Efficacy of balloon-occluded retrograde transvenous obliteration, percutaneous transhepatic obliteration and combined techniques for the management of gastric fundal varices

AIM： To evaluate the effect of three interventional treatments involving transvenous obliteration for the treatment of gastric varices, and to compare the efficacy and adverse effects of these methods, METHODS： From 1995 to 2004, 93 patients with gastric fundal varices underwent interventional radiologic embolotherapy at our hospital. Of the 93 patients, 75 were treated with the balloon-occluded retrograde transvenous obliteration （BRTO） procedure; 8 were with the percutaneous transhepatic obliteration （PTO） procedure; and 10 were with the combined BRTO and PTO therapy. A follow-up evaluation examined the rates of survival, recurrence and rebleeding of the gastric varices, worsening of esophageal varices and complications in each group. RESULTS： The BRTO, PTO, and combined therapy were technically successful in 81% （75/93）, 44% （8/18）, and 100% （10/10） patients, respectively. Recurrence of gastric varices was found in 3 patients in the BRTO group and in 3 patients in the PTO group. Rebleeding was observed in 1 patient in the BRTO group and in 1 patient in the PTO group. The 1- and 3-year survival rates were 98% and 87% in the patients without hepatocellular carcinoma （HCC） in the BRTO group, 100% and 100% in the PTO group, and 90% and 75% in the combined therapy group, respectively. CONCLUSION： Combined BRTO and PTO therapy may rescue cases with uncontrollable gastric fundal varices that remained even after treatment with BRTO and/or PTO, though there were limitations of our study, including retrospective nature and discrepancy in sample size between the BRTO, PTO and combined therapy groups.

Proton pump inhibitor induced collagen expression in colonocytes is associated with collagenous colitis

To elucidate the role of proton pump inhibitors (PPIs) in collagenous disease, direct effect of PPI on colonocytes was *** colitis is a common cause of non-bloody, watery diarrhea. Recently, there has been increasing focus on the use of proton PPIs as a risk factor for developing collagenous colitis. Mouse CT26 colonic cells were treated with PPI and/or PPI-induced alkaline media. Expression of fibrosis-associated genes was examined by RT-PCR. In human materials, collagen expression was examined by ***26 cells expressed a Na+-H+ exchanger gene (solute carrier family 9, member A2). Treatment with PPI and/or PPI-induced alkaline media caused growth inhibition and oxidative stress in CT26 cells. The treatment increased expression of fibrosis inducing factors, transforming growth factor β and fibroblast growth factor 2. The treatment also decreased expression of a negative regulator of collagen production, replication factor C1, resulting in increased expression of collagen types III and IV in association with lipid peroxide. In biopsy specimens from patients with collagenous colitis, type III and IV collagen were increased. Increase of type III collagen was more pronounced in PPI-associated collagenous colitis than in non-PPI-associated *** these findings, the reaction of colonocytes to PPI might participate in pathogenesis of collagenous colitis.

Almost 30 years has passed since the publication of materials on vacuum carburizing technology,and is attracting a great deal of attention as a technology capable of being used as a substitute for gas carburizing technology. However, the technology was not popular except in specific fields. The main reason for this is due to a variety of harmful influences accompanying the sooting problems caused by CH4 or C3H8. We have succeeded in that the occurrence of sooting was suppressed by utilizing acetylene, at extremely low pressure for carburizing (below 1 kPa). This process is now showing the excellent quality and prospects for this technology in terms of quality, economy and safety. At present almost 70 practical mass production furnaces are used in production lines, in Japan and abroad. At this time, we will report summary of the present acetylene vacuum carburizing process and the actual results obtained by these acetylene vacuum carburizing furnaces for mass production.

According to a report by the Ministry of Education,Culture,Sports,Science,and Technology-Japan(MEXT),the number of students who did not attend school in Japan in the fiscal year 2018 was 164,528,and the percentage of students who did not attend school was 1.7%.This is the highest number of students *** has been providing various measures to support students’return to school,such as setting up adaptation classes and reviewing the school structure,intending to reduce the number of students who do not attend ***,it is not always easy to help students return to school,since the problems they face may not be simply academic or psychological in ***,many students have returned to school,but who have not returned to school in the *** this paper,we introduce a special needs classroom for truant ***,the results of a survey of teachers and assistants in this classroom are ***,we will report on our current efforts.

