Diet is critical in diabetes management and new nutritional interventions are continuously being tested in randomized controlled trials. However, to make meaningful conclusions about the efficacy of dietary treatments, it is critical to be certain that the participants adhered to the dietary intervention and the dietary changes are valid. The objective of this meta-analysis was to assess the quality of dietary assessment and whether it might impact on study metabolic outcomes. Methods Four databases, MEDLINE, EMBASE, CINAHL and CENTRAL, were searched, from inception until September 2019 for randomized controlled trials of nutritional interventions in people with non-insulin-dependent type 2 diabetes. Trials that measured nutritional intakes in methods and HbA1 c as an outcome were included. Investigators assessed risk of bias and quality of the dietary measurements using the Cochrane Risk of Bias Assessment Tool 2.0 and a redeveloped EURICA tool, respectively. The study was conducted in accordance with the Preferred Reporting in Systematic Reviews and Meta-analyses. PROSPERO registration number: CRD42019146471. Results Of 2552 records retrieved, 23 studies met the inclusion criteria. Nineteen studies aimed to achieve a reduction in HbA1c, and four studies aimed to maintain HbA1c while improving other metabolic/nutritional outcomes. Two studies were rated as ’good’, six as ’medium’, and 15 as ‘poor’ in the quality assessment of the dietary measurement tool. The majority of studies were rated as high risk of bias. Of those studies with medium or high diet quality assessment, six of eight achieved the desired outcome whereas only four of the 15 other studies achieved the desired clinical outcome for HbA1 c. Conclusion The poor quality of dietary assessment in clinical trials manipulating dietary intakes casts uncertainty on the legitimacy of causal mechanisms attributed to dietary interventions. Fulminant ulcerative colitis associated with steroid-resistant minimal change disease and psoriasis: A case report

A 43-year-old Chinese patient with a history of psoriasis developed fulminant ulcerative colitis after immunosuppressive therapy for steroid-resistant minimal change disease was stopped. Minimal change disease in association with inflammatory bowel disease is a rare condition. We here report a case showing an association between ulcerative colitis, minimal change disease, and psoriasis. The possible pathological link between 3 diseases is discussed.

Hepatoblastoma is the most common primary hepatic tumor of children. However, only a very few cases have been reported in adults. Most studies support treatment with chemotherapy followed by surgical resection. We present the first reported case of adult hepatoblastoma in Taiwan. A 52-year-old female suffered from sudden onset of abdominal pain and general weakness for *** bleeding with hemorrhagic shock was suspected and two massive lesions in both lobes of the liver with hemoperitoneum were noted from imaging *** resection of the larger left lobe tumor and radio-frequency ablation of the right smaller one were performed. The histopathology diagnosis was of a hepatoblastoma.

Testicular sperm extraction and intracytoplasmic sperm injection in non obstructive azoospermia

Objective To report the experience in sperm extraction from testicular biopsies (TESE) performed from March 1996 to July 1998 in men with non obstructive azoospermia (NOA) Methods Comparisons of age, volume of both testes, serum FSH and testosterone in men, and histology of testicular samples in the first cycles between cycles with spermatozoa found and those without spermatozoa found were performed Comparisons of fertilization, cleavage and pregnancy rates between cycles with spermatozoa injected and those with spermatids injected were performed Results Spermatozoa were found in only 12 out of 26 first TESE cycles (46 2%) and other cycles had spermatids (round cells) only Age of men, history of mumps orchitis/oligozoospermia, volume of both testes and serum FSH/testosterone levels in men were not significantly different between cycles with and without spermatozoa The fertilization rate was significantly higher in cycles with spermatozoa injected than those with round cell injections (63 3% vs 23 2%, P <0 0001, Chi squared test) The pregnancy rate was 14 3% per cycle when spermatozoa were injected Conclusions TESE followed by Intracytoplasmic sperm injection (ICSI) is an effective treatment in patients with NOA Less than half of the patients undergoing TESE had spermatozoa recovered Age of men, volume of both testes and serum FSH/ testosterone levels in men were not useful in predicting successful recovery Compared to using ejaculated and epididymal spermatozoa, fertilization and pregnancy rates were achieved when testicular spermatozoa were used for ICSI

Structure and Photoluminescence Properties of Pr3+Ion-Doped BaY2ZnO5Phosphor Prepared Using a Sol-Gel Method?

