Watercore is a physiological disorder in apple(Malus×domestica Borkh.)fruits that appears as water-soaked tissues adjacent to the vascular core,although there is little information on what exactly occurs at cell level in the watercored apples,particularly from the viewpoint of cell water *** combining picolitre pressure-probe electrospray-ionization mass spectrometry(picoPPESI-MS)with freezing point osmometry and vapor pressure osmometry,changes in cell water status and metabolisms were spatially assayed in the same *** the watercored fruit,total soluble solid was lower in the watercore region than the normal outer parenchyma region,but there was no spatial difference in the osmotic potentials determined with freezing point ***,a disagreement between the osmotic potentials determined with two methods has been observed in the watercore region,indicating the presence of significant volatile compounds in the cellular fluids *** the watercored fruit,cell turgor varied across flesh,and a steeper water potential gradient has been established from the normal outer parenchyma region to the watercore region,retaining the potential to transport water to the watercore ***-specific analysis using picoPPESI-MS revealed that together with a reduction in turgor,remarkable metabolic modifications through fermentation have occurred at the border,inducing greater production of watercore-related volatile compounds,such as alcohols and esters,compared with other *** alcohols including ethanol have low reflection coefficients,it is very likely that these molecules would have rapidly penetrated membranes to accumulate in apoplast to *** addition to the water potential gradient detected here,this would physically contribute to the appearance with high tissue transparency and changes in colour ***,it is concluded that these spatial changes in cell water relations are closely associated with watercore symptom

Advantage of autologous blood transfusion in surgery for hepatocellular carcinoma

AIM: To evaluate the significance of autologous blood transfusion (AT) in reducing homologous blood transfusion (HT) in surgery for hepatocellular carcinoma (HCC). METHODS: The proportion of patients who received HT was compared between two groups determined by the time of AT introduction; period A (1991-1994, n = 93) and period B (1995-2000, n = 201). Multivariate logistic regression analysis was performed in order to identify independent significant predictors of the need for HT. We also investigated the impact of AT and HT on long-term postoperative outcome after curative surgery for HCC. RESULTS: The proportion of patients with HT was significantly lower in period B than period A (18.9% vs 60.2%, P < 0.0001). Multivariate logistic regression analysis identified AT administration as a significant independent predictor of the need for HT (P < 0.0001). Disease-free survival in patients with AT was comparable to that without any transfusion. Multivariate analysis identified HT administration as an independent significant factor for poorer disease-free survival (P = 0.0380). CONCLUSION: AT administration significantly decreased the need for HT. Considering the postoperative survival disadvantage of HT, AT administration could improve the long-term outcome of HCC patients.

AIM:To investigate whether fresh frozen plasma (FFP) transfusion affects outcomes following hepatic resection for hepatocellular carcinoma (HCC) in terms of liver function,postoperative complications and cancer ***:We retrospectively compared the incidence of postoperative complications between 204 patients who underwent hepatectomy for HCC with routine FFP transfusion in an early period (1983-1993,Group A) and 293 with necessity for FFP transfusion during a later period (1998-2006,Group B),and also between two subgroups of Group B [22 patients with FFP transfusion (Group B1) and 275 patients without FFP transfusion (Group B2)].Additionally,only in limited patients in Group B1 and Group B2 with intraoperative blood loss≥ 2000 mL (Group B1 ≥ 2000 mL and Group B2 ≥ 2000 mL),postoperative complications,liver function tests,and cancer prognosis were ***:No mortality was registered in Group B,compared to 8 patients (3.9%) of Group *** incidence of morbidity in Group B2 [23.2% (64/275)] was not significantly different from Group B1 [40.9% (9/22)] and Group A [27.0% (55/204)].The incidence of complications and postoperative liver function tests were comparable between Group B1 ≥ 2000 mL vs Group B2 ≥ 2000 *** prognosis did not correlate with administration of FFP,but with tumor-related ***:The outcome of hepatectomy for HCC is not influenced by FFP *** suggest FFP transfusion be abandoned in patients who undergo hepatectomy for HCC.