Chemoradiotherapy for a patient with a giant esophageal fistula

We describe our experience of treatment for a giant esophageal malignant fistula, which has not been reported previously. A 36-year-old woman who was diagnosed as having massive esophageal small cell carcinoma with metastases was treated with chemoradiotherapy. However, a giant esophagomediastinal fistula appeared due to shrinkage of the massive tumor, and all anti-cancer treatment was suspended. However, chemoradiotherapy was restarted at the request of the patient despite the presence of the fistula. After restarting treatment, the giant esophageal fistula was naturally closed despite intensive chemoradiotherapy, and the patient became able to eat and drink. Although the patient finally died, her QOL and prognosis seemed to be improved by the chemoradiotherapy. Anti-cancer treatment could be safely performed despite the presence of a giant fistula. The giant fistula closed while intensive chemotherapy was administered to the patient. Therefore, the presence of a fistula may not be a contraindication for curative chemoradiotherapy. Completion of treatment with proper management and maintenance of patients would be of benefit to patients with fistula.

Recent advances in basic and clinical aspects of inflammatory bowel disease: Which steps in the mucosal inflammation should we block for the treatment of inflammatory bowel disease?

There are four steps in the interaction between intestinal microbes and mucosal inflammation in genetically predisposed individuals from the viewpoints of basic and clinical aspects of inflammatory bowel disease （IBD）. The first step is an interaction between intestinal microbes or their components and intestinal epithelial cells via receptors, the second step an interaction between macrophages and dendritic cells and mucosal lymphocytes, the third step an interaction between lymphocytes and vascular endothelial cells, and the fourth step an interaction between lymphocytes and granulocytes producing proinflammatory cytokines or free radicals and mucosal damage and repair. Recent therapeutic approaches for IBD aim to block these four steps in the intestinal inflammation of patients with IBD.

Although additive manufactured（AM） metals are attractive materials, the fatigue strength of AM metals are considerably weak comparing with that of wrought materials. The mechanical surface treatment such as shot peening can improve the fatigue strength of metallic materials. Recently, a novel mechanical surface treatment using solid-liquid-gas interfacial phenomena induced by pulse laser has been developed. In the present paper, in order to demonstrate the improvement of fatigue strength of AM metals by solid-liquid-gas interfacial phenomena, titanium alloy Ti6Al4V manufactured by electron beam melting EBM was treated by submerged pulse laser and tested by a plane bending fatigue test. The key factors were investigated by evaluating the relation between the fatigue properties and mechanical properties of the surface treated by submerged laser peening, cavitation peening and shot peening.

Bone metastasis from early gastric cancer following non-curative endoscopic submucosal dissection

A 67-year-old male underwent endoscopic submucosal dissection(ESD)to treat early gastric cancer(EGC)in *** lesion(50 mm × 25 mm diameter)was histologically diagnosed as poorly differentiated adenocarcinoma,with an ulcer *** the tumor was confined to the mucosa with no evidence of lymphovascular involvement,the ESD was regarded as a noncurative resection due to the histological type,tumor size,and existence of an ulcer finding(as indicated by the 2010 Japanese gastric cancer treatment guidelines,ver.3).Despite strong recommendation for subsequent gastrectomy,the patient refused *** alternative follow-up routine was designed,which included five years of biannual clinical examinations to detect and measure serum tumor markers and perform visual assessment of recurrence by endoscopy and computed tomography scan after which the examinations were performed *** patient's condition remained stable for eight years,until a complaint of back pain in 2010 prompted further clinical *** scintigraphy indicated increased *** examination of biopsy specimens taken from the lumbar spine revealed adenocarcinoma resembling the carcinoma cells from the EGC that had been treated previously by ESD,and which was consistent with immunohistochemical findings of gastrointestinal tract ***,the diagnosis of bone metastasis from EGC was *** reported rates of EGC recurrence in surgically resected cases range 1.4%-3.4%,but among these bone metastasis is very *** our knowledge,this is the first reported case of bone metastasis from EGC following a non-curative ESD and occurring after an eight-year disease-free interval.
