Clinical Significance of Biological Markers at Primary Operation for Metastatic Breast Cancer

Abstract：Purpose: To identify the prognostic value of biological markers at initial operation for metastatic breast cancer, we measured the presence of estrogen receptor-alpha (ERα), progesterone receptor (PgR) and human epidermal growth factor receptor type 2 (HER2),and histological grade (HG) of tumors. Methods: One-hundred and seventy-six patients, aged 29 to 90 (median: 51 years), with recurrent breast cancer underwent primary operation at our department during the period from 1983 to 2000. Clinicopathological factors examined at primary operation included menopausal symptoms, presence of axillary lymph node metastasis, tumor size, HG, HER2, ERα and *** examined at recurrence included site of primary recurrence, disease-free interval(DFI) and tumor markers, such as CEA and CA15-3. The relationship between these factors and prognosis following recurrence was assessed. Results: Menopausal status, axillary lymph node metastasis and tumor size at primary operation had no significant effect on prognosis. Patients with low HG, positive expession of ERα and PgR, and low HER2 expression had a good prognosis, similar to those with long DFI and distant metastases. After distant metastases, HER2 was found to be the most important prognostic factor following recurrence and in predicting response to drug ***: Biological factors indicating tumor malignancy at the time of the first operation are also important prognostic factors following tumor recurrence.

Risk Factors Associated with In-Hospital Post-Chemotherapy Mortality in Patients with Malignant Musculoskeletal Tumors

Introduction: Reducing the in-hospital post-chemotherapy mortality rate in patients with malignant musculoskeletal tumors is important for improving treatment outcome. This study aimed to investigate the risk factors associated with in-hospital post-chemotherapy mortality in patients with primary malignant musculoskeletal tumors. Methods: Using a Japanese national inpatient database, we retrospectively identified 5039 patients (2920 men and 2131 women;mean age, 39 years) who underwent curative chemotherapy for malignant musculoskeletal tumors between 2007 and 2010. We extracted data on the patients’ characteristics, complications, chemotherapeutic agent use, comorbidities, and in-hospital death. Logistic regression analyses were performed to analyze factors affecting in-hospital post-chemotherapy death in these patients. Results: The overall in-hospital mortality rate was 1.1%. Higher in-hospital mortality rates were significantly associated with a greater volume of blood transfusion (>2500 mL) (odds ratio [OR], 49.71;95% confidence interval [CI], 22.24 - 111.12;p < 0.001), diabetes mellitus (OR, 3.05;95% CI: 1.21 - 7.70;p = 0.019), and older age (OR, 3.05;95% CI, 1.11 - 8.37;p = 0.031). Conclusions: Higher in-hospital post-chemotherapy mortality rates were associated with massive blood transfusion, which was associated with a 16-fold higher risk of in-hospital mortality compared with other risk factors. Blood transfusion volume should be considered an important indicator for deciding whether the next cycle of chemotherapy is administered continuously or not.

IMPACTS OF CLIMATE CHANGE ON TROPICAL CYCLONES IN THE WESTERN NORTH PACIFIC BASIN. PART Ⅱ: LATE TWENTY-FIRST CENTURY PROJECTIONS

This paper reviews the latest studies on the relationship between projected late 21 st century climate changes and tropical cyclone(TC) activity in the western North Pacific(WNP) basin, which is the region of the United Nations Economic and Social Commission for Asia and the Pacific(ESCAP)/World Meteorological Organization(WMO) Typhoon Committee members. Existing studies of projected changes of TC activity in this basin, such as frequency, intensity, precipitation, genesis location and track pattern are summarized, based on an assumed A1 B future climate change scenario. A review of available studies on projected future changes in WNP landfalling TC activity is also *** it remains uncertain whether there has been any detectable human influence on tropical cyclone frequency, intensity, precipitation, track, or related aggregated storm activity metrics in the basin, modeling studies suggest changes in future tropical cyclone activity for the WNP basin. More models project decreases than increases in tropical storm frequency(range-70% to +60%);most studies project an increase in the TC intensity(range-3% to +18%);and all six available studies that include the WNP basin project increases in TC precipitation rates(range +5 to +30%).

