Nanoparticles-based nanochannels assembled on a plastic flexible substrate for label-free immunosensing

A novel, cheap, disposable and single-use nanoparticles-based nanochannel platform assembled on a flexible substrate for label-free immunosensing is pre- sented. This sensing platform is formed by the dip-coating of a homogeneous and assembled monolayer of carboxylated polystyrene nanospheres （PS, 200 and 500 nm-sized） onto the working area of flexible screen-printed indium tin oxide/polyethylene terephthalate （ITO/PET） electrodes. The spaces between the self-assembled nanospheres generate well-ordered nanochannels, with inter-PS particles distances of around 65 and 24 nm respectively. The formed nanochannels are used for the effective immobilization of antibodies and subsequent protein detection based on the monitoring of [Fe（CN）6]^4- flow through diffusion and the decrease in the differential pulse voltammetric signal upon immunocomplex formation. The obtained sensing system is nanochannel-size dependent and allows human immunoglobulin G （IgG）（chosen as a model analyte） to be detected at levels of 580 ng/mL. The system also exhibits an excellent specificity against other proteins present in real samples and shows good performance with a human urine sample. The developed device represents an integrated and simple biodetection system which overcomes many of the limitations of previously reported nanochannels-based approaches and can be extended in the future to several other immuno and DNA detection systems.

Effects of Earth's curvature and radial heterogeneity in dislocation studies:Case studies of the 2008 Wenchuan earthquake and the 2004 Sumatra earthquake

Recently, effects of Earth＇s curvature and radial heterogeneity on coseismic deformations are often investi- gated based on the 2004 Sumatra earthquake. However, such effects are strongly related to earthquake types. As a low dip angle event, the 2004 Sumatra earthquake is not a good seismic case for such a topic since the effects for moderate dip angle events are much bigger. In this study, the half-space and spherical dislocation theories are used, respectively, to calculate co- seismic displacements caused by the 2008 Wenchuan earthquake and the 2004 Sumatra earthquake. Effects of Earth＇s curva- ture and stratification are investigated through the discrepancies of results calculated using the two dislocation theories. Re- sults show that the effects of Earth＇s curvature and stratification for the 2008 Wenchuan earthquake are much larger than those for the 2004 Sumatra earthquake. Ignoring the effects will cause errors up to 100%-200% in far field displacements for a moderate dip angle event like the 2008 Wenchuan earthquake. Such great effects are much bigger than those conclusions of previous studies. Besides, comparison with observations verifies that spherical dislocation theories yield better results than half-space ones in far fields.

Natural regression of fibrosis in chronic hepatitis B

The fibrosis of liver cirrhosis was considered to be irreversible before the anti-viral drugs showed that it is reversible when they lead to continuous suppression of viral replication and inflammation. However, several reports previously showed that fibrosis of type B liver cirrhosis was almost completely absorbed after the natural remission of chronic inflammation. This phenomenon might not be limited to exceptional patients, but rather occur commonly, considering the dynamic clinical features of chronic hepatitis B(CHB), where inactive carrier stage normally follows aggravation of hepatitis and progression of fibrosis at the time of HBe Ag seroconversion. Thus, fibrosis levels of CHB as a hepatocellular carcinoma(HCC)-surveillance marker, particularly those of the inactive stage, could be underestimated, because some of them might have been(pre)cirrhotic in the past and recovered with the natural regression of fibrosis. We argue that cirrhosisinduced HCC mechanisms, rather than direct action of viral genome, may be more common than generally considered in CHB patients. This may have some impact on reconsidering the surveillance rationale for HCC in CHB, from where advanced HCCs tended to be missed. In addition, a molecular marker to assess the cancer-prone characteristics of the liver will definitely be needed to resolve the issue.

Optimal treatment strategies for hepatic portal venous gas:A retrospective assessment

Impact of continuous positive airway pressure therapy for nonalcoholic fatty liver disease in patients with obstructive sleep apnea

BACKGROUND Obstructive sleep apnea(OSA)has been suggested as an independent risk factor for nonalcoholic fatty liver disease(NAFLD),and continuous positive airway pressure(CPAP)is the first-line therapy for *** To clarify the efficacy of effective CPAP therapy on NAFLD of OSA patients by serum markers and transient elastography(TE)using FibroScan®(Echosens,Paris,France).METHODS We prospectively enrolled 123 consecutive patients with OSA who met the indications for *** fibrosis and steatosis were assessed using *** and after 6 mo of CPAP therapy,serum markers and TE were assessed for all *** mean usage rate of CPAP therapy for 6 mo was arbitrarily calculated in each patient and expressed as“mean compliance index”(m-CI).RESULTS In 50 OSA patients with NAFLD,both aspartate aminotransferase(AST)and alanine aminotransferase(ALT)levels were significantly decreased after 6 mo of CPAP *** analysis showed that decreased body weight(BW),decreased body mass index(BMI),decreased AST level,decreased hemoglobin A1c,and high m-CI were significantly related with improved ALT *** multivariate regression model adjusted for quantities of BW change during 6 mo of CPAP therapy,high m-CI tended to improve ALT level(P=0.051).All 17 OSA patients with NAFLD,high m-CI and no BMI changes showed significant improvements in AST and ALT ***,no significant changes in TE data or serum fibrosis markers were *** Some NAFLD could be associated with chronic intermittent hypoxia due to OSA independent of BW *** those cases,adequate reoxygenation from effective CPAP therapy may improve NAFLD.

