Economic and medical benefits of ultrasound screenings for gallstone disease

AIM:To investigate whether screening for gallstone disease was economically feasible and clinically ***:This clinical study was initially conducted in2002 in Taipei,*** study cohort total included2386 healthy adults who were voluntarily admitted to a regional teaching hospital for a physical *** follow-up screening with ultrasound sonography for gallstone disease continued until December 31,2007.A decision analysis using the Markov Decision Model was constructed to compare different screening regimes for gallstone *** economic evaluation included estimates of both the cost-effectiveness and cost-utility of screening for gallstone ***:Direct costs included the cost of screening,regular clinical fees,laparoscopic cholecystectomy,and *** costs represent the loss of productivity attributable to the patient’s disease state,and were estimated using the gross domestic product for 2011 in *** time intervals in screening for gallstone disease were associated with the reduced efficacy and utility of screening and with increased *** cost per life-year gained(average cost-effectiveness ratio)for annual screening,biennial screening,3-year screening,4-year screening,5-year screening,and no-screening was new Taiwan dollars(NTD)39076,NTD 58059,NTD 72168,NTD 104488,NTD 126941,and NTD 197473,respectively(P<0.05).The cost per quality-adjusted life-year gained by annual screening was NTD 40725;biennial screening,NTD64868;3-year screening,NTD 84532;4-year screening,NTD 110962;5-year screening,NTD 142053;and for the control group,NTD 202979(P<0.05).The threshold values indicated that the ultrasound sonography screening programs were highly sensitive to screening costs in a plausible ***:Routine screening regime for gallstone disease is both medically and economically *** screening for gallstone disease should be recommended.

The massive flow of scholarly publications from traditional paper journals to online outlets has benefited biologists because of its ease to access. However, due to the sheer volume of available biological literature, researchers are finding it increasingly difficult to locate needed information. As a result, recent biology contests, notably JNLPBA and BioCreAtIvE, have focused on evaluating various methods in which the literature may be navigated. Among these methods, text-mining technology has shown the most promise. With recent advances in text-mining technology and the fact that publishers are now making the full texts of articles available in XML format, TMSs can be adapted to accelerate literature curation, maintain the integrity of information, and ensure proper linkage of data to other resources. Even so, several new challenges have emerged in relation to full text analysis, life-science terminology, complex relation extraction, and information fusion. These challenges must be overcome in order for text-mining to be more effective. In this paper, we identify the challenges, discuss how they might be overcome, and consider the resources that may be helpful in achieving that goal.

BACKGROUND Mesenteric ischemia is significantly more common in end-stage kidney disease patients undergoing chronic dialysis than in the general population and is associated with high morbidity and mortality. However, reports on prognostic factors in this population are *** To elucidate the in-hospital outcomes of acute mesenteric ischemia in chronic dialysis patients and to analyze protective factors for *** The case data of 426 chronic dialysis patients who were hospitalized in a tertiary medical center for acute mesenteric ischemia over a 14-year period were retrospectively reviewed. Of these cases, 103 were surgically confirmed, and the patients were enrolled in this study. A Cox regression analysis was used to evaluate the protective factors for *** The in-hospital mortality rate among the 103 enrolled patients was 46.6%.Univariate analysis was performed to compare factors in survivors and nonsurvivors, with better in-hospital outcomes associated with a surgery delay(defined as the time from onset of signs and symptoms to operation) < 4.5 d, no shock, a higher potassium level on day 1 of hospitalization, no resection of the colon, and a total bowel resection length < 110 cm. After 1 wk of hospitalization, patients with lower white blood cell count and neutrophil counts, higher lymphocyte counts, and lower C-reactive protein levels had better in-hospital outcomes. The purpose of this paper is to develop a functional method for designing a series of submerged breakwaters on practical topography. The method is used to verify the feasibility and effectiveness of Bragg breakwaters for coastal protection by using field topography. The first part of this paper provides definitions and procedures needed in the design process for applying the mechanism of Bragg reflection. Next, Bragg breakwaters are designed on the basis of the cross-sectional topography and then on the plane topography of the Mi-Tuo coast by following the proposed process and procedure. Numerical modeling （hsu et al., 2003; Wen and Tsai, 2008） was used as a design and assessment tool. Finally, the effectiveness and feasibility of a Bragg breakwater was assessed by practical cases. Based on the mechanism of Bragg reflection, an optimum layout for a series of submerged breakwaters is proposed to protect the Mi-Tuo coast. The results indicate that the proper layout of a series of submerged creakwaters can achieve the objective of beach protection.

A numerical model, Evolution Equation of Mild-Slope Equation （EEMSE） developed by hsu et al. （2003）, was applied to study the Bragg reflection of water waves over a series of rectangular seabed. Three key parameters of the Bragg reflection including the peak coefficient of primary Bragg reflection, its corresponding relative wavelength, and the bandwidth, have shown to be effective in describing the characteristics of the primary Bragg reflection. The characteristics of the Bragg reflection were investigated under the various conditions comprising number, height, and spacing interval of a series of rectangular seabed. The results reveal that the peak of Bragg reflection increases with the increase of rectangular seabed height and number, the bandwidth and the shift value of the Bragg reflection depend on the increase of the rectangular seabed height as well as the decrease of rectangular seabed number, and the relative rectangular seabed spacing in the rang of 3 and 4 could produce higher Bragg reflection. Finally, a correlative and regressive analysis is performed by use of the calculated data. Based on the results of the analysis, empirical equations were established. Our study results can provide an appropriate choice of a series of rectangular seabed field for a practical design.
