Prostate health index can stratify patients with Prostate Imaging Reporting and Data System score 3 lesions on magnetic resonance imaging to reduce prostate biopsies

A prospective cohort of men with localized prostate cancer on active surveillance protocol in hong Kong,China:what did we learn?

This study aimed to report the outcomes of active surveillance(AS)in the management of low-risk prostate cancer(PCa).It recruited87 men who were prospectively followed up according to the Prostate Cancer Research International Active Surveillance(PRIAS)protocol with local adaptation at SH ho Urology Centre,Prince of Wales hospital,hong Kong,*** investigated the predictorsof disease progression and found that baseline prostate-specific antigen density(PSAD)and the presence of the highest ProstateImaging-Reporting and Data System(PI-RADS)score 5 lesion on magnetic resonance imaging(MRI)are significantly correlatedwith disease ***,men with PSAD>0.2 ng ml^(−2)or PI-RADS 4 or 5 lesions had significantly worse upgradingfree survival compared to those with PSAD≤0.2 ng ml−2 and PI-RADS 2 or 3 *** study concludes that AS is a safe andeffective management strategy for selected patients to defer radical treatment and that most disease progression can be detectedafter the first repeated *** combination of PSAD>0.2 ng ml^(−2)and PI-RADS 4 or 5 lesions may serve as a useful predictorof early disease progression and provide a guide to optimize follow-up protocols for men in different risk groups.

The commercial viability of thermoelectric(TE)devices relies heavily on two factors:cost reduction and efficiency *** this study,we first produce p-type Cu_(12)Sb_(4)S_(16-x)(x=0,3,4)using a low-temperature bottom-up approach and demonstrate Cu_(12)Sb_(4)S_(13)to show the best TE performance among the three tested ***,the TE energy conversion efficiency of Cu_(12)Sb_(4)S_(13)is further enhanced by optimizing its electronic band structure through the incorporation of small amounts of *** an optimal Te content of 5 mol%,more than a twofold increase in the TE figure of merit(zT)is *** gain insight into the mechanism of improvement on the transport properties of the mate-rial,we compare the interphase transport mechanism by incorporating nanodomains of different metals(Ag and Cu)into the Cu_(12)Sb_(4)S_(13)*** improved electrical conductivity obtained with Cu_(12)Sb_(4)S_(13)-Te nanocomposites is attributed to a charge flooding of the Cu_(12)Sb_(4)S_(13)*** contrast,excessive down-ward band-bending at the interphases of Ag/Cu metal-semiconductor drastically reduces the electrical ***,a weighted mobility(μw)analysis shows a dominant thermal activation of carri-ers in Cu_(12)Sb_(4)S_(13)-Te *** this material,a strong decrease in lattice thermal conductivity is also found,which is associated with a phonon-carrier scattering *** work shows the impor-tance of proper band-engineering in TE nanocomposites to decouple electrical and thermal transport to enhance TE performance,and the efficacy ofμw for electrical and thermal transport analysis.

＜正＞The infusion of bone marrow cells into the damaged brain has been proposed as a new clinical practice for this disorder. Alternatively, hematopoietic growth factors may have a direct role on neural protection or have a mobilizing effect on bone marrow stem/progenitor cells to circulation for brain repair. Based on our previous findings, there are many similarities between megakaryocytes and neurons on functions and antigen expression such as neural marker MAP2, GFAP and Tau, 5 - HT2A,2B and 2C receptors, dopamine D1 and D2 receptors. The SARS-CoV-2 Omicron variants are notorious for their transmissibility,but little is known about their subgenomic RNA(sgRNA)*** study applied RNA-seq to delineate the quantitative and qualitative profiles of canonical sgRNA of 118 respiratory samples collected from patients infected with Omicron BA.2 and compared with 338 patients infected with non-variant of concern(non-VOC)-D614G.A unique characteristic profile depicted by the relative abundance of 9 canonical sgRNAs was reproduced by both BA.2 and non-VOCD614G regardless of host gender,age and presence of ***,such profile was lost in samples with low viral load,suggesting a potential application of sgRNA pattern to indicate viral activity of individual patient at a specific time point.A characteristic qualitative profile of canonical sgRNAs was also reproduced by both BA.2 and *** presence of a full set of canonical sgRNAs carried a coherent correlation with crude viral load(AUC¼0.91,95%CI 0.88–0.94),and sgRNA ORF7b was identified to be the best surrogate marker allowing feasible routine application in characterizing the infection status of individual *** potentials in using sgRNA as a target for vaccine and antiviral development are worth pursuing.

