Background: Isolated tricuspid valve disease remains a controversial indication for surgical intervention. Many patients referred for surgery already have a poor clinical condition and an advanced New York Heart Association functional class. There is no consensus on the optimal surgical technique for this condition, including on whether to perform the procedure on a beating or an arrested heart and whether to perform valve repair or replacement. Methods: We analyzed four case series between 2015 and 2022 in which patients with secondary tricuspid regurgitation (TR) underwent valve repair on a beating heart and right atrial plication for a dilated right atrium. The TRI-SCORE was calculated for each patient. Results: All patients experienced a favorable postoperative course with significant improvements in heart failure symptoms. TR was markedly reduced;however, in one patient with concomitant mitral regurgitation (MR) and a high TRI-SCORE, MR worsened postoperatively. This patient later died from unknown causes due to multiple comorbidities in the late phase. Conclusions: Tricuspid valve repair on a beating heart was effective for improving the cardiac function, and the TRI-SCORE proved useful as a preoperative risk assessment tool. The underlying mechanism by which TR exacerbates MR requires further investigation.

利用高温冶金炼铁、炼钢技术,开发了一种新型的垃圾气化与熔融系统.住友金属开发的这种体系具有下列基本概念:用顶吹和侧吹氧枪实现稳态生产,产生无二恶英的高热值净化气和高质量的炉渣.系统的基本概念用一台规模为2 t/d的城市垃圾处理小型炉得到澄清.利用顶吹氧枪,由于在炉子上部产生均匀气流,从而可稳定炉子顶气的成分.顶吹氧枪的使用可防止粗粉尘(尺寸＞0.1 mm)的飞散,使熔化的粉尘均匀沉积在炉子内壁上.内壁上形成的粉尘沉积层不但可防止粉尘飞散,而且能保护炉子内壁.在2t/d规模小型炉试验结果的基础上,进一步启动了一台20 t/d规模的示范炉.示范炉和小型炉一样不用焦炭,能稳定地气化和熔化城市垃圾.由于在高温还原气氛下气化和快速冷却,结果可产生无二恶英的高热值净化气.垃圾中的灰分在高温还原气氛下熔融,得到不含重金属的高质量炉渣.

In materials science, the number of d-electrons of transition metals is an essentially important factor controlling characteristics of alloys and compounds. In this paper, we show an example to control the number of d-electrons (holes) by using inner-core electron excitation of zinc atoms. An important feature of our research is that we can make a long lifetime excited electronic state of zinc (3d8), and the life-time of excited zinc is more than 307 days. At first, the experimental apparatus and boundary conditions of the ion-recombination processes were explained. From results of XPS, excited zinc films showed satellites peaks what caused by the final state of 3d8 and the charge transfer final state of 3d10L2. Excited states of zinc were formatted at the surface of substrate caused by ion-recombination process between Zn+ and Zn-. The excited zinc diffused from substrate surface to the surface of the excited zinc thin film. Intensity of excited zinc is proportional to the intensity of electron on the substrate.

A case of abnormal chordae of the mitral valve (MV) associated with systolic anterior motion (SAM) is not commonly encountered in clinical practice. Here we report MV repair via the edge-to-edge technique in a 56-year-old male with MV regurgitation associated with papillary muscle displacement and posterior leaflet prolapse. Echocardiography revealed obstruction of the left ventricular outflow tract caused by SAM because of papillary muscle displacement and mitral regurgitation associated with posterior leaflet prolapse due to chordae rupture. MV repair was successfully performed by reconstruction with an Alfieri stitch. The outcome of this case demonstrated that this surgical technique is preferable for MV abnormalities with no complex malformations.

Palisaded encapsulated neuroma (PEN;solitary circumscribed neuroma) is a benign and hyperplastic lesion consisting of Schwann cells. PEN of the lower lip was reported by Tomich and Moll [1] 35 years ago. However, the accumulation of the information about PEN which occurred in the oral mucosa was not enough. This article describes a case of a PEN on the upper lip of a 41-year-old woman. The lesion with 0.7 cm diameter was performed excisional biopsy. Histologically, the tumor was almost circumscribed by thin fibrous capsule, and consisted of diffusely and dense proliferation of the spindle shape cells arranged in interlacing fascicles. Focal suggestions of nuclear pal-isaded growth were indicated within the tumor. Immunohistochemicallly, the fascicles of tumor cells were positive for S-100 protein, and vimentin and negative for α-actin and GFAP. A few numbers of axons were demonstrated by anti-neurofilament antibody in this lesion. Therefore, the definitive diagnosis was PEN.