We describe a polarization rotator based on a parallel-core structure consisting of a silicon nanowire waveguide and a silicon-nitride *** 60-μm-long rotator provides a polarization extinction ratio of more than 10 dB with excess loss of less than 1 *** addition,the extremely wide bandwidth of more than 150 nm expected from calculations is confirmed in experiments.A study of the fabrication tolerance of our rotator indicates that fabrication error of around 25 nm is allowable.

Adult Congenital Lumbar Kyphosis Requiring Anteroposterior Correction and Fusion: A Case Report with 32-Year Follow-Up

Congenital pure kyphosis due to failure of vertebral body segmentation is a relatively rare entity, and surgical intervention is infrequent compared to that for failure of vertebral body formation [1] [2]. There are very few reports of long-term follow-up of surgical treatment in patients with congenital pure kyphosis, and all the reported cases were diagnosed as failure of formation and had an age at the time of surgery of less than 18 years. It is important for orthopedic surgeons to follow the postoperative course of rare cases over 30 years. Here, we present a surgically treated case with ultra-long term follow-up of a 50-year-old patient with congenital pure kyphosis of the lumbar spine. Imaging of the lumbar spine showed six vertebrae and an unsegmented bar at L3-4 causing a pure kyphosis of 54°. The wedge-shaped block vertebra had 4 pedicles with the neural foramen between the pedicles without concomitant disc space, with compensatory thoracic hypokyphosis and lower lumbar hyperlordosis. One-stage correction and fusion surgery using anterior opening and posterior closing osteotomy was successfully performed. Both clinical and radiographic results were excellent and have been maintained for over 30 years postoperatively. The basic principle in the surgical treatment of adult spinal deformity is to achieve and maintain a good global sagittal balance over time. This case reaffirms the importance of spinopelvic harmony.

A Patient with Spinal Arteriovenous Malformation Treated with Lumbar Vertebral Surgery Due to Misdiagnosis

Spinal arteriovenous malformation (AVM) is a rare disease that arises more commonly from the lower thoracic and upper lumbar spine. It is classified based on the development pattern and the shunt location. The developmental mechanism of the symptoms is thought to be as follows. Spinal venous return is impaired by high-pressure arterial blood flowing into the coronary sinus via a shunt and venous pressure is promoted, which causes spinal cord symptoms to progress gradually. Listlessness, pain, and an abnormal sensation of the lower limb are possible initial symptoms. Spinal AVM may cause intermittent claudication and bladder and rectal disturbance, and differentiation from lumbar degenerative diseases and arteriosclerosis obliterans is required, which may cause difficulty with diagnosis. We encountered a patient in whom intermittent claudication was treated with lumbar decompression and fixation at another hospital, but symptoms did not improve. The patient was diagnosed with spinal AVM at our hospital and symptoms were improved by surgery. Because the symptoms did not improve despite being treated with surgery for spinal canal stenosis, we strongly suspected spinal intermittent claudication, and we performed a spinal CT and MRI after myelography for the entire spinal cord, and identified the lesion in the thoracic spinal cord. Regarding the postoperative outcome and prognosis of spinal dural AVF, early diagnosis and early treatment have been proposed as prognostic factors because the postoperative outcome is poor in patients with high preoperative severity and a long duration of illness. In our patient, the preoperative JOA score was 6, showing high preoperative severity, and the duration of illness was 1.5 years before diagnosis, which may explain the limited improvement of the JOA score to 20 at final follow-up. In a case with these characteristics, we suggest that close examination of the entire spinal cord is needed for effective treatment.

