Vaginal Trachelectomy for Retained Cervical Stump after Supracervical Hysterectomy: Technical Tips and Outcomes

Background and Objectives: The supracervical hysterectomy (SCH) has resulted in a group of patients with a retained cervix at risk of persistent symptoms, who may require a trachelectomy in the future. This study was to evaluate the efficacy of vaginal trachelectomy (VT) after a previous SCH. Methods: This was a prospective study that includes 13 cases with different ages and different complaints, sharing the same primary operation supracervical hysterectomy. They have different pathologies of the SCH specimen but they share the same completion surgery. The surgical outcome was analyzed. Results: Thirteen patients underwent vaginal trachelectomy for recurrent symptoms. The ages of patients were ranged from 37 years to 68 years (Mean ± SD, 56.4 ± 10.7). SCH was most commonly performed for abnormal uterine bleeding AUB (7/13, 53.8%), pelvic mass (5/1, 38.5%), and pelvic pain (1/13, 7.7%), the symptoms leading to vaginal trachelectomy were the same as those leading to supracervical hysterectomy. The median interval time from SCH to seeking medical help for the persistence or recurrence of symptoms and to VT was 2 weeks (1 to 96 weeks). Concomitant procedures were laparoscopic removal of both ovaries in 2 cases and pelvic lymphadenectomy in 1 case. The median length of operation was 45 minutes. In all cases, symptoms leading to trachelectomy resolved completely after surgery, and patients reported a significant improvement. Conclusions: The cervix, left behind at subtotal hysterectomy, requires removal, the vaginal route is probably the safest, and least traumatic. Vaginal radical trachelectomy appears to be feasible and safe for the treatment of endometrial malignancy discovered after supracervical hysterectomy.

Assessment of Urinary Albumin/Creatinine Ratio in Non-Obese Non-Diabetic Patients with Nonalcoholic Fatty Liver Disease

Nonalcoholic fatty liver disease (NAFLD) is a pathological condition seen as histological change ranging from simple steatosis to steatohepatitis, advanced fibrosis and liver disease among patients without significant alcohol consumption. Microalbuminuria which is defined as the urinary albumin excretion of 30 - 300 mg/24h has been reported to be a risk factor for renal and cardiovascular disorders. It also has independent correlation to high mortality in diabetic and hypertensive patients. NAFLD is affecting non obese non diabetic individuals;Microalbuminuria is correlated to visceral adipose tissue even in non diabetic non obese patients with limited studies in this aspect. Microalbuminuria is considered as a risk factor for cardiovascular and chronic kidney disease. Aim of the work: To assess urinary albumin creatinine ratio in non-obese non-diabetic patients with nonalcoholic fatty liver disease. Patients and methods: Total number of 80 patients with NAFLD that were non diabetic non obese patients. Abdominal ultrasonography and laboratory investigations were done. Results: Eighty non-obese, non-diabetic subjects (32 women, 48 men) with the mean age of 50.9 were included in this study. The population of the study was classified into four groups according to ultrasonographic degrees of steatosis. Control Group (A), (No. 25) 31.25% of total cases are reported as no fat accumulation in liver. Group B;No. 21 26.25% of total cases are reported as had mild steatosis (NAFLD 1). Group C;No. 18 (22.5%) of total cases and reported as had moderate fat accumulation (moderate steatosis). Group D;No. 16 (20%) of total cases reported had severe fat accumulation (severe steatosis). Urinary albumin creatinine ratio also shows increase in its values with increasing in the degree of steatosis among different groups which is highly statistically significant. Presence of Alternaria in Scalp of Patients with Alopecia Areata： Triggering Factor or Coexistence？

AA （Alopecia areata） is the most frequent cause of inflammation-induced hair loss, affecting 0.1 to 0.2% of population worldwide. The development of organ-specific autoimmune reactions directed against anagen hair follicles seems to play a key role in the pathogenesis of alopecia areata. However, the triggering antigen（s） responsible for inducing autoimmune phenomena in these individuals remain unknown. Viral, bacterial or fungal pathogens have been implied as possible triggering factors of autoimmune reactions. The present study aims to identify the role of dematiaceous fungi in the pathogenesis of alopecia areata. 30 patients diagnosed clinically as alopecia areata and 30 normal age matched persons have undergone mycological examination. Mycology examination of the epidermal scrapings was done by DME （direct microscopic examination）, culture on SDA （sabouraud＇s dextrose agar） and imaging. There is significantly higher percentage of positive results for Alternaria species by culture on sabouraud＇s agar in patients group （20%） compared to controls （13.3%） P-value 〈 0.05. The possible role of Alternaria antigens （e.g. antigens involved in melanin synthesis） in triggering autoimmunity in alopecia areata still needs further research on a wider scale of cases.

A new heterogeneous catalyst composed of Pd nanoparticles immobilized within a HypoG el resin has been prepared in the absence of any ligands using an extensive cross-linking *** newly developed nanocatalyst was characterized by N_2 adsorption-desorption,X-ray diffraction(XRD),transmission electron microscopy(TEM),scanning electron microscopy(SEM),energy dispersive X-ray(EDX),Fourier transform infrared spectroscopy and inductively coupled plasma-mass spectrometer(ICP-MS)*** and XRD results revealed that the Pd nanoparticles were well dispersed with diameters in the range of 4–12 nm,and an average size of about 8 *** cross-linked Pd catalyst demonstrated excellent catalytic activity towards the synthesis of a series of biaryl compounds by the reaction of various aryl halides(e.g.,bromides andiodides)with phenylboronic acid in the presence of tetrabutylammonium ***-MS analysis indicated that there was only 0.25%weight loss of Pd(0.55±0.02 ppm)from the supported catalyst after the first cycle ***,the catalyst showed excellent reusability(up to five uses)with consistently high levels of catalytic activity following its recovery by filtration.