Late-onset acute rejection after living donor liver transplantation

AIM： TO investigate the incidence and risk factors of late-onset acute rejection （LAR） and to clarify the effectiveness of our immunosuppressive regime consisting of life-long administration of tacrolimus and steroids. METHODS： Adult living donor liver transplantation recipients （n = 204） who survived more than 6 mo after living donor liver transplantation were enrolled. Immunosuppression was achieved using tacrolimus and methylprednisolone. When adverse effects of tacrolimus were detected, the patient was switched to cyclosporine. Six months after transplantation, tacrolimus or cyclosporine was carefully maintained at a therapeutic level. The methylprednisolone dosage was maintained at 0.05 mg/kg per day by oral administration. Acute rejections that occurred more than 6 mo after the operation were defined as late-onset. The median follow-up period was 34 too. RESULTS： LAR was observed in 15 cases （7%） and no chronic rejection was observed. The incidence of hyperlipidemia, chronic renal failure, new-onset post-transplantation diabetes, and deep fungal infection were 13%, 2%, 24%, and 17%, respectively. Conversion from tacrolimus to cyclosporine was required in 38 patients （19%）. Multivariate analysis revealed that a cyclosporine-based regimen was significantly associated with *** happen at a low incidence, supporting the safety and efficacy of the present immunosuppression regimen for living donor liver transplantation.

Effects of Gaussian filter in processing GRACE data: Gravity rate of change at Lhasa,southern Tibet

In this paper, the spatial gravity distribution over Tibetan Plateau and the gravity rate of change at Lhasa for different Gaussian filter radii are computed using GRACE data. Results show that the estimate of the gravity rate of change is spatialradius-dependent of the Ganssian filter. The GRACE-estimated gravity rate of change agrees well with the surface measured one. In other words, the GRACE-estimated gravity rate of change has a limited value as that obtained by surface measurement when the spatial filter radius reaches zero. Then numerical simulations are made for different spatial radii of the Gaussian filter to investigate its behaviors when applied to surface signals. Results show that the estimate of a physical signal is filter-radius dependent. If the computing area is equal to or less than the mass area, especially for a uniformly distributed mass, the estimate gives an almost correct result, no matter what filter radius is used. The estimate has large error because of the signal leakage caused by harmonic truncation if the computing area is much bigger than the mass distribution （or inverse for a small mass anomaly）. If a mass anomaly is too small, it is difficult to recover it from space observation unless the filter radius is extremely small. If the computing point （or area） is outside the mass distribution, the estimated result is almost zero, particularly for small filter radii. These properties of the Gaussian filter are helpful in applying GRACE data in different geophysical problems with different spatial position and geometrical size. We further discuss physical sources causing the scalar gravity change at Lhasa. Discussions indicate that the gravity rate of change at Lhasa is not caused by the present-day ice melting （PDIM）（or Little Ice Age, LIA） effect because no ice melting occurs in Lhasa city and nearby. Spinal Epidural Cavernous Hemangioma of the Thoracic Spine: A Case Report

Cavernous hemangiomas can arise in any region of the body, including the central nervous system. Spinal cavernous hemangiomas account for 5% - 12% of all cases of vertebral vascular malformation. Most of these are of vertebral origin, and cases that are non-vertebral in origin are rare. We encountered a patient with a relatively rare spinal epidural cavernous hemangioma of the thoracic spine that was non-vertebral in origin. The patient was a 63-year-old man. He had become aware of bilateral leg pain and numbness about 2 months earlier, and gait disturbance appeared gradually thereafter. On MRI, a lesion showing iso-intensity on T1-weighted imaging and high intensity on T2-weighted imaging was detected at the 7th thoracic vertebra. On gadolinium contrast-imaging, the lesion was found to be a homogenously-enhanced dumbbell-shaped extradural spinal neoplasm protruding from the left 7th/8th thoracic intervertebral foramen. A neurogenic tumor was suspected based on myelography and MRI findings, and complete tumorectomy was performed, which improved the lower limb symptoms and gait disturbance. The histopathological diagnosis was cavernous hemangioma. Epidural hemangiomas arise from the vertebra in many cases, and pure spinal epidural cavernous hemangiomas are rare. It is difficult to make a preoperative diagnosis because there are no specific imaging findings that can differentiate these tumors. It may be important to consider this disease before surgery in the differential diagnosis of epidural tumors.
