ⅰ-Ⅱ trials. It is obvious that in the near future many different targeted therapies will be in use for treatment of AGC. In this review, the current status of targeted therapies in the treatment of AGC and gastroesophageal junction tumors, including HER(2-3) inhibitors, epidermal growth factor receptor inhibitors, tyrosine kinase inhibitors, antiangiogenic agents, c-MET inhibitors, mammalian target of rapamycin inhibitors, agents against other molecular pathways fibroblast growth factor, Claudins, insulin-like growth factor, heat shock proteins, and immunotherapy, will be discussed.

Heavy Metal Distribution Map in Soil by Using GIS Techniques

A detailed investigation was conducted to understand the contamination characteristics and distributions of heavy metal pollution in terms of contributions of the heavy metal concentrations as mg/kg ofCd, Cr, Cu, Ni, Zn, Pb, Fe and Mn in the urban soil in Eskisehir city center. The amount of these heavy metals were determined from 15 soil sample points collected within urban area and every sample point included 6 separated samples for chemical analyses. The results indicated that concentration values of all metals except Ni and Cr in soils were below the risky limit pollution values which are recommended by Turkey Ministry of Environment and Forestry in some sample points. Spatial distribution maps were created and recoded, in terms of these heavy metals concentrations as contribution to heavy metal pollution in soil, through Geographical Information Systems techniques. As a result, risky areas were modeled in terms of contamination of heavy metal and it is shown that, every different risky area can be interpreted based on buildings in city center quickly and easily.

*** aimed to evaluate the relationships between acne severity, anxiety, depression and disease-specific quality of life in patients with ***.A total of 61 patients with acne vulgaris and 38 healthy volunteers were included in the *** severity was assessed by the Global Acne Grading *** patients were asked to complete the Acne Quality of Life Scale (AQOL), Dermatology Life Quality Index (DLQI) and Hospital Anxiety and Depression Scale (HAD), and healthy controls to fill only the *** mean HAD anxiety subscale (HAD-A) and HAD depression subscale (HAD-D) scores of the patients were significantly higher than those of the *** rates of subjects at risk for anxiety (26.2%) and for depression (29.5%) were significantly higher in the patient group than in the control group (0%and 7.9%, respectively).We found no correlations between acne severity and scores of AQOL, DLQI, HAD-A and *** and DLQI scores were positively correlatedwith HAD-Aand HADD scores in the patient *** patients at risk for anxiety had significantly higher scores on AQOL and DLQI compared to those who were not at *** were no statistically significant differences between the female and male patients with respect to AQOL, DLQI, HAD-A and HAD-D ***-sion.(1) Irrespective of the degree of severity, patients with acne are at increased risk for anxiety and depression compared to the normal population.(2) Acne negatively affects quality of life, and the greater the impairment of quality of life due to acne, the greater the level of anxiety and depression.(3) A greater impairment of dermatologic quality of life seems to put the patient at an increased risk for anxiety disorder.

The aim of this study was to examine the prognostic factors and treatment outcomes of cervical esophageal carcinoma(CEC)patients who underwent definitive chemoradiotherapy(CRT).The clinical data of 175 biopsyconfirmed CEC patients treated with definitive CRT between April 2005 and September 2021 were retrospectively *** prognostic factors predicting overall survival(OS),progression-free survival(PFS),and local recurrence-free survival(LRFS)were assessed in uni-and multivariable *** median age of the entire cohort was 56 years(range:26–87 years).All patients received definitive radiotherapy with a median total dose of 60 Gy,and 52%of the patients received cisplatin-based concurrent *** 2-year OS,PFS,and LRFS rates were 58.8%,46.9%,and 52.4%,respectively,with a median follow-up duration of 41.6 ***’performance status,clinical nodal stage,tumor size,and treatment response were significant prognostic factors for OS,PFS,and LRFS in univariate ***-complete treatment response was an independent predictor for poor OS(HR=4.41,95%CI,2.78–7.00,p<0.001)and PFS(HR=4.28,95%CI,2.79–6.58,p<0.001),whereas poor performance score was a predictor for worse LRFS(HR=1.83,95%CI,1.12–2.98,p=0.02)in multivariable ***-two patients(29.7%)experienced grade II or higher *** this multicenter study,we demonstrated that definitive CRT is a safe and effective treatment for patients with *** radiation doses were found to have no effect on treatment outcomes,but a better response to treatment and a better patient performance status did.

目的:评估中心性浆液性脉络膜视网膜(CSCR)病变中盐皮质激素受体基因-2G/C单核苷酸多态性,以及基因多态性和血浆皮质醇水平的关系。方法:选取60例中心性浆液性脉络膜视网膜病变患者和50例正常人作为研究对象。患者皆患有急性中心性浆液性脉络膜视网膜病变,即浆液性视网膜脱离和视网膜色素上皮脱离或功能障碍(排除其它可能导致渗出的疾病,比如脉络膜新生血管、炎症或浸润病变)。为避免皮质醇水平的昼夜变化,上午8时到10时之间采集外周血样,检测盐皮质激素受体基因多态性(rs2070951)和血浆皮质醇水平。结果:CSCR组的基因型频率分布为G/C(46.6%),G/G(26.7%)和C/C(26.7%)。两组间基因型分布无统计学差异(P=0.96)。研究结果显示,CSCR组的血浆皮质醇水平为401.2±162.1 nmol/L,对照组为296.8±130.1nmol/L,两组间差异有统计学意义(P<0.01)。血浆皮质醇水平在G/C(345.0±137.0 nmol/L),G/G(369.2±165.3 nmol/L)和C/C(395.3±188.8 nmol/L)基因型之间不存在差异(P=0.50)。结论:盐皮质激素受体基因多态性与中心性浆液性脉络膜视网膜病变和血浆皮质醇水平无关。

