A Sandwich Technique Employing Right Ventricular Incision to Repair Posterior Ventricular Septal Rupture with Right Ventricular Wall Dissection: A Case Report

Background: Ventricular septal rupture (VSR) leading to right ventricular (RV) wall dissection is an extremely rare and life-threatening complication of inferior myocardial infarction (MI) with posterior VSR. Its rare incidence and complex pathology make it difficult to select the appropriate surgical procedures to prevent fatalities. Case Presentation: A 68-year-old woman was transferred to our hospital because of a post-infarction VSR 12 days after symptom onset. Short-axis image obtained using transthoracic echocardiography demonstrated a large posterior VSR. Moreover, the VSR was continuous, with a large echo-free space in the posterior wall of the right ventricle. Color echocardiography showed blood flowing into the echo-free space through the septal defect and blood flowing out into the RV lumen. Coronary angiography revealed complete occlusion of the second segment of the right coronary artery. Thus, dissection of the posterior wall of the right ventricle that continued into the RV lumen was considered to have been caused by the posterior VSR caused by an inferior MI. The patient underwent urgent surgery to repair the VSR using the sandwich double-patch technique by making a posterior RV incision that was repaired using a third patch. No additional procedure was required to block the flow from the cavity of the RV wall dissection into the RV lumen. Postoperative echocardiography and contrast-enhanced computed tomography demonstrated that the VSR was closed securely and the RV wall dissection was almost completely thrombosed. Conclusion: In this case, a patient with a posterior VSR and RV wall dissection was successfully treated using the sandwich double-patch technique with a posterior RV incision. No additional procedure may be needed for RV wall dissection when a secure VSR repair is complete;however, close follow-up is essential to improve the long-term prognosis.

Driving skill classification in curve driving scenes using machine learning

Driver support and infotainment systems can be adapted to the specific needs of individual drivers by assessing driver skill and state. In this paper, we present a machine learning approach to classifying the skill at maneuvering by drivers using both longitudinal and lateral controls in a vehicle. Conceptually, a model of drivers is constructed on the basis of sensor data related to the driving environment, the drivers＇ behaviors, and the vehi- cles＇ responses to the environment and behavior together. Once the model is built, the driving skills of an unknown driver can be classified automatically from the driving data. In this paper, we demonstrate the feasibility of using the proposed method to assess driving skill from the results of a driving simulator. We experiment with curve driving scenes, using both full curve and segmented curve sce- narios. Six curves with different radii and angular changes were set up for the experiment. In the full curve driving scene, principal component analysis and a support vector machine-based method accurately classified drivers in 95.7 % of cases when using driving data about high- and low/average-skilled driver groups. In the cases with seg- mented curves, classification accuracy was 89 %.

While micronuclei (MN) store extranuclear DNA and cause genome instability, the effects of nuclear envelope (NE) assembly defects associated with MN on genome instability remain largely unknown. Here, we investigated the NE protein distribution in MN using HeLa human cervical cancer cells. Under the standard condition and two pharmacological culture conditions, we found that three types of NE protein assemblies were associated with MN: 1) intact NE assembly, in which both core and non-core NE proteins were evenly present;2) type I assembly, in which only core NE proteins were detectable;and 3) type II assembly in which a region deficient for both core and non-core NE proteins existed and a pattern recognition receptor, cyclic guanosine monophos-phate-adenosine monophosphate synthase, was frequently detected. Our findings provide experimental settings and a method of grouping MN-associated NE defects, which may be helpful for researchers who are interested in regulation of genome and nuclear organization relevant to cancer development.

Communications network damage resulting from a large disaster causes difficulties in the ability to rapidly understand the current situation and thus make appropriate decisions towards mitigating problems,such as where to send and dispense emergency *** research outlined in this paper focuses on the rapid construction of a network after a disaster *** study suggests ZigBee and geographic information systems(GIS)technologies to resolve these problems and provide an effective communication *** experimental results of the ZigBee network system are presented,examples are provided of the mapping and analysis undertaken using GIS for the disaster-stricken area of Tsukuba City,Japan,and the communications node arrangements are determined for this *** results demonstrate the effectiveness of establishing such a communications system for supporting efforts to relieve disaster-damaged areas.