Habitual alcohol consumers’ understanding of “moderate drinking”: A cross-sectional study in Japan

The Healthy Japan 21 project launched in FY2000 advocated dissemination of the knowledge that intake of about 20 g of pure alcohol per day represents a “moderate” drinking level. The aim of the present study was to explore various points that have been debated with regard to “moderate drinking”. An Internet-based questionnaire survey was administered to normal adults who habitually drank alcohol one or more days a week, and we studied the amount of alcohol consumed by those who responded that they did moderately without fail. Gender-based logistic regression analysis was conducted to examine the relationship between “non-moderate drinking” on the one hand and drinking frequency, marital status, and lifestyle habits on the other. Responses were obtained from 1088 persons (548 men and 540 women) (response rate: 57.6%), of whom 31.9% of the men and 53.6% of the women responded that they drank moderately without fail. Among these drinkers, 54.6% of the men and 78.2% of the women in reality drank moderately. For both men and women, the odds ratio (OR) for “non-moderate drinking” increased as drinking frequency increased. When “1 to 2 days a week” was taken as the reference, the OR values for “every day” were 8.69 (95% confidence interval [CI]: 4.98 - 15.14) for men and 7.35 (95% CI: 4.20 - 12.88) for women. Furthermore, significantly higher ORs were observed among both single men and single women, and among male smokers. The present study has clarified that dissemination of knowledge about “moderate drinking” has not been effective. Encouraging more awareness of the importance of drinking frequency is also considered necessary.

To investigate the surgical therapies for gastric cancer (GC) patients of age 85 or older in a multicenter *** opportunities for elderly GC patients have expanded in conjunction with extended life expectancy. However, the number of cases encountered in a single institution is usually very small and surgical therapies for elderly GC patients have not yet been standardized completely. In the present study, a total of 134 GC patients of age 85 or older who underwent surgery in 9 related facilities were retrospectively investigated. The relationships between surgical therapies and clinicopathological or prognostic features were ***-nine of the patients (66%) presented with a comorbidity, and 26 (19% overall) presented with more than two comorbidities. Radical lymphadenectomy was performed in 59 patients (44%), and no patient received pre- or post-operative chemotherapy. Forty of the patients (30%) experienced perioperative complications, but no surgical or perioperative mortality occurred. Laparoscopic surgery was performed in only 12 of the patients (9.0%). Univariate and multivariate analyses of the 113 patients who underwent R0 or R1 resection identified the factors of pT3/4 and limited lymphadenectomy as predictive of worse prognosis (HR = 4.68, P = 0.02 and HR =2.19, P = 0.05, respectively). Non-cancer-specific death was more common in cStage I patients than in cStage II or III patients. Limited lymphadenectomy correlated with worse cancer-specific survival (P = 0.01), particularly in cStage II patients (P < 0.01). There were no relationships between limited lymphadenectomy and any comorbidities, except for cerebrovascular disease (P = 0.07).CONCLUSIONNon-cancer-specific death was not negligible, particularly in cStage I, and gastrectomy with radical lymphadenectomy appears to be an effective treatment for cStage II elderly GC patients.

Despite recent advances in surgical techniques and perioperative management, the prognosis of pancreatic cancer(PCa) remains extremely poor. To provide optimal treatment for each patient with Pca, superior biomarkers are urgently needed in all phases of management from early detection to staging, treatment monitoring, and prognosis. In the blood of patients with cancer, circulating tumor cells(CTCs) and cell-free nucleic acids(cf NAs), such as DNA, m RNA, and noncoding RNA have been recognized. In the recent years, their presence in the blood has encouraged researchers to investigate their potential use as novel blood biomarkers, and numerous studies have demonstrated their potential clinical utility as a biomarker for certain types of cancer. This concept, called "liquid biopsy" has been focused on as a less invasive, alternative approach to cancer tissue biopsy for obtaining genetic and epigenetic aberrations that contribute to oncogenesis and cancer progression. In this article, we review the available literature on CTCs and cfN As in patients with cancer, particularly focusing on PCa, and discuss future perspectives in this field.

Mortality and Morbidity Following Surgery for Primary Malignant Musculoskeletal Tumors in the Pelvis and Limbs: A Retrospective Analysis Using the Japanese Diagnosis Procedure Combination Database

Introduction: Resection of malignant pelvic tumors has long been considered to be associated with higher postoperative mortality and morbidity than resection of malignant limb tumors. We compared the postoperative adverse events of pelvic tumor surgery and limb tumor surgery using a national inpatient database. Methods: We identified patients who underwent surgery for primary musculoskeletal malignant tumors of the pelvis or limbs between July and December in 2007- 2010 using the Japanese Diagnosis Procedure Combination inpatient database. We calculated the risk-adjusted odds ratio for the occurrence of postoperative complications following pelvic tumor surgery with reference to limb tumor surgery using a multivariable logistic regression analysis. Results: Of 3255 eligible patients, 3116 underwent limb tumor surgery and 139 underwent pelvic tumor surgery. In-hospital mortality was 0.6% and 0.7% and postoperative complication rates were 8.2% and 18.7%, respectively. The rate of blood transfusion and duration of anesthesia over 480 min were higher in the pelvic tumor group. Blood transfusion volume and duration of anesthesia were independently associated with worse outcomes, but there was no significant association between tumor location and occurrence of postoperative complications (odds ratio 1.18, 95% confidence interval 0.73 - 1.88, p = 0.502). Conclusions: Blood transfusion volume and duration of anesthesia were significant predictors of outcome. Our data demonstrate that the higher morbidity rate after pelvic tumor resection could result from the larger blood transfusion volume and longer anesthesia duration.
