In recent years,the serrated neoplasia pathway where serrated polyps arise as a colorectal cancer has gained considerable attention as a new carcinogenic *** serrated polyps are histopathologically classified into hyperplastic polyps(HPs),sessile serrated lesions,and traditional serrated adenomas;in the serrated neoplasia pathway,the latter two are considered to be *** western countries,all colorectal polyps,including serrated polyps,apart from diminutive rectosigmoid HPs are ***,in Asian countries,the treatment strategy for colorectal serrated polyps has remained ***,in this review,we described the clinicopathological features of colorectal serrated polyps and proposed to remove HPs and sessile serrated lesions≥6 mm in size,and traditional serrated adenomas of any size.

AIM： To investigate the histopathological and geneticdifferences between polypoid growth （PG） and nonpolypoid growth （NPG） submucosal invasive colorectal carcinoma （CRC）.METHODS： A total of 96 cases of submucosal CRC were divided into two groups according to their growth type;60 cases of PG and 36 cases of NPG. The size, histological degree of dysplasia, depth of submucosal invasion and lymph node metastasis were compared between the two groups. Furthermore, expression of p53 was detected by immunohistochemical staining, and K-ras gene mutation was examined by polymerase chain reaction based single-strand conformation polymorphism （SSCP）.RESULTS： The average size of the lesions in the NPG group was significantly smaller than those in the PG group （7.5 mm vs 13.8 mm, P 〈 0.001）. The histological degree of dysplasia tended to be more severe in NPG group, while the incidence of submucosal massive invasion and the lymph node metastasis were both significantly higher in the NPG type than in the PG group （64.3% vs 43.3%, P = 0.004; 43% vs 7%, P =0.008, respectively）. In addition, K-ras gene mutations were detected in 67% of lesions in the PG group, but none in the NPG group, while no difference in p53immunohistochemical expression was found between the two groups.CONCLUSION： Compared with PG submucosal CRC,NPG type demonstrates more frequent submucosal massive invasion, more lymph node metastasis and a higher degree dysplasia. Genetically, NPG type shows much less frequent K-ras mutation.

Effective use of the Japan Narrow Band Imaging Expert Team classification based on diagnostic performance and confidence level

Five years have passed since the Japan Narrow Band Imaging Expert Team (JNET) classification was proposed in 2014. However, the diagnostic performance of this classification has not yet been established. We conducted a retrospective study and a systematic search of Medical Literature Analysis and Retrieval System On-Line. There were three retrospective single center studies about the diagnostic performance of this classification. In order to clarify this issue, we reviewed our study and three previous studies. This review revealed the diagnostic performance in regards to three important differentiations.(1) Neoplasia from non-neoplasia;(2) malignant neoplasia from benign neoplasia;and (3) deep submucosal invasive cancer (D-SMC) from other neoplasia. The sensitivity in differentiating neoplasia from non-neoplasia was 98.1%-99.8%. The specificity in differentiating malignant neoplasia from benign neoplasia was 84.7%-98.2% and the specificity in the differentiation D-SMC from other neoplasia was 99.8%-100.0%. This classification would enable endoscopists to identify almost all neoplasia, to appropriately determine whether to perform en bloc resection or not, and to avoid unnecessary surgery. This article is the first review about the diagnostic performance of the JNET classification. Previous reports about the diagnostic performance have all been retrospective single center studies. A large-scale prospective multicenter evaluation study is awaited for the validation.

We review our studies on how to identify the most appropriate models of diseases, and how to determine their parameters in a quantitative manner given a short time series of biomarkers, using intermittent androgen deprivation therapy of prostate cancer as an example. Recently, it has become possible to estimate the specific parameters of individual patients within a reasonable time by employing the information concerning other previous patients as a prior. We discuss the importance of using multiple mathematical methods simultaneously to achieve a solid diagnosis and prognosis in the future practice of personalized medicine.

Target plasmas, on which the formation of the electrostatic potentials and the improvement of the confinement are studied, are produced with ICRF in the GAMMA 10 tandem mirror. The ion temperature of more than 10 keV has been achieved in relatively low density plasmas. When the strong ICRF heating is applied, it is observed that the high frequency and the low frequency fluctuations are excited and suppress the increase of the plasma parameters. Recently, a new high power gyrotron system has been constructed and the ECRH power in plug extends up to 370 kW. The improvement of the confinement due to the formation of the potential in the axial direction and the strong radial electric field shear has been observed.

Granular cell tumor (GCT) of the biliary system is rare. It is reported that it occurs more commonly in young black women. We report here our seldom experience of a Japanese case in whom icterus was found as a first symptom just after a caesarean operation. A 36-year-old Japanese woman developed icterus after delivery by the Caesarean operation. A surgical operation was performed without can deny that there was a tumor-related change in a bile duct as a result of examination for various images. As a result of pathological evaluation, GCT was diagnosed. By the preoperative organization biomicroscopy result, it was not able to be attachd a right diagnosis. It was thought that this tumor, although rare, should be considered as one of the causes of biliary stenosis in the younger population.

Association between colonic polyps and diverticular disease

AIM: To evaluate the association between colonic polyps and diverticular disease in Japan. METHODS: We retrospectively reviewed the medical records of 672 consecutive patients who underwent total colonoscopy between August 2006 and April 2007 at Nishinjo Hospital, Okinawa, Japan. Patients with a history of any of the following were excluded from the study: previous polypectomy, colonic resection, and inflammatory bowel diseases. The association between colonic polyps and diverticular disease was analyzed by logistic regression analysis, adjusted for age and sex. RESULTS: Prevalence of colonic polyps in all patients with diverticular disease was significantly higher than that in those without diverticular disease (adjusted odds ratio 1.7). CONCLUSION: Our data showed that patients with diverticular disease have a higher risk of colonic polyps compared to those without.

Effect of Miraculin on Sweet and Sour Tastes Evoked by Mixed Acid Solutions

Miraculin is a glycoprotein, and it changes the perceived quality and intensity of tastes (especially, sourness and sweetness). The oral application of miraculin in a high concentration elicits a strong sweetness for sour tastants. However, the mixing of different tastants increases or decreases the intensity of the taste elicited in comparison with the intensities elicited by the individual sour tastants. The synergy between different (e.g., sweetness and sourness) or similar taste qualities can be responsible for the changes in the perceived taste intensities. Despite the characteristics of miraculin and the sour tastant mixtures, the effects of miraculin on mixed acid solutions are still unknown. The goals of this study were to assess the following: 1) the intensities of the sourness and sweetness of mixed acid solutions before and after the oral application of miraculin and 2) the intensity of the sweetness of the mixed acid solutions after the oral application of miraculin. For twenty healthy young adults, sensory evaluation experiments examined the perceived intensities of taste (sweetness, sourness, and astringency) and the overall odor of six organic acid solutions (2.3 × 10–2 M of citric acid and three-binary and two-trinary mixtures including citric acid that ranged from 2.24 to 2.48 in average pH). The application of miraculin elicited sufficient sweetness, but it did not show statistically significant differences in the intensities of sweetness among the six acid solutions. Except for the sweetness, all of the four sensory items exhibited significant changes among the six solutions both before and after the application of miraculin, while the average scores for these items did not correlate with the average solution pH values. These results suggest that the binary and trinary mixed acid solutions affect the sourness both before and after the oral application of miraculan;however, there was no effect on the sweetness.