The Pr3+ ion-doped BaY2ZnO5 phosphor with the orthorhombic structure was synthesized successfully using a sol-gel method in this study. The SEM images show that the BaY2ZnO5:Pr3+ phosphor particles are aggregational but have an isotropic distribution for 2 mol% Pr3+ ions doped. Under an excitation wavelength of 311 nm, the emission bands that appear in the emission spectra are due to the 3P0→3H4,5,6, 1D2→3H4 and 3P0→3F2 electron transition of Pr3+ ion, and it is the same as that for solid state reaction preparation. Comparing to the solid state reaction preparation, the intensities of the 3P0→3H4 transition were increased by about 6.5 times for sol-gel method. The enhancement in emission intensity is because the activators have more homogeneous contribution in host for the sol-gel method preparation. In addition, the color tone did not change very obviously, which located around the green light region for Pr3+ ion concentrations increasing. The color stability is better for sol-gel method than that for the solid state reaction preparation.

An Improved Differential Evolution and Its Industrial Application

In this paper, an improved Differential Evolution (DE) that incorporates double wavelet-based operations is proposed to solve the Economic Load Dispatch (ELD) problem. The double wavelet mutations are applied in order to enhance DE in exploring the solution space more effectively for better solution quality and stability. The first stage of wavelet operation is embedded in the DE mutation operation, in which the scaling factor is governed by a wavelet function. In the second stage, a wavelet-based mutation operation is embedded in the DE crossover operation. The trial population vectors are modified by the wavelet function. A suite of benchmark test functions is employed to evaluate the performance of the proposed DE in different problems. The result shows empirically that the proposed method out-performs signifycantly the conventional methods in terms of convergence speed, solution quality and solution stability. Then the proposed method is applied to the Economic Load Dispatch with Valve-Point Loading (ELD-VPL) problem, which is a process to share the power demand among the online generators in a power system for minimum fuel cost. Two different conditions of the ELD problem have been tested in this paper. It is observed that the proposed method gives satisfactory optimal costs when compared with the other techniques in the literature.

COVID-19 outbreaks in residential care homes in Hong Kong and effectiveness of vaccine against severe outcomes

Background:COVID-19 outbreaks in residential care homes for the elderly(RCHEs)and for persons with disability(RCHDs)have caused significant morbidity and mortality during 5th epidemic in Hong *** article reviewed COVID-19 outbreaks situation and estimated the effectiveness of receiving at least two-dose of COVID-19 vaccine in preventing severe ***:To estimate attack rates and vaccination coverage,documentation on COVID-19 infection and their vaccination records of residential care homes(RCH)residents reported between December 31,2021 and May 31,2022 were reviewed,and infected cases were follow-up for 4 weeks for severe outcomes or *** between vaccination coverage against attack rate by types of homes was *** RCH residents with available information were included in the analysis of vaccine effectiveness against severe outcomes and ***:COVID-19 vaccination coverage was low in RCHDs(median 0.46,IQR:0.24-0.76)and very low in RCHEs(median 0.08,IQR:0.00-0.19).Higher attack rates were recorded among RCHE residents(median 0.84,IQR:0.64-0.93)and higher case fatality rate(CFR:28.1%)than in RCHDs(median 0.58,IQR:0.31-0.84;CFR:3.9%).The attack rate decreased when vaccination coverage increased for both RCHEs(ρ=−0.131,p<0.001)and RCHDs(ρ=−0.333,p<0.001).Comparing with infected residents who were unvaccinated/vaccinated with one-dose,receiving at least two-dose was estimated to be effective in reducing severe outcomes in 31%and 36%of infected RCHE and RCHD residents respectively;with greater reduction in mortality among RCHD than RCHE residents(54%and 38%,respectively).Vaccine effectiveness of two-dose of BNT162b2 against severe outcomes and death are higher than that of ***:Increasing COVID-19 vaccination could have significant impact on reducing the risk of COVID-19 outbreaks in *** least two-dose of COVID-19 vaccine is still effective in reducing severe outcomes and death among infected residents in

“道”与“术”是中国传统文化中的两个概念。“道”指基本概念、基本原则、准则和规则，“术”则是“道”在日常生活中的应用。若将这两个概念应用于临床医学，“道”就是指每一位优秀临床医师都应遵循的基本原则和准则，即医德优良、医学知识丰富、医技精湛，以及将其应用于治病救人的能力。此外，每一位临床医师都应致力于终生医学继续教育和培训，扩展医学知识。对于一位优秀的外科医师而言，一个额外的要求就是拥有一双妙手。“术”指外科医师要通过自己的双手来运用医学知识和技能。“道”与“术”平衡是成为一位优秀外科医师的基本要求，也是其未来发展的基石。外科临床研究是基于良好的临床工作和结果，而好的临床研究能够促进临床工作和结果，两者相辅相成，形成良性循环。换言之，外科临床研究与创新是“道”与“术”平衡的最终成果。