Early thrombomodulin-α administration outcome for acute disseminated intravascular coagulopathy in gastrointestinal surgery

AIM To investigate the efficacy of thrombomodulin(TM)-α for treatment of disseminated intravascular coagulopathy(DIC) in the field of gastrointestinal surgery. METHODS Thirty-six peri-operative DIC patients in the field of gastrointestinal surgery who were treated with TM-α were retrospectively investigated. The relationships between patient demographics and the efficacy of TM-α were examined. Analysis of survival at 28 d was also performed on some parameters by means of the Kaplan-Meier method. Relationships between the ini-tiation of TM-α and patient demographics were also evaluated. RESULTS Abscess formation or bacteremia was the most frequent cause of DIC(33%), followed by digestive tract perforation(31%). Twenty-six patients developed DIC after surgery, frequently within 1 wk(81%). TM-α was most often administered within 1 d of the DIC diagnosis(72%) and was continued for more than 3 d(64%). Although bleeding tendency was observed in 7 patients(19%), a hemostatic procedure was not needed. DIC scores, systemic inflammatory response syndrome(SIRS) scores, quick-sequential organ failure assessment(qS OFA) scores, platelet counts, and prothrombin time ratios significantly improved after 1 wk(P < 0.05, for all). The overall survival rate at 28 d was 71%. The duration of TM-α administration(≥ 4, ≤ 6) and improvements in DIC-associated scores(DIC, SIRS and q SOFA) at 1 wk were significantly better prognostic factors for 28-d survival(P < 0.05, for all). TM-α was administered significantly earlier to patients with severe clinical symptoms, such as high qS OFA scores, sepsis, shock or high lactate values(P < 0.05, for all). CONCLUSION Early administration of TM-α and improvements in each parameter were essential for treatment of DIC. The diagnosis of patients with mild symptoms requires further study.

Liquid biopsy in patients with hepatocellular carcinoma: Circulating tumor cells and cell-free nucleic acids

Hepatocellular carcinoma(HCC), with its high incidence and mortality rate, is one of the most common malignant tumors. Despite recent development of a diagnostic and treatment method, the prognosis of HCC remains poor. Therefore, to provide optimal treatment for each patient with HCC, more precise and effective biomarkers are urgently needed which could facilitate a more detailed individualized decision-making during HCC treatment, including the following; risk assessment, early cancer detection, prediction of treatment or prognostic outcome. In the blood of cancer patients, accumulating evidence about circulating tumor cells and cell-free nucleic acids has suggested their potent clinical utilities as novel biomarker. This concept, so-called "liquid biopsy" is widely known as an alternative approach to cancer tissue biopsy. This method might facilitate a more sensitive diagnosis and better decision-making by obtaining genetic and epigenetic aberrations that are closely associated with cancer initiation and progression. In this article, we review recent developments based on the available literature on both circulating tumor cells and cell-free nucleic acids in cancer patients, especially focusing on Hepatocellular carcinoma.

Chitosan Sub-micron Particles Prepared Using Sulfate Ion Salt as Bacteriostatic Materials in Neutral pH Condition

In this paper, the newly developed ion exchange phase separation method to create chitosan sub-micron particles is introduced: 1) chitosan was dissolved in a lactic acid aqueous solution. 2) the obtained chitosan solution was added stepwise in a sodium sulfate aqueous solution and cooled down to 5℃ to become slightly turbid through agglutination. 3) desalinating and deacidifying of the mixture was carried out by a dialyzing tube method. IR spectroscopy and elemental analysis indicated that the agglutination of chitosan was induced by crosslinking effect with an electrostatic interaction between sulfate anions and amino groups in the glucosamine unit although large excess of Na2SO4 caused undesirable further agglutination of the resultant chitosan particles. As a result, the proper amount of Na2SO4 was approximately 1.0 - 10.0 equivalent for the amino group to create the chitosan particles with a sub-micron size. In addition, we investigated an antibacterial activity test for Escherichia coli of the obtained chitosan particles. The significant antibacterial activity was observed in incubation even at neutral pH condition while the chitosan microbeads (size: ca 200 ∫m) prepared by the conventional method and chitosan granules (size: ca 600 ∫m) as starting materials showed almost no antibacterial activity in the same condition.

