Sofosbuvir/Ribavirin therapy for patients experiencing failure of ombitasvir/paritaprevir/ritonavir + ribavirin therapy: Two cases report and review of literature

BACKGROUND The effectiveness of sofosbuvir/ribavirin(SOF/RBV) combination therapy,which is one of the 1 st-choice therapeutic options for patients with hepatitis C virus(HCV) genotype 2(HCV-G2) in Japan according to the most recent version of the Japan Society of Hepatology guideline, for patients who experienced failure of the ombitasvir/paritaprevir/ritonavir plus ribavirin(OBV/PTV/r+RBV) combination therapy, which was another option for patients with HCV-G2, is *** SUMMARY We evaluated the effects of SOF/RBV combination therapy in two patients with genotype 2 a who could not achieve a sustained virological response(SVR) by OBV/PTV/r+RBV combination therapy. One patient was complicated with VogtKoyanagi-Harada(VKH) disease. Resistance-associated variations before SOF/RBV combination therapy were not detected in two patients. Both patients had an SVR at 12 wk after the treatment(SVR12). Regarding adverse events(AEs), itching, chill, a dull feeling in the throat and cough as well as increase of alanine transaminase level were shown in one patient, while a headache and deterioration of light aversion probably due to the recurrence of VKH disease were shown in the other patients. In addition, the latter patient developed arthralgia and morning stiffness approximately 7 wk after the therapy and turned out to be diagnosed with rheumatoid *** SOF/RBV therapy might be effective for patients experiencing failure of OBV/PTV/r+RBV therapy, but caution should be taken regarding the AEs.

关于本次世界经济危机,马克思主义经济学家认为原因有二:一是认为这场危机是"新自由主义的失败";二是将其当作由生产过剩引发的资本主义循环性危机去理解。但是,观点二并没有解释这场经济危机为何成为了"百年不遇"的事件;而观点一也没有解释自由主义的失败是资本主义的失败。笔者认为,新自由主义是资本主义衰落的必然结果,所以,新自由主义的失败就是资本主义的失败。为此,对上述两种观点进行了否定,认为这次危机的本质是发达资本主义国家不想适应"零增长"的新的经济社会制度。另外,反对金融资本主义,认为应该重视马克思的生产性劳动理论。

A Phase II Study of Erlotinib in Patients with Previously Treated Non-Small Cell Lung Cancer

Background: Erlotinib has been reported to be effective for the treatment of non-small cell lung cancer (NSCLC). To evaluate the efficacy and safety of erlotinib under conditions similar to daily clinical practice, a phase II trial was conducted in Japanese patients with previously treated NSCLC. Methods: The eligibility criteria were stage IIIB/IV NSCLC, a performance status (PS) of 0 - 2, and previous treatment with 1 - 2 non-EGFR-TKI regimens. Patients received erlotinib (150 mg/day) orally until disease progression or intolerable toxicity occurred. The primary endpoint was the objective response rate (ORR). In addition, the disease control rate (DCR), progression-free survival (PFS), overall survival (OS), safety, and EGFR gene mutation status were evaluated. Results: Thirty-eight patients were enrolled, and 37 patients were evaluated. The median age was 69 years (range, 50 - 80 years). Patient characteristics were as follows: 26 were male and 11 were female;12 had a PS of 0, 20 had a PS of 1, and 5 had a PS of 2;and 26 had adenocarcinoma, and 11 had non-adenocarcinoma histology. The ORR and DCR were 21.6% (95% confidence interval [CI], 11.4% - 37.2%) and 54.1% (95% CI, 35.9% - 66.6%), respectively. Twenty-seven patients could be evaluated for EGFR gene status (12, mutated;15, wild-type). The ORR for EGFR-mutated patients was 41.7%, while that for patients with wild-type EGFR was 13.3%. The median PFS was evaluated as 4.4 months (95% CI, 2.2 - 10.7 months). The median OS was 14.9 months (95% CI, 9.2 months - not reached). Common adverse events were tolerable skin toxicities, diarrhea, and stomatitis. In addition, interstitial lung disease occurred in 8.1% of patients. Conclusion: As efficacy and safety were similar to previous studies, erlotinib was found to be effective for Japanese patients with previously treated NSCLC in clinical practice.

Effects of uniaxial compressive strain on the electronic- transport properties of zigzag carbon nanotubes

Recently, substantial attention has been paid to the strain sensitivity of the carbon nanotubes＇（CNTs＇） electronic properties. In this study, the relationships between the geometric structures and electronic states of zigzag CNTs under uniaxial compressive strain were investigated. We found that different factors dominate the electronic states of zigzag CNTs depending on the strain regions： the initial stage of the strain loading, which lasts until column-buckling deformation begins, and the strain regions corresponding to column- and shell-buckling deformations. Because shell-buckling deformation significantly increases the re-orbital angle, the angle between the π orbital axis vectors of adjacent atoms, strong localization of the density of states （LDOS） occurs in the buckled area. We also analyzed the current able to pass through deformed CNTs using a tight-binding-based Green＇s function approach and determined that the current can be significantly suppressed by applying uniaxial compressive strain. Our method of predicting the electronic state of a deformed CNT based on the π-orbital angle is expected to be useful for predicting the electronic properties of CNT-based electronic devices and sensors.

Role of gut microbiota and Toll-like receptors in nonalcoholic fatty liver disease

Emerging data have shown a close association between compositional changes in gut microbiota and the development of nonalcoholic fatty liver disease(NAFLD).The change in gut microbiota may alter nutritional absorption and *** addition,gut microbiota are a source of Toll-like receptor(TLR)ligands,and their compositional change can also increase the amount of TLR ligands delivered to the *** ligands can stimulate liver cells to produce proinflammatory ***,the gut-liver axis has attracted much interest,particularly regarding the pathogenesis of *** abundance of the major gut microbiota,including Firmicutes and Bacteroidetes,has been considered a potential underlying mechanism of obesity and NAFLD,but the role of these microbiota in NAFLD remains *** reports have demonstrated that certain gut microbiota are associated with the development of obesity and *** instance,a decrease in Akkermansia muciniphila causes a thinner intestinal mucus layer and promotes gut permeability,which allows the leakage of bacterial *** to increase Akkermansia muciniphila improve the metabolic parameters in obesity and *** children,the levels of Escherichia were significantly increased in nonalcoholic steatohepatitis(NASH)compared with those in obese *** can produce ethanol,which promotes gut ***,normalization of gut microbiota using probiotics or prebiotics is a promising treatment option for *** addition,TLR signaling in the liver is activated,and its downstream molecules,such as proinflammatory cytokines,are increased in *** data,TLR2,TLR4,TLR5,and TLR9 have been shown to be associated with the pathogenesis of ***,gut microbiota and TLRs are targets for NAFLD treatment.
