Comprehensive review of post-liver resection surgical complications and a new universal classification and grading system

Liver resection is the gold standard treatment for certain liver tumors such as hepatocellular carcinoma and metastatic liver tumors. Some patients with such tumors already have reduced liver function due to chronic hepatitis, liver cirrhosis, or chemotherapy-associated steatohepatitis before surgery. Therefore, complications due to poor liver function are inevitable after liver resection. Although the mortality rate of liver resection has been reduced to a few percent in recent case series, its overall morbidity rate is reported to range from 4.1% to 47.7%. The large degree of variation in the post-liver resection morbidity rates reported in previous studies might be due to the lack.of consen-sus regarding the definitions and classification of postliver resection complications. The Clavien-Dindo(CD) classification of post-operative complications is widely accepted internationally. However, it is hard to apply to some major post-liver resection complications because the consensus definitions and grading systems for posthepatectomy liver failure and bile leak.ge established by the International Study Group of Liver Surgery are incompatible with the CD classification. Therefore, a unified classification of post-liver resection complications has to be established to allow comparisons between academic reports.

Back.round: the physical integration of South America through a Bioceanic Corridor has been idealized for a long time by the countries that mak. up Mercosur. This integration will have impacts on the population residing in the cities where the route will be built. Among them, Porto Murtinho, southern Mato Grosso on the border with Paraguay, stands out, where a bridge will be built over the Paraguay River that will link.these two countries. Methods: a retrospective, quantitative and cross-sectional study of a descriptive type of documentary approach that sought to analyze the prevalence of arboviruses, Sexually Transmitted Infections (STIs) and Diseases Related to Inadequate Environmental Sanitation (DRIES) notified and stored in the National System of Notifiable Diseases (SINAN). The sample consisted of new cases of these pathologies in the municipality of Porto Murtinho registered between January 2017 and December 2020. Results: there was an increase of 10% in the total number of diseases within this period. The number of dengue cases increased 36 times;the incidence of Syphilis, HIV and HPV decreased;and, finally, maintenance of cases of Gonorrhea, Leptospirosis, Yellow Fever, Zik., Chik.ngunya, Hepatitis A and B. Conclusions: actions are needed to prevent the increase of these pathologies together with the construction of the Bioceanic Route, thus avoiding damage population health and increased consumption of government resources.

AIM:To investigate the clinical and biochemical factors associated with visceral fat accumulation in the general ***:We enrolled 1004 subjects who underwent a medical health check.p between April 2008 and March *** medical health check.p included the following tests:Height,body weight,waist circumference(WC),systolic blood pressure,diastolic blood pressure,urinalysis,blood-cell counts,blood chemistry,electrocardiography,chest radiography,and abdominal computed tomography(CT)for visceral fat *** patients’medical history and lifestyle factors were collected privately by nurses using a selfadministered questionnaire,and they included questions regarding physical activity,sleep duration,dietary habits,smok.ng,and alcohol *** fat area(VFA)was defined as the sum of the intraperitoneal fat area at the level of the umbilicus with CT density in the range of-150 to-50 Hounsfield ***:The mean age and body mass index(BMI)of the study subjects were 57.0 years and 24.4 k./*** both male and females,v FA was significantly andpositively correlated with WC(r=0.532,Pk./m2,and triglyceride level≥149 in males(Pk./m2,high-density lipoprotein cholesterol levelk.ng in females(P<0.05).CONCLUSION:These results suggest that gender differences exist in the clinical and biochemical parameters associated with visceral fat accumulation.

AIM: To describe the anal cushion lifting(ACL) method with preliminary clinical results. METHODS: Between January to September 2007, 127 patients who received ACL method for hemorrhoid was investigated with informed consent. In this study, three surgeons who specialized in anorectal surgery performed the procedures. Patients with grade two or more severe hemorrhoids according to Goligher's classification were considered to be indicated for surgery. The patients were given the choice to undergo either the ACL method or theligation and excision method. ACL method is an original technique for managing hemorrhoids without excision. After dissecting the anal cushion from the internal sphincter muscle, the anal cushion was lifted to oral side and ligated at the proper position. Clinical characteristics and outcomes of patients were recorded including complications after surgery. RESULTS: A total of 127 patients were enrolled. Their median age was 42(19-84) years, and 74.8% were female. In addition, more than 99% of the patients had grade 3 or worse hemorrhoids. The median followup period was 26(0-88) mo, and the median operative time was 15(4-30) min. After surgery, analgesics were used for a median period of three days(0-21). Pain control was achieved using extra-oral analgesic drugs, although some patients required intravenous injections of analgesic drugs. The median duration of the patients' postoperative hospital stay was 7(2-13) d. A total of 10 complications(7.9%) occurred. Bleeding was observed in one patient and was successfully controlled with manual compression. Urinary retention occurred in 6 patients, but it disappeared spontaneously in all cases. Recurrent hemorrhoids developed in 3 patients after 36, 47, and 61 mo, respectively. No anal stenosis or persistent anal pain occurred. CONCLUSION: We consider that the ACL method might be better than all other current methods for managing hemorrhoids.

From April to July 2018,a data sample at the peak.energy of the T(4 S) resonance was collected with the Belle Ⅱ detector at the Superk.k. electron-positron *** is the first data sample of the Belle Ⅱ *** Bhabha and digamma events,we measure the integrated luminosity of the data sample to be(496.3±0.3±3.0) pb-1,where the first uncertainty is statistical and the second is *** work.provides a basis for future luminosity measurements at Belle Ⅱ.

Melanonychia striata represents a diagnostic dilemma for dermatologists. The use of dermoscopy to assess the nail has advantages over clinical examination. However, when compared to sk.n lesions, it gives fewer details. We describe two cases of melanonychia striata submitted to dermoscopic examination of the nail bed and matrix. This is a new procedure that enables observing dermoscopic characteristics that are not visualized in the nail plate, thus, providing additional information.

Vasculitis is an inflammation of vessel walls,followed by alteration of the blood flow and damage to the dependent *** can cause local or diffuse pathologic changes in the gastrointestinal (GI) *** variety of GI lesions includes ulcer,submucosal edema,hemorrhage,paralytic ileus,mesenteric ischemia,bowel obstruction,and life-threatening *** endoscopic and radiographic features of GI involvement in vasculitisare reviewed with the emphasis on small-vessel vasculitis by presenting our typicalcases,including Churg-Strauss syndrome,HenochSch nlein purpura,systemic lupus erythematosus,and Beh et's *** endoscopic features are ischemic enterocolitis and *** computed tomographic findings include bowel wall thick.ning with the target sign and engorgement of mesenteric vessels with comb *** of endoscopic and radiographic GI manifestations can help mak. an early diagnosis and establish treatment strategy.
