A study of the effects of saliva stimulation by nizatidine on dry mouth symptoms of primary biliary cirrhosis

AIM: To elucidate the effect of saliva stimulation by nizatidine on oral symptoms of primary biliary cirrhosis (PBC) by administering it to PBC cases. METHODS: From among 73 cases that had been definitively diagnosed as PBC at our hospital by February 2010, we selected 27 cases of PBC, 4 males and 23 females, as subjects. We obtained subjects' consent after giving them a full explanation of the administration of nizatidine. Nizatidine 150 mg was administered internally twice daily, after morning and evening meals. To observe changes in the quantity of saliva secreted, chewing gum tests were carried out four times: before the initial dose, and after 6 mo, 12 mo and 24 mo of administration. For subjective dry mouth symptoms, a visual analog scale (VAS) method was used to assess their feelings of oral dryness and eating difficulty, five times: before the initial dose, and after 1, 6, 12 and 24 mo of administration in 8 cases. The nutritional condition and the hepatic functional reserve were compared between before and after the nizatidine treatment.RESULTS: The result of a chewing gum test on the subjects before the administration of nizatidine showed that 50% produced less than 10 mL of saliva, i.e. , the standard under which cases are considered to have hyposalivation. The results of these tests showed that the quantity of saliva secreted was 10.5 ± 6.8 mL before administration of nizatidine, 10.9 ± 6.0 mL after 6 mo, 10.6 ± 4.9 mL after 12 mo, and 11.8 ± 6.8 mL after 24 mo administration. Thus, there was a slowly increasing trend in the quantity of saliva in the whole group. The percentage of subjects with saliva production above 10 mL was 45.8% after 6 mo administration of nizatidine, that is, only a slight change from before its administration, but it was 64.3% after 12 mo, that is, a significant increase. The saliva secretion by subject patients was examined before the beginning of administration of nizatidine, 12 mo later, and 24 mo later, and Fisher's combined probability test was used to examine the results

AIM To clarify the association between aldo-keto reductase family 1 member B10(AKR1B10) expression and hepatocarcinogenesis after hepatitis C virus eradication.METHODS In this study,we enrolled 303 chronic hepatitis C patients who had achieved sustained virological response(SVR) through interferon-based antiviral therapy. Pretreatment AKR1B10 expression in the liver was immunohistochemically assessed and quantified as a percentage of positive staining area by using image-analysis software. A multivariate Cox analysis was used to estimate the hazard ratios(HRs) of AKR1B10 expression for hepatocellular carcinoma(HCC) development after achieving SVR. The cumulative incidences of HCC development were evaluated using Kaplan-Meier analysis and the log-rank test.RESULTS Of the 303 chronic hepatitis C patients,153(50.5%) showed scarce hepatic AKR1B10 expression,quantified as 0%,which was similar to the expression in control normal liver tissues. However,the remaining 150 patients(49.5%) exhibited various degrees of AKR1B10 expression in the liver,with a maximal AKR1B10 expression of 73%. During the median follow-up time of 3.6 years(range 1.0-10.0 years),8/303 patients developed HCC. Multivariate analysis revealed that only high AKR1B10 expression(≥ 8%) was an independent risk factor for HCC development(HR = 15.4,95%CI: 1. 8- 1 3 2. 5,P = 0. 0 1 2). T h e 5- y e a r c u m u l a t i v e incidences of HCC development were 13.7% and 0.5% in patients with high and low AKR1B10 expression,respectively(P < 0.001). During the follow-up period after viral eradication,patients expressing high levels of AKR1B10 expressed markedly higher levels of alanine aminotransferase and α-fetoprotein than did patients exhibiting low AKR1B10 expression.CONCLUSION Chronic hepatitis C patients expressing high levels of hepatic AKR1B10 had an increased risk of HCC development even after SVR.

Hepatic vein tumor thrombus(HVTT),together with portal vein tumor thrombus(PVTT),is a major determinant of survival outcome in patients with advanced hepatocellular carcinoma(HCC).HVTT can be an oncologic emergency,especially when it invades beyond IVC into the right atrium,where it can cause sudden death from pulmonary embolism.Clinical studies using large numbers of patients are generally difficult in this relatively rare and urgent disease.In this context,the multicenter study conducted by Chen et al.is significant,albeit retrospective(1).

