Background: Several large series have established endoscopic transnasal sellar surgery as the procedure of choice for removal of tumors in the sellar area. Although this procedure provides a less invasive approach to the sella, it entails complications such as nasal bleeding, impaired olfactory function, atrophic rhinitis, synechiae, etc. No studies have yet reported potential morbidities such as empty nose syndrome (ENS), although patients have a relatively empty nasal cavity after surgery. Therefore, we sought to verify the percentage of patients who truly met the diagnostic criteria for ENS after endoscopic transnasal sellar surgery, determine the variation between pre- and postoperative scores in each Empty Nose Syndrome 6-Item Questionnaire (ENS6Q) item, and further evaluate the symptoms that may affect the patients the most after surgery. Methods: Between March 2015 and January 2019, eventually 11 patients who underwent extended endoscopic transnasal sellar surgery in Shin Kong Wu Ho-Su Memorial Hospital, a tertiary referral medical center in Taipei, Taiwan, were enrolled. The patients completed the recently validated ENS6Q after surgery. Results: One patient met the objective diagnostic criteria for ENS (score ≥ 11 in ENS6Q). Significant differences were observed in the pre- and postoperative total ENS6Q scores. The pre- and postoperative scores of “nose feeling too open” and nasal crusting showed statistically significant differences. Further, compared with the other items, the postoperative score of nasal crusting increased most obviously, and it may be the most apparent operation-related symptom. Conclusion: This study is the first to report the possibility of developing ENS after endoscopic transnasal sellar surgery. Although the transnasal endoscopic approach is a safe and minimally invasive procedure for the treatment of sellar lesions, possible complications such as ENS should be considered.

Trachypithecus,which currently contains 20 species divided into four groups,is the most speciose and geographically dispersed genus among Asian *** several morphological and molecular studies,however,its evolutionary history and phylogeography remain poorly ***’s langur(Trachypithecus phayrei) is one of the most widespread members of the genus,but details on its actual distribution and intraspecific taxonomy are limited and ***,to elucidate the evolutionary history of Trachypithecus and to clarify the intraspecific taxonomy and distribution of ***,we sequenced 41 mitochondrial genomes from georeferenced fecal samples and museum specimens,including two *** analyses revealed a robustly supported phylogeny of Trachypithecus,suggesting that the *** group branched first,followed by the *** group,and the *** and *** groups most *** four species groups diverged from each other 4.5-3.1 million years ago(Ma),while speciation events within these groups occurred much more recently(1.6-0.3 Ma).Within ***,we found three clades that diverged 1.0-0.9 Ma,indicating the existence of three rather than two *** the phylogenetic species concept and based on genetic,morphological,and ecological differences,we elevate the *** subspecies to species level,describe a new species from central Myanmar,and refine the distribution of the three ***,our study highlights the importance of museum specimens and provides new insights not only into the evolutionary history of *** but the entire Trachypithecus genus as well.

Making use of the high propensity of 2-cyano-2-cycloalkenones to undergo conjugate addition with various carbanions and the high reactivity of the ensuing α -cyano ketone system, a number of new annulation processes have been developed recently in our laboratories. As shown in Eq. 1 (n=1) with a specific example, one such process involves the addition of 3-butenylmagnesium bromide, followed by a palladium (Ⅱ) acetate mediated oxidative cyclization, to facilitate methylenecyclopentane ring formation. This annulation process could be readily extended to effect methylenecyclohexane ring formation (Eq. 1, n=2), using 4-pentenylmagnesinm bromide as the initial reagent, and to install the carbomethoxy-substituted methylenecyclopentane and methylenecyclohexane rings, using the carbanions derived from methyl 4-pentenoate and methyl 5-hexenoate, respectively (Eq. 2). In another annulation process, the addition of the enolate of methyl 5-chloropentanoate is involved initially, and the ring formation is readily effected by an intramolecular alkylation process. A specific example is given in Eq. 3.

