Inferior oblique weakening surgery on ocular torsion in congenital superior oblique palsy

AIM: To investigate changes in fundus excyclotorsion after inferior oblique myectomy or myotomy. METhoDS: The records of 21 patients undergoing strabismus surgery by a single surgeon between 2009 and 2012 were examined. Only patients who had undergone an inferior oblique myectomy or myotomy,with or without horizontal rectus muscle surgery, were evaluated. Digital fundus photographs were obtained,and the angle formed by a horizontal line passing through the optic disc center and a reference line connecting the foveola and optic disc center was measured. Associated clinical factors examined include age at the time of surgery, presence or absence of a head tilt, degree of preoperative vertical deviation,torsional angle, inferior oblique muscle overaction/superior oblique muscle underaction, and surgery laterality. Whether the procedure was performed alone or in combination with a horizontal rectus muscle surgery was also examined. RESULTS: Mean preoperative torsional angle was12. 0 ± 6. 4 °, which decreased to 6. 9 ± 5. 7 ° after surgery(P howed that preoperative torsional angle, degree ofinferior oblique overaction, and age at surgery independently and significantly affected postoperative torsional ***: Mean torsional angle decreased after inferior oblique myectomy. Degree of preoperative torsional angle, inferior oblique overaction, and age at surgery influence postoperative torsional angle.

Medulloepitheliomas are the second most common type of pediatric intraocular tumors. They display clinical characteristics similar to retinoblastomas, such as leukocorias and intraocular endophytic masses, and are extremely difficult to clinically differentiate from retinoblastomas.

Automatic Discontinuity Classification of Wind-turbine Blades Using A-scan-based Convolutional Neural Network

Recent development trends in wind power generation have increased the importance of the safe operation of wind-turbine blades(WTBs). To realize this objective, it is essential to inspect WTBs for any defects before they are placed into operation. however, conventional methods of fault inspection in WTBs can be rather difficult to implement, since complex curvatures that characterize the WTB structures must ensure accurate and reliable inspection. Moreover, it is considered useful if inspection results can be objectively and consistently classified and analyzed by an automated system and not by the subjective judgment of an inspector. To address this concern,the construction of a pressure-and shape-adaptive phased-array ultrasonic testing platform, which is controlled by a nanoengine operation system to inspect WTBs for internal defects, has been presented in this paper. An automatic classifier has been designed to detect discontinuities in WTBs by using an A-scanimaging-based convolutional neural network(CNN). The proposed CNN classifier design demonstrates a classification accuracy of nearly 99%. Results of the study demonstrate that the proposed CNN classifier is capable of automatically classifying the discontinuities of WTB with high accuracy, all of which could be considered as defect candidates.

Novel mechanisms in functional dyspepsia

Functional dyspepsia （FD） is a highly prevalent but heterogeneous disorder in which multiple pathogenetic mechanisms are involved. Although there are many studies that have investigated various pathophysiologic mechanisms, the underlying casual pathways associated with FD remain obscure. The currently proposed pathophysiologic mechanisms associated with FD include genetic susceptibility, delayed as well as accelerated gastric emptying, visceral hypersensitivity to acid or mechanical distention, impaired gastric accommodation, abnormal fundic phasic contractions, abnormal antroduodenal motility, acute and chronic infections, and psychosocial comorbidity. A greater understanding of the abnormalities underlying FD may lead to improved management. The aim of this editorial is to provide a critical overview of current pathophysiologic concepts in functional dyspepsia.

Amelioration effects of traditional Chinese medicine on alcohol-induced fatty liver