Laparoscopic Choledochoduodenostomy for Biliary Alleviation

Choledochoduodenostomy (CDD) was very useful alternatives for treatment for patients with common bile duct (CBD) stones, especially recurrent stones, giant stones with choledochal dilatation, and difficult or failed cases by endoscopic treatment. Furthermore, CDD was also applied to biliary bypass by malignant obstruction. In these days, minimal invasive laparoscopic approach is adapted in these disorders. We have conducted to perform a laparoscopic CDD for biliary alleviation for patients with endoscopic management of difficult CBD stone with choledochal dilatation. A side-to-side CDD was created intra-corporeally using water-tight running absorbable suture by handmade 4-0 monofilament with double side needles, starting from the right side of choledochus, and continued along posterior wall until the left side of the choledochus followed by anterior-wall anastomosis as the same manner. Five patients were treated successfully by this laparoscopic procedure and remained well without bile leakage and reflux cholangitis for the short-term follow-up. The median operative time and intracorporeally anastomosis time were 182 (167 - 209) min and 33 (30 - 38) min, respectively. Median blood loss was 32 (little-90) ml, median hospital stay was 7 (5 - 14) days, and median follow-up time was 18 months. Although this series was relatively small, this laparoscopic technique is feasible and safe for biliary alleviation, especially for endoscopic management of difficult or failed CBD stones, and would also potentially adapt to biliary bypass by malignant obstruction.

＜正＞ They have, for a long time, cried for town planning with the initiative by residents in order to make landscapes in harmony with existing areas. It needs to reassess local resources. In this research, we selected the Okamoto, district in the Higashinada ward, in the Kobe city where there are regulations for landscaping. The purpose of these regulations was to create it in harmony with relating to town planning with the initiative by residents. We conducted a landscape investigation using the caption evaluation method. In mis sequence, all the participants were students and this was a part of graphic science education. Furthermore, we reported the results of the investigation at a workshop of "Okamoto the Beautiful Town" council, which was followed by a discussion to re-appraise local resources. We explored further into the possibilities of the caption evaluation method in such a sequence.

Endoventricular Spiral Plication for Ischemic Dilated Cardiomyopathy

Surgical ventricular restoration (SVR) procedures have been developed;however, their long-term effectiveness remains controversial. Although a series of endoventricular spiral plication (ESP) has been rarely reported and its long prognosis is still unknown;this method has a unique concept of left ventricular (LV) restoration without artificial patch materials. Here, we describe the case of a patient with ischemic cardiomyopathy and ischemic mitral regurgitation who successfully underwent ESP, mitral valve repair, and coronary artery bypass grafting. ESP was effective in papillary muscle approximation for avoiding heart failure;however, the noted improvement of LV wall thickening might be temporary.

Real-time electroholography using a single spatial light modulator and a cluster of graphics-processing units connected by a gigabit Ethernet network

Systems containing multiple graphics-processing-unit(GPU)clusters are difficult to use for real-time electroholography when using only a single spatial light modulator because the transfer of the computer-generated hologram data between the GPUs is *** overcome this bottleneck,we propose a rapid GPU packing scheme that significantly reduces the volume of the required data *** proposed method uses a multi-GPU cluster system connected with a cost-effective gigabit Ethernet *** tests,we achieved real-time electroholography of a three-dimensional(3D)video presenting a point-cloud 3D object made up of approximately 200,000 points.
