BACKGROUND In 2016,the Food and Drug Administration approved the first hybrid closed-loop(HCL)insulin delivery system for adults with type 1 diabetes(T1D).There is limited information on the impact of using HCL systems on patient-reported outcomes(PROs)in patients with T1D in real-world clinical *** this independent study,we evaluated glycemic parameters and PROs over one year of continuous use of medtronic’s 670G HCL in real-world clinical *** To assess the effects of hybrid closed loop system on glycemic control and quality of life in adults with *** We evaluated 71 patients with T1D(mean age:45.5±12.1 years;59%females;body weight:83.8±18.7 kg,body mass index:28.7±5.6 kg/m2,A1C:7.6%±0.8%)who were treated with HCL at Joslin Clinic from 2017 to *** measured A1C and percent of glucose time-in-range(%TIR)at baseline and 12 *** measured percent time in auto mode(%TiAm)for the last two weeks preceding the final visit and assessed PROs through several validated quality-of-life surveys related to general health and diabetes *** At 12 mo,A1C decreased by 0.3%±0.1%(P=0.001)and%TIR increased by 8.1%±2.5%(P=0.002).The average%TiAm was only 64.3%±32.8%and was not associated with A1C,%TIR or ***,provided at baseline and at the end of the study,showed that the physical functioning submodule of 36Item Short-Form Health Survey increased significantly by 22.9%(Pmia fear survey/worry scale decreased significantly by 24.9%(Pm Areas In Diabetes reduced significantly by-17.2%(P=0.002).The emotional burden submodules of dietary diversity score reduced significantly by-44.7%(P=0.001).Furthermore,analysis of Clarke questionnaire showed no increase in awareness of hypoglycemic ***-5 showed no improvements in subject’s wellbeing among participants after starting the 670G HCL ***,analysis of Pittsburgh Sleep Quality Index showed no difference in sleep quality,sleep latency,or duration o

AIm： To compare argon laser photocoagulation and intrastromal injection of voriconazole as adjunctive treatment modalities in cases of resistant mycotic corneal ulcers. mETHODS： Two groups each of them included 20 cases of resistant mycotic corneal ulcers. Both groups treated with local and systemic specific antimicrobial drugs guided with culture and sensitivity results. In one group argon laser photocoagulation was used as an adjunctive therapy to the specific antifungal drugs and in the other group, intrastromal injection of voriconazole was done besides the specific antifungal drugs. The 40 cases included in the study were proven according to culture and sensitivity to be 28 cases with pure fungal results and 12 cases with mixed （fungal and bacterial）. In argon laser group, argon laser irradiation of the corneal ulcer was performed using argon laser 532 nm wavelength （Carl Zeiss LSL 532s AG; meditec, Inc.） after fluorescein staining. In the other group, voriconazole solution （500 pg/mL） was prepared and injected in the corneal stroma. All cases were followed up for 3mo after healing was achieved. RESULTS： Complete healing of the epithelial defect and resolution of stromal infiltration with no adverse effects were achieved in argon laser group in duration ranged from 2-4wk in 90% of cases. In voriconazole group 4 cases needed amniotic membrane graft due to thinning and 16 cases healed in duration ranged from 2- 6wk （80% of cases）. CONCLUSION： Argon superior to intrastromal treatment of resistant fungal laser photocoagulation is voriconazole injection in corneal ulcers.

BACKGROUND： Nerve root avulsion is a frequent finding in patients with brachial plexus injury following road traffic accidents or as a result of severe arm traction during complicated deliveries. This injury constitutes a challenging clinical and surgical problem. The morphological characteristics of motoneurons after nerve root avulsion deserve further analysis. OBJECTIVE： To study the different morphological changes of α-motoneurons under light and electron microscopy after C8 spinal ventral rootlets avulsion and transection at various stages. DESIGN： Controlled animal study. SETTING： Department of Anatomy, King Faisal University. mATERIALS： The experiment was carried out at the Department of Anatomy, College of medicine, King Faisal University between January 2005 and march 2006. Six adult Sprague Dawley rats weighing 200-350 g, irrespective of gender, were used for this study. The animals were bred at the animal house, College of medicine, King Faisal University, and fed on rat maintenance diet. Water and standard diet were supplied ad libitum. Animal interventions were carried out according to animal ethical standards. mETHODS： Three animals were randomly chosen for avulsion of the right ventral rootlets of C8 spinal nerves. The other three received transection of the right ventral rootlets of C8 spinal nerves. ①Avulsion experiment： After rats were anesthetized, the right ventral rootlets of C8 spinal nerves were identified. The ventral rootlets were avulsed from the spinal cord by traction with a fine hook （Fine Science Tools Inc., No. 10031-13, Germany）. Traction was exerted in a direction parallel to the course of the spinal root. Under the operating microscope, the C8 segment was exactly located. After checking the successfulness of the surgical procedure, the C8 segment was separated from the spinal cord. A generalized continuity equation extending the ordinary continuity equation is found using quanternions to show it is compatible with Dirac, Schrǒdinger, Klein-Gordon and diffusion equations. This generalized equation is Lorentz invariant. The transport properties of electrons are found to be governed by the Schr6dinger-like equation and not by the diffusion equation.