Current Advances in Transcatheter Intervention for Children Born with Congenital Heart Defects: A Review of Literature

This review aims to sum up the improvements witnessed in the field of interventional cardiology during recent times. The last decade has witnessed significant technical advances in the field of radiological imaging and also in interventional cardiology which has helped to offer more non-invasive solutions for the management of congenital heart defects. This has resulted from the use of advanced 3-dimensional fusion imaging instead of conventional 2-dimensional angiography, applying interactive real-time enhancement and using computed tomography and Magnetic Resonance Imaging for interventional procedures. Similarly the introduction of next generation devices, have not only improved the final outcome of the procedure but also has helped in reducing the challenges that were faced before and with the initial generation of devices. These advances have helped not only in reducing the radiation exposure, the use of contrast medium dose but also have resulted in improved early survival. The field of neonatal cardiology has advanced at an unprecedented pace. The transcatheter closure of patent ductus arteriosus has evolved over time and now it has been made possible at much lower body weight. Similarly, early use of stents for aortic coarctation has been found effective in some patients, especially when surgical intervention had been denied. The application of the hybrid approach for the management of complex congenital heart defects has also been effectively applied. More recently transcatheter placement of the pulmonary valve has been introduced for severely stenotic and/or regurgitant pulmonary valve in adolescents and adults. It is anticipated that in near future, this procedure would be available for relatively younger patients. In conclusion: last 2 decades have improvised pediatric interventional cardiology to incorporate less invasive solutions for CHD. Role of Multidetector CT in Staging of Gastric Carcinoma

Background: Radiology plays an essential role in the diagnosis, staging and surveillance of oncology patients. CT is the most sensitive imaging modality in the workup of these patients. Aim of the Study: The aim of this work is to detect the role of MDCT (multidetector computed tomography) in the preoperative investigation of gastric adenocarcinoma patients according to TNM staging. Patients and Methods: This is a prospective study enrolling 20 patients who had histologically proven adenocarcinoma based upon an upper gastrointestinal endoscopic biopsy for MDCT staging of gastric carcinoma during the period from June 2016 to June 2017. The MSCT data were correlated and compared with the histopathological results. The study was conducted in the Radiology Department of Assiut University Hospital using 64-MDCT (Toshiba Aquilion). Results: According to our study results, the sensitivity of determining T1 stage on CT scan can’t be detected as there was only 1 case pathologically proved T1 and overstaged as T2 by MSCT;however, accuracy and specificity are quite high, which was 95% and 100% respectively. For T2 stage tumors (25.0% of cases), accuracy is 95%, sensitivity—100%, and specificity—93.7%. According to our results the accuracy and sensitivity of T3 staging are 75% and 100%, while those of T4 stage were 75% and 44.4% respectively. Tumor was correctly staged in 14 of 20 patients (the valid T staging rate was 70.0%). Tumor was under-staged in 5 of 20 patients (25.0%) (staged as T3, but pathologically proven to be T4a). As regards N staging accuracy found results for N0 (62.5%), N2 (87.5%) and N3 (75%), while N1 accuracy recorded 37.5%. As regards the nodal staging sensitivity which had a range from (0% for N4) to (66% for N2) this wide range of sensitivity demonstrates the problem of CT in nodal staging. As regards sensitivity of M0, accuracy was 100% and 85% respectively. While that of M1 was (62.5%) and (85%) respectively. Conclusion: MSCT can be the first choice fo

Clinical and Experimental Study of the Toxic Neuropsychiatric Effects of Formaldehyde Exposure: Has Garlic a Protective Role?

Background: The central nervous system is one of the most important systems affected by formaldehyde (FA). Aim of Work: This study was designed to assess its toxic neuropsychiatric effects both clinically and experimentally and the protective effects of garlic. Methods: Clinically: 20 workers in the gross anatomy laboratory and 20 libertarians underwent a standardized clinical assessment including medical, neurological and psychiatric examination. Experimental: 40 male rats were divided randomly into four groups. Group I is control group. Group II received 10 mg/kg of FA intraperitoneally once daily for 14 days. Group III was treated with fresh garlic juice (1 ml/100g body weight) once daily by oral gavage for 14 days. Groups VI received fresh garlic juice plus formaldehyde daily for 14 days. At the end of the experiment, the rats’ brains were obtained for histological examination and biochemical analysis. Results: Clinical and psychiatric profile of FA exposed persons’ revealed cognitive impaired, anxious and depressed persons. There were hostile persons with more hostility toward outside. Experimentally, hippocampal and frontal superoxide dismutase and reduced glutathione showed highly significant decrease while malondialdehyde and nitric oxide level showed highly significant increase in formaldehyde treated group when compared with control group. Also histopathological changes in the hippocampal and frontal cortices by light microscope revealed many distorted cells with deeply stained shrunked nuclei and cytoplasm was surrounded by vacuolated pale areas in FA exposed group. Minimization of biochemical and histopathological changes were observed in combined formaldehyde and garlic treated group. Conclusion: The profiles of personality arouse dangerous affairs about the toxic impact of FA on persons, family, and society. Formaldehyde-induced neuronal damage, oxidative stress and lipid peroxidation in brain were minimized by addition of garlic.