目的:探讨淫羊藿总黄酮(EF)对衰老大鼠脾淋巴细胞Th1、Th2、Th3各类细胞因子的调节作用。方法:采用流式细胞仪,监测比较老龄大鼠脾脏淋巴细胞凋亡率;采用Th1、Th2、Th3 cDNA芯片,分别检测正常青年组(YC)、老龄大鼠组(OM)、EF组、NF-KB阻断剂组(PDTC)、PDTC+EF组的Th1、Th2、Th3,各类细胞因子信号差异表达。结果:①老龄大鼠普遍存在淋巴细胞过度凋亡现象,EF可显著抑制这种过度凋亡状态。②YC组,Th1、Th2、Th3类细胞因子之间处于平衡状态(P>0.05);OM组Th1、Th2、Th3类细胞因子表达比YC组增高,呈Th2、Th3类细胞因子优势应答(P<0.05)。③EF组Th1、Th2、Th3类细胞因子均较OM组明显降低(P<0.05),其趋势与YC组近似,并且Th1、Th2、Th3类细胞恢复平衡。④NFKB通路抑制后(PDTC组),Th1、Th2、Th3类细胞因子平衡失调,Th2优势更加明显(P<0.05);而PDTC+EF组,Th1、Th2、Th3各组表达值则较PDTC组下调,其组内Th1和Th2也重新达到一种相对平衡状态。结论:老龄大鼠脾淋巴细胞过度凋亡,Th1、Th2、Th3类细胞因子分泌增高,Th2、Th3类细胞因子优势应答;EF可以抑制淋巴细胞过度凋亡,校正老龄大鼠Th1、Th2、Th3类细胞因子之间的比例失衡,重塑Th1-Th2-Th3细胞免疫调节网络的良性平衡。

目的探讨饮茶与胆道癌和胆石症的关系.方法采用全人群病例对照研究.研究对象为上海市区新发胆道癌患者627例,包括胆囊癌368例,肝外胆管癌191例和壶腹癌68例;按年龄(每5岁1组)频数配对的对照人群959人;同时收集胆石症患者1037例.采用非条件Logistic回归模型,分析饮茶与胆道癌、胆石症的关系. 结果与不饮茶者比较,女性胆囊癌、肝外胆管癌和胆石症组中现仍饮茶者的调整OR分别为0.57(95%CI: 0.34～0.96)、0.53(95%CI: 0.27～1.03)和0.71(95%CI: 0.51～0.99),肝外胆管癌OR值随饮茶年龄的提前及饮茶年限的增加而降低,趋势检验达到显著性水平.男性胆囊癌、肝外胆管癌和胆石症组OR均＜1,但尚无统计学意义.结论饮茶对女性胆囊癌、肝外胆管癌可能具有保护作用,这一保护作用不依赖于胆石症而具有独立性.

The foothill of western Taiwan Island is mainly covered by young sedimentary rock formations, such as the Pliocene Cholan formation, Kueichulin formation, and the Plio-Pleistocene Toukoshan formation Hsiangshan facies. Due to their low strength, those formations are categorized to weak rocks. As the rapid development, in recent years, there are more and more tunnel constructions in this soft rock area. Various difficulties were encountered during constructions;however, most of the countermeasures are still on the empirical side. This study first developed a physical model system to qualitative and quantitative simulate tunneling in soft rock. Then, the finite element software ABAQUS was applied to numerically model the physical model and a real soft rock tunneling case.

BACKGROUND Hepatitis C virus(HCV)genotype 6(HCV-6)infection is prevalent predominantly in Southeast Asia,and the data on the virologic response of HCV-6 to direct-acting antivirals(DAAs)are sparse in people living with human immunodeficiency virus(HIV)(PLWH).AIM To assess the virologic response of HCV-6 to DAAs in *** From September 2016 to July 2019,PLWH coinfected with HCV-6 initiating DAAs were *** investigations were performed at baseline,the end of treatment,and 12 wk *** Of the 349 PLWH included(mean age 48.9 years,82.5%men),80.5%comprised people who inject drugs,18.1%men who have sex with men,and 1.4%*** hepatitis B virus infection was present in 12.3%of the included PLWH,liver cirrhosis 10.9%,hepatocellular carcinoma 0.9%,and previous HCV treatment experience 10.9%.The mean baseline plasma HCV RNA was 6.2 log10 IU/m *** with glecaprevir/pibrentasvir was initiated in 51.9%,sofosbuvir/ledipasvir 41.5%,sofosbuvir/velpatasvir 6.3%,and sofosbuvir/daclatasvir 0.3%.At DAA initiation,antiretroviral therapy containing tenofovir alafenamide was given in 26.4%,tenofovir disoproxil fumarate 34.4%,non-tenofovir alafenamide/tenofovir disoproxil fumarate 39.3%,non-nucleoside reverse-transcriptase inhibitors 30.4%,protease inhibitors 4.0%,and integrase strand transfer inhibitors 66.8%;94.8%of the included patients had CD4 counts≥200 cells/mm3 and 96.0%had plasma HIV RNA<50 copies/m ***,96.8%achieved undetectable plasma HCV RNA(<30 IU/m L)at end of treatment;and 92.3%achieved sustained virologic response 12 wk off-therapy in the intention-to-treat analysis(93.5%in patients receiving sofosbuvir-based DAAs and 91.2%in those receiving glecaprevir/pibrentasvir).CONCLUSION Similar to the observation made in HIV-negative patients,sustained virologic response 12 wk offtherapy with DAAs is high in PLWH coinfected with HCV-6.