中枢神经系统肿瘤领域的知识体系庞大,理论与技术不断进步。中枢神经系统肿瘤的临床诊治更是个大学问。2019年第11和12期以及2020年第2期,我在《中国现代神经疾病杂志》主持了3期“脑胶质瘤”专题,内容主要涵盖我国脑胶质瘤临床诊断与治疗指南/规范建设、中枢神经系统肿瘤病理分类进展、脑胶质瘤临床试验创新体系及临床疗效反应评价体系、脑胶质瘤侵袭迁移及免疫治疗研究、脑科学及脑功能研究等临床与转化问题。2020年第2期,我与著名神经病理学家Daniel ***教授作为共同通讯作者在Glioma发表“A contemporary molecular view of diffuse gliomas with implications for diagnosis”。2020年第12期,我在《中华神经医学杂志》以“脑胶质瘤的临床诊治撷要”为题,评述当前恶性脑胶质瘤临床诊疗的现实问题。以专题组稿形式推动中枢神经系统肿瘤热点问题的讨论以及新理论和新技术的学习,尤其是在疫情之下,形式适宜、开卷有益。

Effect of organic mineral supplementation in reducing oxidative stress in holstein calves during short‑term heat stress and recovery conditions

Background This study investigated the effects of inorganic and organic minerals on physiological responses,oxidative stress reduction,and rumen microbiota in holstein bull calves(123.81±9.76 kg;5 months old)during short-term heat stress(HS)and recovery *** holstein calves were randomly assigned to four treatment groups:no mineral supplementation(Con),inorganic minerals(IM),organic minerals(OM),and high-concentration organic minerals(hoM)and two thermal environments(HS and recovery)using 4×2 factorial arrangement in a crossover design of four periods of 35 *** were maintained in a temperature-controlled *** experimental period consisted of 14 d of HS,14 d of recovery condititon,and a 7-d washing *** Body temperature and respiration rate were higher in HS than in the recovery conditions(PhoM-supplemented calves in both HS(90.38μg/dL)and recovery periods(102.00μg/dL)(Png the HS period,the serum cortisol was 20.26 ng/mL in the hoM group,which was 5.60 ng/mL lower than in the control group(PhoM group during HS,whereas it was highest in the hoM group(2.58 mmol Trolox equivalent/L)during the recovery period(PhoM supplementation during the HS and recovery periods,whereas SOD and GPX levels were not significantly affected(P>0.05).The principal coordinate analysis represented that the overall rumen microbiota was not influenced by mineral supplementation;however,temperature-induced microbial structure shifts were indicated(PERMANOVA:P<0.05).At the phylum level,Firmicutes and Actinobacteria decreased,whereas Fibrobacteres,Spirochaetes,and Tenericutes increased(P<0.05),under HS *** genus Treponema increased under HS conditions,while Christensenella was higher in recovery conditions(P<0.05).Conc

目的病人针灸治疗时,漏针可能造成严重后果。为增强针灸安全性,本研究将开发一个基于深度学习的自动云端数针系统。方法本研究利用拍有针灸针的手机图片为曾预先训练的定向目标检测模型(Oriented R-CNN)进行转移学习,以开发一种能够自动计算图片中针灸针数量的目标检测算法。我们首先拍摄了一个包含590张图片的训练集以及一个包含1025张图片的验证集。随后在带有NVIDIA Tesla T4图形处理器(GPU)的Google Colab环境中部署了MMRotate工具箱为模型进行训练。我们将训练的模型与Telegram bot手机小程序界面集成,并评估模型的准确率、精确率和召回率。本小程序中云端数针系统的速度则以端到端处理时间计算。结果在每张图片20根针的验证集情景中,我们的定向检测模型在准确率、精确率和召回率上分别达到96.49%、99.98%和99.84%,平均端到端处理时间为1.535秒。结论这项云服务系统在速度、准确性和可靠性上的提升展示了深度学习物体检测技术改进针灸实践的潜力。

Background:The current World Health Organization(Who) classification of nasopharyngeal carcinoma(NPC) con?veys little prognostic *** study aimed to propose an NPC histopathologic classification that can poten?tially be used to predict prognosis and treatment ***:We initially developed a histopathologic classification based on the morphologic traits and cell differentia?tion of tumors of 2716 NPC patients who were identified at Sun Yat?sen University Cancer Center(SYSUCC)(training cohort).Then,the proposed classification was applied to 1702 patients(retrospective validation cohort) from hospitals outside SYSUCC and 1613 patients(prospective validation cohort) from *** efficacy of radiochemotherapy and radiotherapy modalities was compared between the proposed *** used Cox proportional hazards models to estimate hazard ratios(HRs) with 95% confidence intervals(CI) for overall survival(OS).Results:The 5?year OS rates for all NPC patients who were diagnosed with epithelial carcinoma(EC;3708 patients),mixed sarcomatoid?epithelial carcinoma(MSEC;1247 patients),sarcomatoid carcinoma(SC;823 patients),and squamous cell carcinoma(SCC;253 patients) were 79.4%,70.5%,59.6%,and 42.6%,respectively(P horter OS than patients with EC(HR = 1.44,95% CI = 1.27–1.62),SC(HR = 2.00,95% CI = 1.76–2.28),or SCC(HR = 4.23,95% CI = 3.34–5.38).Radiochemotherapy significantly improved survival compared with radiotherapy alone for patients with EC(HR 49–0.75),and possibly for those with SCC(HR = 0.67,95% CI = 0.56–0.80),MSEC(HR = 0.58,95% CI = 0..74–1.28).= 0.63;95% CI = 0.40–0.98),but not for patients with SC(HR = 0.97,95% CI = 0Conclusions:The proposed classification offers more information for the prediction of NPC prognosis compared with the Who classification and might be a valuable tool to guide treatment decisions for subtypes that are associ?ated with a poor prognosis.