Association of depressive symptoms with dietary habits among Japanese physicians

The aim of this study was to determine the association of depressive symptoms with dietary habits among physicians working in hospitals in Japan. We mailed an anonymous questionnaire with the Quick Inventory of Depressive Symptomatology and items about dietary habits to 10,000 randomly selected physicians. Logistic regression analysis was used to explore the association of depressive symptoms with dietary habits. The participants comprised 3862 physicians (3025 men and 837 women). Among the respondents, 252 (8.3%) men and 88 (10.5%) women were determined to be in depressive symptoms. For men, “never eat a balanced diet” (Odds ratio;95% confidence interval, 2.54;1.70 - 3.80), and for women “eat quite small meals” (3.81;1.14 - 13.1), “always eat until full” (4.40;1.48 - 13.1), and “never eat a balanced diet” (3.05;1.49 - 6.23) were associated with depressive symptoms. For men, “do not eat too much” (0.57;0.39 - 0.83) was associated with prevention of depression. Physicians should recognize the important role of healthy dietary habits in good mental health. Hospitals should intervene to help physicians address depressive symptoms and dietary habits.

Background: Basic principle for the treatment of pyogenic spondylitis (PS) is conservative care, but surgical intervention is often required when conservative treatment may fail. We have experienced many conservative cases of various complications due to long-term bed rest and poor pain control. Recently we have adopted percutaneous pedicle screw (PPS) fixation for the treatment of PS as a minimally invasive spine stabilization (MISt) fusion to reduce such morbidity of the conservative care. Objective: To evaluate the impact of PPS fixation in patients with PS. Study Design: A retrospective analysis of the medical records. Evaluation of Reactive Oxygen Species (ROS) Generated on the Surface of Copper Using Chemiluminesence

The antibacterial activity of copper is well-known from an ancient civilization, however, its biocidal mechanism has not been necessarily elucidated. Notwithstanding up to now, mainly 4 processes have been proposed. Among them, it is cleared that 4 kinds of reactive oxygen species (ROS): hydroxyl radical ·OH, hydrogen per oxide H2O2, superoxide anion ·O-2 and singlet oxygen 1O2, play an important role for contact-killing of bacteria, viruses and fungi. In this paper, generation of ROS on the surfaces of copper plates heated from room temperature to 673 K for 4.2 × 102 s in air, was investigated using the chemiluminescence. ROS have been evaluated by selecting the most suitable scavengers, such as 2-propanol for ·OH, sodium pyruvate for H2O2, nitro blue tetrazolium for ·O-2, and sodium azide NaN3 for 1O2. At the same time the outermost surface of copper, on which thin film of cuprous oxide Cu2O was first formed and then cupric oxide CuO was laminated on Cu2O, was examined by thin-film XRD and TEM analysis to estimate the amounts and kinds of copper oxides. It was found that the most amounts of ROS were obtained for the 573 K-heated Cu plate and they were composed of ·OH, H2O2, and ·O-2..

The Effects of Anma (Traditional Japanese Massage)—Randomised Trial

Background and Objective: Anma is a traditional Japanese bodywork therapy that has not been widely known and used in the West. There have been only a few Anma studies published in English journals. To study the effect of Anma (traditional Japanese massage) among participants who have neck and shoulder stiffness symptoms (so-called Katakori, in Japanese). Methods: The study participants consisted of seventy-seven (Study 1), thirteen (Study 2), and twenty (Study 3) adults with “Katakori” symptoms. The research design is as follows: (Study1) Randomized, Parallel-Group, Controlled Trial. (Study 2) Crossover Clinical Trial. (Study 3) Randomized, Parallel-Group, Controlled Trial. And we conducted Anma treatment for 45 minutes (Treatment) or rest in lying position for 45 minutes (Control). Results: In study 1, the symptom of “Katakori” was relieved after Anma treatment in Anma group (p d: 2.2). There was a significant interaction between the Anma group and the control group (p < 0.001). In study 2, MBV significantly increased following Anma treatment. There was a significant interaction between the Anma group and the control group (p = 0.022). In study 3, the symptom of “Katakori” was relieved after Anma treatment in the treatment group. There were no significant interactions between the groups in VAS and MBV values. Discussion and Conclusions: The study demonstrated that Anma therapy decreases “Katakori” symptoms while increasing MBV in the shoulder region.
