Effectiveness of co-Testing in cervical cancer Screening Program in Macao SAR

Background: cervical cancer remains a significant public health concern in Macao SAR despite the implementation of a cervical cancer screening program and HPV vaccination. To improve early detection, Macao SAR introduced HPV DNA testing alongside cytology (co-testing) as the primary screening method in 2019. This study evaluates the effectiveness of co-testing in identifying cervical precancerous lesions (cIN2+) compared to cytology alone. Methods: We conducted a retrospective analysis of women aged 30 - 65 years who participated in the routine cervical cancer screening program in Macao SAR Primary Healthcare centers from 2019 to 2022. Data from over 70,000 women were analyzed, comparing the detection rates of cIN2+ through co-testing and cytology alone. Women with abnormal cytology or positive HPV results were referred for colposcopy. Results: The introduction of co-testing led to a significant increase in the detection of cIN2+, particularly in women with atypical squamous cells of undetermined significance (AScUS) or negative for intraepithelial lesion or malignancy (NILM) cytology results. Between 2019 and 2022, the percentage of women with AScUS/NILM and any high-risk HPV (hrHPV) positive who were diagnosed with cIN2+ after colposcopy were 24%, 13%, 10% and 7.5% respectively. This highlights the ability of co-testing to identify high-risk individuals who would have been missed by cytology alone. Discussion: Our findings demonstrate the effectiveness of co-testing in improving the sensitivity of cervical cancer screening in Macao SAR. The inclusion of HPV DNA testing allows for better risk stratification of women with AScUS/NILM cytology, leading to more targeted referrals for colposcopy and timely detection of precancerous lesions. Effect of Money Supply on Real Output and Price in china

Over the past 30 years, china has achieved remarkable long-term economic growth. Using quarterly data, we study the effects of money supply on real output and inflation in china between 1993 and 2008. To this end, we use money supply shocks afler filtering out the expected component of the money supply. Our findings provide evidence supporting the asymmetric effect of positive and negative money supply shocks on real output and inflation in china. That is, real GDP growth in china responds to negative money supply shocks but not positive money supply shocks. In addition, inflation responds to positive money supply shocks but not negative money supply shocks. We conclude that the People＇s Bank of china＇ s policy of steady monetary growth appears to be appropriate. Our study offers important policy implications for china.

chinese traditional sports culture is an organic part of chinese traditional culture. It originated from the need for national survival and life. Since its inception, the imprint of chinese traditional culture carries the genetic inheritance of the traditional culture of the chinese nation. According to statistics, there are 977 kinds of traditional sports of various ethnic groups in china. compared with other sports, the huge number of traditional sports can meet the various needs of students for sports and attract and extend the outdoor sports time of teenagers. Martial arts, as examples, are highly interesting. Under the background of implementing the Opinions on Further Strengthening the Protection of Intangible cultural Heritage and the Outline for Building a Strong Sports country, the inheritance of "martial arts," as a non-material cultural heritage in sports, is particularly important. Many traditional martial arts sports are not only the intangible cultural heritage of china but also the wisdom crystallization of chinese ancestors.

Automatic fovea detection and choroid segmentation for choroidal thickness assessment in optical coherence tomography

AIM:To develop an automated model for subfoveal choroidal thickness(SFcT)detection in optical coherence tomography(OcT)images,addressing manual fovea location and choroidal contour ***:Two procedures were proposed:defining the fovea and segmenting the *** localization from B-scan OcT image sequence with three-dimensional reconstruction(LocBscan-3D)predicted fovea location using central foveal depression features,and fovea localization from two-dimensional en-face OcT(LocEN-2D)used a mask region-based convolutional neural network(Mask R-cNN)model for optic disc detection,and determined the fovea location based on optic disc relative *** segmentation also employed Mask ***:For 53 eyes in 28 healthy subjects,LocBscan-3D’s mean difference between manual and predicted fovea locations was 170.0μm,LocEN-2D yielded 675.9μ***-2D performed better in non-high myopia group(P=0.02).SFcT measurements from Mask R-cNN aligned with manual ***:Our models accurately predict SFcT in OcT ***-3D excels in precise fovea localization even with high ***-2D shows high detection rates but lower accuracy especially in the high myopia *** both models offers a robust SFcT assessment approach,promising efficiency and accuracy for large-scale studies and clinical use.

central insulin resistance, the diminished cellular sensitivity to insulin in the brain, has been implicated in diabetes mellitus, Alzheimer’s disease and other neurological disorders. However, whether and how central insulin resistance plays a role in the eye remains unclear. Here, we performed intracerebroventricular injection of S961, a potent and specific blocker of insulin receptor in adult Wistar rats to test if central insulin resistance leads to pathological changes in ocular structures. 80 mg of S961 was stereotaxically injected into the lateral ventricle of the experimental group twice at 7 days apart, whereas buffer solution was injected to the sham control group. Blood samples, intraocular pressure, trabecular meshwork morphology, ciliary body markers, retinal and optic nerve integrity, and whole genome expression patterns were then evaluated. While neither blood glucose nor serum insulin level was significantly altered in the experimental or control group, we found that injection of S961 but not buffer solution significantly increased intraocular pressure at 14 and 24 days after first injection, along with reduced porosity and aquaporin 4 expression in the trabecular meshwork, and increased tumor necrosis factor α and aquaporin 4 expression in the ciliary body. In the retina, cell density and insulin receptor expression decreased in the retinal ganglion cell layer upon S961 injection. Fundus photography revealed peripapillary atrophy with vascular dysregulation in the experimental group. These retinal changes were accompanied by upregulation of pro-inflammatory and pro-apoptotic genes, downregulation of anti-inflammatory, anti-apoptotic, and neurotrophic genes, as well as dysregulation of genes involved in insulin signaling. Optic nerve histology indicated microglial activation and changes in the expression of glial fibrillary acidic protein, tumor necrosis factor α, and aquaporin 4. Application effect of case management nursing based on patient safety in patients with prostate cancer

BAcKGROUND Globally,prostate cancer has become a major threat to men's health,with an increasing incidence and causes serious effects on the quality and length of life of *** the rapid development of medical technology,which provides treatments,including surgery,radiotherapy,and endocrine therapy,the treatment of patients with prostate cancer,especially with endocrine therapy,has become a major challenge in clinical treatment owing to the lengthy course of treatment,side effects of drugs,and impact of the disease on the psychological and physiological functioning of the patient,producing poor treatment adherence and a decline in quality of *** the nursing intervention,the anxiety and depression scores of the observation group were significantly lower than those of the control group(Pcore,sexual function,and hormone function were significantly higher than those in the control group(PcONcLUSION case management guidance based on patient safety effectively reduced anxiety and depression in patients undergoing endocrine therapy for prostate cancer and improved their quality of life,treatment compliance,and satisfaction.