Discrepancy between the Image Held by Nursing Students of Themselves as Employed Nurses during the Pre-Employment Period and the Post-Employment Reality Faced by Novice Nurses

The difference between the expectations of work an individual forms before joining an organization and their perception of work after becoming an organization member is called reality shock and has a significant impact on the reasons why new nurses leave their jobs. To reduce reality shock, it is necessary to clarify the discrepancy between the reality and the image of oneself after employment, which is the source of expectations. However, while there is a large body of literature on reality shock among nurses, no studies have specifically investigated student nurses’ image of post-employment confidence. We conducted semi-structured interviews with eight nursing students who had received job offers and 10 professional nurses up to two years after graduation to clarify the differences between the image that nursing students hold of themselves as employed nurses during the period immediately before employment and the reality that they find after employment. As a result, four core categories (with 25 categories) related to participants’ images of themselves as employed nurses were extracted: life after employment, adaptation to work, adaptation to the workplace, and career foundation-building. Fourteen categories were extracted regarding the reality that nurses found after employment;upon comparing and classifying these categories according to the four core categories, a lack of concreteness in the nursing students’ image of themselves as employed nurses was identified. The results suggest that support measures, both in the basic nursing education program and in clinical hospital practice, to improve the concreteness of the image that nursing students hold of themselves as employed nurses and of nursing work can reduce reality shock and prevent job turnover upon their entry into the workforce.

Rate of local tumor progression following radiofrequency ablation of pathologically early hepatocellular carcinoma

AIM To evaluate whether pathologically early hepatocellular carcinoma(HCC) exhibited local tumor progression after radiofrequency ablation(RFA) less often than typical *** Fifty pathologically early HCCs [tumor diameter(mm): mean, 15.8; range, 10-23; follow-up days after RFA: median, 1213; range, 216-2137] and 187 typical HCCs [tumor diameter(mm): mean, 15.6; range, 6-30; follow-up days after RFA: median, 1116; range, 190-2328] were enrolled in this retrospective study. The presence of stromal invasion(namely, tumor cell invasion into the intratumoral portal tracts) was considered to be the most important pathologic finding for the diagnosis of early HCCs. Typical HCC was defined as the presence of a hyper-vascular lesion accompanied by delayed washout using contrastenhanced computed tomography or contrast-enhanced magnetic resonance imaging. Follow-up examinations were performed at 3-mo intervals to monitor for signs of local tumor progression. The local tumor progression rates of pathologically early HCCs and typical HCCs were then determined using the Kaplan-Meier *** During the follow-up period for the 50 pathologically early HCCs, 49(98%) of the nodules did not exhibit local tumor progression. However, 1 nodule(2%) was associated with a local tumor progression found 636 d after RFA. For the 187 typical HCCs, 46(24.6%) of the nodules exhibited local recurrence after RFA. The follow-up period until the local tumor progression of typical HCC was a median of 605 d, ranging from 181 to 1741 d. Among the cases with typical HCCs, local tumor progression had occurred in 7.0%(7/187), 16.0%(30/187), 21.9%(41/187) and 24.6%(46/187) of the cases at 1, 2, 3 and 4 years, respectively. Pathologically early HCC was statistically associated with a lower rate of local tumor progression, compared with typical HCC, when evaluated using a log-rank test(P = 0.002). ＜正＞FGF8, a member of the fibroblast growth factor （FGF） family, has been shown to play important roles in different developing systems, such as central nervous system （CNS） and limbuds. Mouse embryonic carcinoma P19 cells could be induced by retinoic acid （RA） to differentiate into neuroectodermal cell lineages, and this process is both RA and cell aggregation dependent. In this report, we showed that FGF8 mRNA and protein are transiently upregulated upon P19 cell aggregation. Overexpression of FGF8 in monolayer P19 cells could promote RA-induced P19 cell neural differentiation without aggregation. Inhibition of FGF8 expression by RNA interference （RNAi） and neutralizing of FGF8 protein by soluble FGFR3-Fc, as well as inhibition of FGF signaling by FGF receptor inhibitor, SU5402, could suppress P19 cell neural differentiation. We also showed that RA and FGF8 might induce P19 cell neural differentiation by synergistically inhibiting bone morphogenetic protein （BMP） signaling through distinct mechanisms. FGF8 inhibits Smadl nuclear translocation through Erk phosphorylation, and RA induces expression of Noggin and RA receptors, RARs, which virtually blocked Smadl-dependent transcriptional activity.

An all-sky and a scanning Fabry-Perot interferometers (FPI) have been developed for observations of vertical winds and two-dimensional atmospheric motions in the low-thermosphere and the mesosphere region by the measurement of Doppler shift and broadening of airglow emission lines. The system of FPI is described and the method of data analysis is proposed, an example of observation is also given.