During the initial stages of software development,the primary goal is to define precise and detailed requirements without concern for software realizations.Security constraints should be introduced then and must be based on the semantic aspects of applications,not on their software architectures,as it is the case in most secure development methodologies.In these stages,we need to identify threats as attacker goals and indicate what conceptual security defenses are needed to thwart these goals,without consideration of implementation details.We can consider the effects of threats on the application assets and try to find ways to stop them.These threats should be controlled with abstract security mechanisms that can be realized by abstract security patterns(ASPs),that include only the core functions of these mechanisms,which must be present in every implementation of them.An abstract security pattern describes a conceptual security mechanism that includes functions able to stop or mitigate a threat or comply with a regulation or institutional policy.We describe here the properties of ASPs and present a detailed example.We relate ASPs to each other and to Security Solution Frames,which describe families of related patterns.We show how to include ASPs to secure an application,as well as how to derive concrete patterns from them.Finally,we discuss their practical value,including their use in“security by design”and IoT systems design.

BACKGROUND Angioedema is a disorder characterized by edema of the face,lips,tongue,and extremities due to increased vascular permeability.Angioedema of the tongue usually occurs bilaterally,and the incidence of unilateral angioedema of the tongue is rare.This study reports a rare case of unilateral angioedema of the tongue with no identifiable cause and repeated recurrence even after discontinuation of an angiotensin-converting enzyme inhibitor.CASE SUMMARY The patient was a 65-year-old woman with pre-existing hypertension and hyperlipidemia and had been receiving 20 mg/d of lisinopril.She was diagnosed with angioedema due to unilateral swelling of the tongue.No airway obstruction was observed,and the symptoms improved following the administration of 0.3 mg of epinephrine intramuscularly and 125 mg of methylprednisolone intravenously.Although lisinopril was discontinued,unilateral angioedema of the tongue continued to recur every 2-5 mo,with the symptoms improving following the administration of prednisolone and an antihistamine.Daily oral administration of 500 mg of tranexamic acid after dinner prevented the recurrence of angioedema.CONCLUSION Careful monitoring and identification of the underlying mechanism play a crucial role in the treatment of angioedema.

BACKGROUND Phosphoglyceride crystal deposition disease(PCDD)is a rare acquired disease in which phospholipid crystals deposit in bone and soft tissue long after surgery,trauma,or repeated injections.CASE SUMMARY A 60-year-old-woman was referred to our department because of multiple abdominal masses after open splenectomy for idiopathic thrombocytopenic purpura 29 years earlier.All the masses showed marked fluorodeoxyglucose(FDG)uptake on ^(18)F-fluorodeoxyglucose positron emission tomography(^(18)F-FDGPET)and were strongly suspected to be malignant tumors.Surgical biopsies were performed,and the abdominal masses were found to be aligned vertically,three in a row,along the tissue layers cut in the patient’s previous surgery.Pathological finding of the specimens showed foreign body granuloma consisting of histiocytes and multinucleated giant cells accumulating around needle-like crystals.The crystals were confirmed as phosphoglyceride by Raman spectroscopy,and PCDD was diagnosed.To our knowledge,this is the first report of PCDD diagnosed by Raman spectroscopy.CONCLUSION We made a definitive diagnosis of PCDD in a patient with multiple tumors showing marked FDG uptake on ^(18)F-FDG-PET by incisional biopsy and composition analysis using Raman spectroscopy,a method that has not previously been reported for the diagnosis of PCDD.

The photoperiodic control of flowering time is essential for the adaptation of plants to variable environments and for successful reproduction. The identification of genes encoding florigens, which had been elusive but were supposedly synthesized in leaves and then transmitted to shoot apices to induce floral transitions, has greatly advanced our understanding of the photoperiodic regulation of flowering. Studies on the photoperiodism of Arabidopsis, a model long-day plant, revealed the molecular mechanisms regulating the expression of the Arabidopsis florigen gene FT, which is gradually induced in response to increase in day length. By contrast, in rice, a model short-day plant, the expression of the florigen gene Hd3a （an FTortholog in rice） is regulated in an on/off fashion, with strong induction under short-day conditions and repression under long-day conditions. This critical day length dependence of Hd3a expression enables rice to recognize a slight change in the photoperiod as a trigger to initiate floral induction. Rice possesses a second florigen gene, RFT1, which can be expressed to induce floral transition under non-inductive long-day conditions. The complex transcriptional regulation of florigen genes and the resulting precise control over flowering time provides rice with the adaptability required for a crop species of increasing global importance.

Bio-based chemical production has drawn attention regarding the realization of a sustainable society.In vitro metabolic engineering is one of the methods used for the bio-based production of value-added chemicals.This method involves the reconstitution of natural or artificial metabolic pathways by assembling purified/semi-purified enzymes in vitro.Enzymes from distinct sources can be combined to construct desired reaction cascades with fewer biological constraints in one vessel,enabling easier pathway design with high modularity.Multiple modules have been designed,built,tested,and improved by different groups for different purpose.In this review,we focus on these in vitro metabolic engineering modules,especially focusing on the carbon metabolism,and present an overview of input modules,output modules,and other modules related to cofactor management.
