Objective： The complexity, heterogeneity and capacity of malignant neoplastic cells and tumors for rapid change and evolution suggest that living-cell-based biological-systems approaches to cancer treatment are merited. Testing this hypothesis, the tumor marker, metabolic activity, and overall survival（OS） responses, to the use of one such system, implantable macrobeads [RENCA macrobeads（RMBs）], in phase I and IIa clinical trials in advanced,treatment-resistant metastatic colorectal cancer（m CRC） are described ***： Forty-eight m CRC patients（30 females; 18 males）, who had failed all available, approved treatments,underwent RMB implantation（8 RMB/kg body weight） up to 4 times in phase I and phase IIa open-label ***, labs [tumor and inflammation markers, lactate dehydrogenase（LDh）] and positron emission tomography-computed tomography（PET-CT） imaging to measure number/volume and metabolic activity of the tumors were performed pre-and 3-month-post-implantation to evaluate safety and initial efficacy（as defined by biological responses）. PET-CT maximum standard uptake value（SUVmax）（baseline and d 90; SUVmax ≥2.5）, LDh,and carcinoembryonic antigen（CEA） and/or cancer antigen 19-9（CA 19-9） response（baseline, d 30 and/or d 60）were assessed and compared to ***： Responses after implantation were characterized by an at least 20% decrease in CEA and/or CA 19-9 in75% of patients. Fluorodeoxyglucose（FDG）-positive lesions（phase I, 39; 2 a, 82） were detected in 37/48 evaluable patients, with 35% stable volume and stable or decreased SUV（10） plus four with necrosis; 10, increased tumor volume, SUV. LDh levels remained stable and low in Responders（R）（d 0–60, 290.4–333.9）, but increased steadily in Non-responders（NR）（d 0–60, 382.8–1,278.5）（d 60, P=0.050）. Responders to RMBs, indicated by the changes in the above markers, correlated with OS（R mean OS=10.76 months; NR mean OS=4.9 months; P=0.0006）.Conclusions： The correlations of the tumor marker, tumor volume

Total laparoscopic removal of accessory gallbladder:A case report and review of literature

Accessory gallbladder is a rare congenital anomaly occurring in 1 in 4000 births, that is not associated with any specific symptoms. Usually this cannot be diagnosed on ultrasonography and hence they are usually not diagnosed preoperatively. Removal of the accessory gallbladder is necessary to avoid recurrence of symptoms. h-type accessory gallbladder is a rare anomaly. Once identified intra-operatively during laparoscopic cholecystectomy, the surgery is usually converted to open. By using the main gallbladder for liver traction and doing a dome down technique for the accessory gallbladder, we were able to perform the double cholecystectomy with intra-operative cholangiogram laparoscopically. Laparoscopic cholecystectomy was performed in 27-year-old male for biliary colic. Prior imaging with computer tomography-scan and ultrasound did not show a duplicated gallbladder. Intraoperatively after ligation of cystic artery and duct an additional structure was seen on its medial aspect. Intraoperative cholangiogram confirmed the patency of intra-hepatic and extra-hepatic biliary ducts. Subsequent dissection around this structure revealed a second gallbladder with cystic duct(h-type). Pathological analysis confirmed the presence of two gallbladders with features of chronic cholecystitis. It is important to use cholangiogram to identify structural anomalies and avoid complications.

Innovative Approach to Increase Name Plate Capacity of Oil and Gas Gathering Centre

Purpose： The oil and gas gathering and processing facility of Kuwait Oil Company is built with a nameplate capacity of X MBOPD （thousand barrels oil per day） with 50% water cut. however, the facility was operating with a water cut of 35%. This comprehensive technical study was conducted to evaluate possibility of increasing oil processing capacity of this facility in line with current lower water cut and other operational flexibilities available in the facility without utilizing its design margin. Topic： This paper shares an innovative approach to increase name plate capacity of oil and gas processing facility utilizing available operational flexibility and operational margins with minor modification. It shares a case study where facility capacity is increased by around 19% without utilizing design margins of equipment or pipeline. Method： The study includes theoretical verification and analysis of all major equipment and piping to identify available capacity and limitation, in order to utilize available additional margin and to propose debottleneck options to overcome limitations. Achievement： The study confirmed that, facility name plate capacity can be revised from X MBOPD （with 50% w.c （water cut）） to X ＋ 32 MBOPD （with： 45% w.c） minor modification in separator and utilizing margin available in feed specification ofdesalter trains.

Cr-free bi-metallic SBA-15-supported Co–Cu catalysts were examined in the conversion of biomass-derived α-, β-unsaturated aldehyde(furfural) to value-added chemical furfuryl alcohol(FOL).Co–Cu/SBA-15 catalysts with a fixed Cu loading of 10 wt% and varying Co loadings(2.5, 5, and 10 wt%) were prepared by the impregnation method. The catalysts were characterized by X-ray diffraction, N2 sorption, h2 temperature-programmed reduction, scanning electron microscopy, energy-dispersive X-ray spectroscopy, high-resolution transmission electron microscopy, CO chemisorption, and inductively coupled plasma mass spectrometry. The influence of different reaction parameters such as temperature, pressure, catalyst dosage, and furfural concentration on the catalyst performance was evaluated. Relative to catalysts supported on amorphous silica, the current SBA-15-supported Co–Cu catalysts displayed higher performance, attaining a furfural conversion of 99% and furfuryl alcohol selectivity of 80%. The catalytic reactions were conducted in a 100-mL autoclave at 170 °C and 2 MPa h2 pressure for 4 h.

CD5 is constitutively expressed on T cells and a subset of mature normal and leukemic B cells in patients with chronic lymphocytic leukemia(CLL).Important functional properties are associated with CD5 expression in B cells,including signal transducer and activator of transcription 3 activation,IL-10 production and the promotion of B-lymphocyte survival and ***,the pathway(s)by which CD5 influences the biology of B cells and its dependence on B-cell receptor(BCR)co-signaling remain *** this study,we show that CD5 expression activates a number of important signaling pathways,including Erk1/2,leading to IL-10 production through a novel pathway independent of BCR *** pathway is dependent on extracellular calcium(Ca2+)entry facilitated by upregulation of the transient receptor potential channel 1(TRPC1)*** also show that Erk1/2 activation in a subgroup of CLL patients is associated with TRPC1 *** this subgroup of CLL patients,small inhibitory RNA(siRNA)for CD5 reduces TRPC1 ***,siRNAs for CD5 or for TRPC1 inhibit IL-10 *** findings provide new insights into the role of CD5 in B-cell biology in health and disease and could pave the way for new treatment strategies for patients with B-CLL.