AIM： To examine the effects of traditional Chinese medicine （TCM） on alcohol-induced fatty liver in rats. TCM consists of Astragalus membranaceus, Morus alba, Crataegus pinnatifida, Alisma orientale, Salvia miltiorrhiza, and Pueraria lobata. METhoDS： The rats were separated randomly into five groups. One （the CD group） was fed a control diet for 10 wk, another （the ED group） fed an ethanol-containing isocaloric liquid diet for 10 wk, and the last three （the TCM group） were fed an ethanol-containing isocaloric liquid diet for 10 wk and dosed orally with TCM （222 mg/kg.d, TCM222; 667 mg/kg·d, TCM667; and 2 000 mg/kg.d, TCM2000, respectively） weekly during the last 4 wk. RESULTS： ED group developed fatty liver according to lipid profile and liver histological findings. Compared with the control group, liver/body weight, serum triglyceride （TG） and total cholesterol （TC）, liver TG and TC, serum alanine aminotransferase （ALT） and aspartic aminotransferase （AST） significantly increased in the ED group. Whereas, in the rats administered with TCM, liver/body weight, serum TG and TC, liver TG and TC, serum ALT and AST were significantly decreased, and the degree of hepatic lipid droplets was markedly improved compared with those in the ED group. CONCLUSION： TCM treatment causes significant reduction in alcohol-induced lipid hepatic accumulation, reversing fatty liver and liver damage, and can be used as a remedy for alcoholic fatty liver.

Does intraoperative frozen section really predict significant positive surgical margins after robot-assisted laparoscopic prostatectomy?A retrospective study

Therapeutic DC vaccination with IL-2 as a consolidation therapy for ovarian cancer patients： a phase Ⅰ/Ⅱ trial

While ovarian cancer （OvCa） responds well to surgery and conventional chemotherapy, a high recurrence rate of advanced OvCa is observed. In this phase Ⅰ/Ⅱ study, 10 OvCa patients with minimal residual disease were treated with autologous dendritic cells （DCs） and IL-2 to evaluate the safety and feasibility of this therapeutic strategy and to characterize the antigen-specific immune alterations induced through this treatment. Approximately 4 months after initial debulking and chemotherapy, patients received two subcutaneous doses of autologous monocyte-derived DCs pulsed with autologous tumor lysate and keyhole limpet hemocyanin （KLH） at 4-week intervals. After each DC inoculation, low-dose （200 mlU） IL-2 was introduced for 14 consecutive days as an immune adjuvant. The vaccination was well tolerated. In three out of 10 patients, the inclusion status after the initial therapy showed the maintenance of complete remission （CR） after DC vaccination for 83, 80.9 and 38.2 months without disease relapse. One patient with stable disease （SD） experienced the complete disappearance of tumor after DC vaccination, and this status was maintained for 50.8 months until tumor recurrence. In two patients with partial response （PR） was not responding to DC vaccination and their disease recurred. In the three patients with disease free long-term survival, significant immune alterations were observed, including increased natural killer （NK） activity, IFN-γ-secreting T cells, immune-stimulatory cytokine secretion and reduced immune-suppressive factor secretion after DC vaccination. Thus, in patients with NED status and increased overall survival, DC vaccination induced tumor-related immunity, potentially associated with long-term clinical responses against OvCa.

Hydroxyapatite(HA)is a representative substance that induces bone *** research team extracted nanohydroxyapatite(EH)from natural resources,especially equine bones,and developed it as a molecular biological ***(PEI)was used to coat the EH to develop a gene *** verify that PEI is well coated in the EH,we first observed the morphology and dispersity of PEI-coated EH(pEH)by electron *** pEH particles were well distributed,while only the EH particles were not distributed and ***,the existence of nitrogen elements of PEI on the surface of the pEH was confirmed by EDS,calcium concentration measurement and fourier transform infrared spectroscopy(FT-IR).Additionally,the pEH was confirmed to have a more positive charge than the 25 kD PEI by comparing the zeta *** a result of pGL3 transfection,pEH was better able to transport genes to cells than 25 kD *** verification as a gene carrier for pEH,we induced osteogenic differentiation of DPSCs by loading the BMP-2 gene in pEH(BMP-2/pEH)and delivering it to the *** a result,it was confirmed that osteogenic differentiation was promoted by showing that the expression of osteopontin(OPN),osteocalcin(OCN),and runt-related transcription factor 2(RUNX2)was significantly increased in the group treated with BMP-2/*** conclusion,we have not only developed a novel nonviral gene carrier that is better performing and less toxic than 25 kD PEI by modifying natural HA(the agricultural byproduct)but also proved that bone differentiation can be effectively promoted by delivering BMP-2 with pEH to stem cells.
