BACKGROUND Angioimmunoblastic T-cell lymphoma(AITL)is a common subtype of peripheral T-cell *** half of patients with AITL may concurrently present with *** numbers of plasma cells in the bone marrow are commonly observed at *** tumors mimic plasma cell myelomas,hindering a conundrum of clinical diagnoses and potentially delaying appropriate *** SUMMARY A 78-year-old woman experienced poor appetite,weight loss of 5 kg,fatigue 2 months before presentation,and shortness of breath 2 d before presentation,but no fever or night *** examination revealed splenomegaly and many palpable masses over the bilateral axillary regions,approximately>2 cm in size,with rubbery consistency and no *** tests revealed anemia and thrombocytopenia,lactate dehydrogenase level of 153 U/L,total protein level of 10.9 g/dL,albumin to globulin ratio of 0.2,and immunoglobulin G level more than the upper limit of 3000 mg/*** free kappa and lambda light chain concentrations were 451 and 614 mg/L,respectively.A pathological examination confirmed the diagnosis of *** initial treatment was the cyclophosphamide,epirubicin,vincristine,and prednisolone *** this treatment,pleural effusion was controlled,and the patient was discharged in a stable condition and followed up in our outpatient *** This report highlights the importance of differentiating reactive plasmacytosis from plasma cell myeloma in patients with hypergammaglobulinemia.A precise diagnosis of AITL requires a comprehensive evaluation,involving clinical,immunophenotypic,and histological findings conducted by a multidisciplinary team to ensure appropriate treatment.

Assessment and Visualization of Ki67 Heterogeneity in Breast Cancers through Digital Image Analysis

The Ki67 index (KI) is a standard clinical marker for tumor proliferation;however, its application is hindered by intratumoral heterogeneity. In this study, we used digital image analysis to comprehensively analyze Ki67 heterogeneity and distribution patterns in breast carcinoma. Using Smart Pathology software, we digitized and analyzed 42 excised breast carcinoma Ki67 slides. Boxplots, histograms, and heat maps were generated to illustrate the KI distribution. We found that 30% of cases (13/42) exhibited discrepancies between global and hotspot KI when using a 14% KI threshold for classification. Patients with higher global or hotspot KI values displayed greater heterogenicity. Ki67 distribution patterns were categorized as randomly distributed (52%, 22/42), peripheral (43%, 18/42), and centered (5%, 2/42). Our sampling simulator indicated analyzing more than 10 high-power fields was typically required to accurately estimate global KI, with sampling size being correlated with heterogeneity. In conclusion, using digital image analysis in whole-slide images allows for comprehensive Ki67 profile assessment, shedding light on heterogeneity and distribution patterns. This spatial information can facilitate KI surveys of breast cancer and other malignancies.

目的评价正念干预对不孕症女性心理健康状况和生活质量的影响。方法系统检索11个中英文数据库(PubMed、Embase、Scopus、Web of Science、The Cochrane Library、CINAHAL、PsycINFO、中国知网、万方、中国生物医学文献数据库和维普数据库)中有关正念干预对不孕症女性心理健康状况和生活质量的随机对照试验,检索时限为建库至2023年11月,并补充检索纳入研究的参考文献和灰色文献。2名研究护士分别进行文献筛选和数据提取,并采用RevMan 5.4软件对纳入文献进行Meta分析。结果纳入14篇文献,共计1078例不孕症女性。Meta分析显示,正念干预可降低不孕症女性焦虑[SMD=-1.80,95%CI(-1.98,-1.62),P<0.001]、抑郁[SMD=-1.52,95%CI(-1.97,-1.08),P<0.001]和压力[SMD=-0.89,95%CI(-1.11,-0.67),P<0.001]得分;提高生活质量中生理功能[MD=14.36,95%CI(12.31,16.40),P<0.001]、生理职能[MD=8.13,95%CI(6.16,10.10),P<0.001]、精力[MD=12.27,95%CI(10.18,14.36),P<0.001]和精神健康维度得分[MD=17.66,95%CI(15.62,19.70),P<0.001]。结论正念干预可作为不孕症女性心理健康和生活质量提升的有效技术,提示在临床实践中可积极开展正念干预,改善患者心理问题和提高生活质量水平。

Neuroregenerafion is a complex topic in neurosci- ence and includes 3 concepts： neurogenesis, neuro- plasticity, and neurorestoration. After injury of the nervous system, axons have the capacity for self-re- pair, regrowth or proliferation. The peripheral ner- vous system is more effective at restoring damaged axons than the central nervous system （CNS）. This is because formation of scar tissue in the CNS in- fluences neural regrowth or synthesis of growth-in- hibiting proteins, thereby preventing reconstruction of a neural circuit （Silver and Miller, 2004; Enciu et al., 2011）. Parkinson＇s disease （PD） and Alzheimer＇s disease （AD） are two most common degenerative diseases of the CNS among the elderly.

Innate immune responses regulate morphogenesis and degeneration:roles of Toll-like receptors and Sarm1 in neurons

The central nervous system is recognized as an immunoprivileged site because peripheral immune cells do not typically enter it. Microglial cells are thought to be the main immune cells in brain. However, recent reports have indicated that neurons express the key players of innate immunity, including Toll-like receptors （TLRs） and their adaptor proteins （Sarml, Myd88, and Trif）, and may produce cytokines in response to pathogen infection. In the absence of an immune challenge, neuronal TLRs can detect intrinsic danger signals and modulate neuronal morphology and function. In this article, we review the recent findings on the involvement of TLRs and Sarml in controlling neuronal morphogenesis and neurodegeneration. Abnormal behaviors in TLR- and Sarml-deficient mice are also discussed.

Comparison between FLO-2D and Debris-2D on the Application of Assessment of Granular Debris Flow Hazards with Case Study

Numerical simulation has been widely applied to the assessment of debris flow hazards. In East Asia and especially Taiwan, the most widely used numerical programs are FLO-2D and Debris-aD. Although these two programs are applied to the same engineering tasks, they are different in many aspects. These two programs were compared according to their fundamental theories, input and output data, computational algorithms and results. Using both programs, the simulations of a real debris flow with abundant granular material induced by landslides at Xinfa village in southern Taiwan are performed for comparison. The simulation results show that Debris- 2D gives better assessment in hazard area delineating and flow depth predicting. Therefore, Debris-2D is better for simulation of granular debris flows.