Synapsin Polymorphisms Could Be Correlated with Stroop Simple Reaction Time Scores

Objective: The aim of this study was to research the relationship between Attention Deficit Hyperactivity Disorder (ADHD) and the synapsin III -196G>A and -631C>G polymorphisms and study their impact on neurocognition and behavior in Turkish children and adolescents. Methods: A total of 201 ADHD-diagnosed children and 100 control subjects aged between 8 and 15 years were recruited, and genetic material was obtained from saliva. In the diagnostic assessments, the KSADS- PL semi-structured interview was applied. Children with any comorbid psychiatric diagnosis (with the exclusion of oppositional defiant disorder (ODD)), medical conditions, prior psychotropic drug use history or IQ score below 80 were excluded. For the behavioral and ADHD symptom assessments, the Turgay DSM-IV Disruptive Behaviors Rating Scale, Teacher Report Form (TRF) and Child Behavior Checklist (CBCL) were completed by the parents and teachers. Neurocognitive profiles were evaluated with the CNS-Vital Signs computerized neurocognitive test battery. Results: No significant difference in ADHD prevalence was observed between subjects with the synapsin III gene -196G>A polymorphism and -631C>G polymorphisms. These polymorphisms were also not associated with subtypes of ADHD. We found a relationship between both polymorphisms and Stroop simple reaction time. Conclusion: Synapsin’s effect could be limited during childhood, but synapsin polymorphisms could be associated with Stroop simple reaction time.

肠易激综合征5-Ht2A受体基因中-1438G/A和102T/C多态性与5-Ht2A基因多态性的关系

Goals: The aim of this study is to investigate whether there were any association between the 102 T/C and-1438 G/A polymorphisms of the 5-HT2A receptor gene and IBS, and abdominal pain, anxiety and depression. Background: Genes involved in serotonin (5-HT)metabolism are good candidates for the pathogenesis of irritable bowel syndrome (IBS). Recently, a silent polymorphism in the 5-HT2A receptor gene was identified that is defined by a T to C transition at position 102. Also, a novel G to A base change at position-1438 of the promoter region has been detected in 5-HT2A receptor gene. Study: Fifty-four patients with IBS diagnosed according to the Rome 1 criteria and 107 healthy individuals were included in the study. PCR was used to amplify a 468-bp (G→ A) and 342-bp (T→ C) fragment of genomic DNA containing the polymorphism. Hospital anxiety and depression scale was used to assess the risk of depression and anxiety. Severity of chronic abdominal pain was determined by visual analogue scale (VAS). Results: It was shown that there was a high incidence of homozygote C allele of the 102T/C polymorphism (22.2% ; OR: 7.89, P =0.04) and homozygote A allele of the-1438 G/A promoter region (37% ; OR: 11.14, P = 0.01) in patients with *** risk of having an anxiety disorder was 83.3% in patients with C/C genotype, which was higher than other allele carrying patients, and overall mean (52.7% ). (x2 = 8.56, P = 0.014). The patients with T/T genotype had a VAS score of 54.93 ± 2.59 mm, which was significantly higher than that of the patients with other genotypes (p1= 0.02, p2 = 0.001). Conclusion: This study suggests that the patients with homozygote C allele of the 102 T/C polymorphism of the 5-HT2A receptor gene, have a high risk of IBS. On the other hand, T/T genotype of 102T/C polymorphism may be associated with more severe pain in patient with IBS.

Although the effectiveness of methotrexate (MTX) in the treatment of psoriasis is very well established, the mechanism of action is poorly understood. It was suggested that the therapeutic effect of MTX in psoriasis might be mediated by inhibition of adhesion molecule expression. The aim of our study was to investigate the different effects of MTX treatment on cell proliferation, inflammatory infiltrate, adhesion molecules, and angiogenesis in psoriasis, and to clarify the mechanism by which MTX exerts its therapeutic effects. Clinical response, the morpho- phenot- ypic changes, epidermal thickness, and mitosis count were analyzed and the expression of CD31 and ICAM- 3, proliferative markers such as Ki- 67, PCNA, were evaluated by immunohistochemical techniques in lesional psoriatic epidermis, before and after the treatment with MTX in ten patients. In posttreatment biopsies a decrease in the degree of epidermal hyperplasia and a significant reduction in the severity of the inflammatory infiltrate (P< 0.05) were observed. In addition,CD31 and ICAM- 3 expression was significantly decreased ondermal cellular infiltrate, (respectively; P< 0.05,P< 0.01). Ki67 and PCNA expression were suppressed concurrently in about 90% of cases (P< 0.01). We suggest that MTX may have an inhibitory effect on an initial integral component of the pathways that lead to psoriasis. Immunopharmacologic intervention in adhesion event has the potential to improve psoriasis. Inhibition of revascularization may be another mechanism of actionof MTX.
